7. In most cases(some contracts are an exception),the original document and this routing slip CB
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!

MODIFICATION#1 TO SUBGRANT AGREEMENT BETWEEN
THE DIVISION OF EMERGENCY MANAGEMENT
AND
COLLIER COUNTY
This Modification is made and entered into by and between the State of Florida,
Division of Emergency Management ("the Division"), and Collier County ("Sub-
Recipient"), to modify Agreement Number G0093, which began on July 1, 2020 ("the
Agreement").
WHEREAS, the Division and the Sub-Recipient have entered into the Agreement,
pursuant to which the Division has provided a subgrant to Sub-Recipient under the
Emergency Management Performance Grant program of$110,834.00;
WHEREAS, the Agreement will expire on June 30, 2021; and,
WHEREAS, the Division and the Sub-Recipient desire to extend the terms of the
Agreement; and
WHEREAS,the Division and the Sub-Recipient desire to modify the Agreement;
and,
WHEREAS, in consideration of the mutuat promises of the parties contained herein,
the parties agree as follows:
1. Paragraph (3) PERIOD OF AGREEMENT hereby amended to read as follows:
(a)This agreement period will begin on July 01, 2020, and will end on September
30, 2021, unless terminated earlier in accordance with provisions of this
Agreement, including, but riot limited to Paragraph (12) TERMINATION, of
this Agreement.
2, All provisions not in conflict with this Modification remain in full force and effect,
and are to be performed at the level specified in the Agreement.

the dates set out below.
