Per the original scope of work,Cardno Inc.performed a thorough Bridge Inspection Report(BIR)for Structure No.030184. Findings in the Consultant's SIR indicated that the depth measured from the left bridge rail at Pier 13 during the 2017 Bridge Inspection may have exceeded the scour critical depth by 1.3 feet.The concern was that the original revetment system,which was installed to protect the pier against scour,may have been moved by Hurricane Irma. • On Change Order No.1,the Consultant was asked to prepare an assessment detailing the findings of the inspection and the condition of this scour countermeasure system for a not-to-exceed fee of$18,049.16 and 72 calendar days. Per the rationale of Change Order No.1,Change Order No.2 would be processed once the results of the proposed assesment were known. Per the attached Exhibit A-Scope of Services for Change Order No.2,the Consultant will design scour countermeasures,as well as provide permitting services required for said scour measures,for a not-to-exceed amount of$67,973.32 and an additional 365 days. 2. The scope described above was not included In the original contract as the potential conditions described above were not known prior to the BIR performed by the Consultant. 3. Due to concerns with the scour countermeasure system including potential shifting due to Hurricane Irma, the CONSULTANT provided further investigation and evaluation of the Scour Countermeasures at Piers 6, 7, 8, 9, 12, 13, 14, 15, 16. Based on recommendations from the investigation and evaluation, this scope includes the design of scour countermeasures as well as the additional permitting services required for the scour countermeasures into the CONSULTANT'S "Stan Gober Memorial Bridge Repair Assessment Report". A-3 1 6A22 6a DRAINAGE ANALYSIS 6a.17 Technical Special Provisions The CONSULTANT will develop technical special provisions for the scour countermeasure repairs. 6a.18 Other Drainage Analysis Data Collection The CONSULTANT will collect information and data necessary to perform the study including bathymetric data, topographic maps, bridge inspection reports, scour evaluation reports, and bridge plans. Also during this task, The CONSULTANT will compile the design hydraulic conditions at the site from the existing modeling. This task will not include a site visit. Recommendations Regarding Existing Protection The CONSULTANT will develop recommendations regarding rehabilitation, augmentation, or removal of the existing protection. Scour Countermeasure Design The CONSULTANT will develop the conceptual design for the scour protection. The CONSULTANT shall copy in the Project Manager on all permit related correspondence and meetings. 8.2 Field Work The CONSULTANT shall perform the following Environmental Field Work. 8.2.2 Establish Wetland Jurisdictional Lines and Assessments Wetland jurisdictional lines will not be established because all maintenance work to be performed is over water. Submerged Aquatic Vegetation (SAV) survey and Essential Fish Habitat(EFH) assessment will be performed in the vicinity of the scour protection system only, for scour protection system repair. SAV/EFH assessment/substrate assessment for contractor barge access clearance is not included. 8.2.3 Species Survey The CONSULTANT shall perform preliminary species surveys. 8.3 Agency Verification of Wetland Data The CONSULTANT shall perform field visit with both agencies, at the same time, and shall submit required follow-up information. 8.4 Complete and Submit All Required Permit Applications The CONSULTANT shall perform the following work as part of the Environmental Permit Application process. 8.4.1 Complete and Submit All Required Wetland Permit Applications The CONSULTANT shall complete and submit all required wetland permit applications. This scope of work assumes a SFMWD Exemption Verification or General Permit for bridge maintenance, and a USAGE Nationwide Permit. The scope includes completing and submitting permit applications, including the Uniform Mitigation Assessment Method (UMAM), assessing impacts, compiling A-5 16A22 -- Environmental Resource Permit (ERP) Environmental Support document, and assisting with responses to Requests for Additional Information. This scope assumes no permanent wetland impacts and no mitigation will be required. Cardno, Inc. requested preparation of this Scope of Services from INTERA Incorporated. The purpose of this Scope of Services is to provide recommendations regarding the existing scour protection and to develop to design scour countermeasures for several of the interior bents. Assumptions We based our scope of services on the following assumptions: 1. Cardno will provide 1NTERA with survey data (bathymetric and topographic) from the bridge location. 2. Cardno will provide INTERA with plans of the existing bridge. 3. Design will rely upon existing hydraulic modeling of the area contained in the Phase 2 Scour Evaluation Report for the bridge. Scope of Services Task I Data Collection INTERA will collect information and data necessary to perform the study including bathymetric data, topographic maps, bridge inspection reports, scour evaluation reports, and bridge plans. Also during this task, INTERA will compile the design hydraulic conditions at the site from the existing modeling. This task will not include a site visit. Task 2 Recommendations Regarding Existing Protection As part of the rehabilitation project,Cardno indicated that an underwater inspection will occur to evaluate the condition of the existing scour protection. Based on the results of this inspection, INTERA will develop recommendations regarding rehabilitation,augmentation,or removal of the existing protection. Task 3 Scour Countermeasure Design INTERA will develop the conceptual design for the scour protection. This includes defining the horizontal and vertical extents, specifying the type of material (e.g. sizing rubble riprap), and incorporating any design constraints. INTERA will document all methods, procedures, and results in a letter report. Additionally, as pall of this task, INTERA will review the construction plans and specifications developed by Cardno to ensure that the final drawings reflect the conceptual design as well as to address any variances as they arise. Task 4 Quality Control Review INTERA will conduct a complete internal review before submission to the Cardno for its review and comment.
