EXECUTIVE SUMMARY
Recommendation to approve Request for Quotation # 20-8110, “Immokalee Health and Library
Parking Lot,” under Agreement No. 16-6663, Annual Agreement for Roadway Contractors to
Bonness, Inc., authorize a budget amendment and the issuance of a purchase order in the amount
of $267,623.
OBJECTIVE: To fully restore the parking lot and entrances serving the Immokalee Health Department
and Immokalee Library and enhance the drainage system on the property to effectively manage
stormwater.
CONSIDERATIONS: The Immokalee Health Department Building and Immokalee Library are located
at the Southwest corner of Immokalee Drive and North 1st Street in Immokalee, Florida. The two
buildings share a parking lot and set of entrances that also serve the Immokalee satellite office of the
David Lawrence Center.
The site and parking lot are in need of a full restoration and upgrades to the stormwater drainage system
on the property. This project seeks to fully restore the parking lot and entrances, enhance the site’s ability
to manage stormwater and increase the accessibility of the site for disabled persons.
On November 23, 2020, utilizing Agreement No. 16-6663, Roadway Contractors Annual Agreement, the
Facilities Management Division requested quotes for the “Immokalee Health and Library Parking Lot”
project. Staff sent the request for quotes to the four contractors under contract with the County under this
agreement. Four (4) contractors submitted quotations by the December 9, 2020 deadline. Considering
available budgets, staff subsequently requested that the contractors resubmit bids broken out by project
area in order to move forward with the area of the site most utilized and in need of restoration. This final
bid tabulation is shown below:

The lowest bid for this project was above the original opinion of probable cost provided by project
consultant Agnoli Barber & Brundage (ABB). After reviewing the submitted bids, considering
fluctuations in the labor and materials market and consulting with ABB, staff determined the lowest quote
by the Bonness, Inc. is fair and reasonable and seeks to move forward with the base scope of the project
with an owner’s allowance of $15,000 for a total of $267,623.
Bonness, Inc. has been in business for over 30 years in Collier County and has worked for the County on
various continuing service contracts and large projects.
FISCAL IMPACT: A budget amendment in the amount of $272,623 is required to reallocate funding
from within the County-Wide Capital Project Fund (301). The budget amendment includes the amounts
needed for the purchase order ($267,263) and permitting expense ($5,000 estimated). The source of
funding is a transfer from the General Fund (001).

INTRODUCTION
Collier County Board of County Commissioners Facilities Management Division, (hereinafter, “County”) has issued this Request for
Quote (hereinafter, “RFQ”) with the intent of obtaining bid submittals listed vendors on the 16-6663 contract in accordance with the
terms, conditions and specifications stated or attached. The vendor, at a minimum, must achieve the requirements of the Specifications
or Scope of Work stated.
BACKGROUND
The goal of this project is to fully restore the parking lot at Immokalee Health Facility in accordance with the accompanying construction
documents by Agnoli, Barber & Brundage, Inc. The below scope and attachments detail the areas to be repaired, provides documents
that illustrate the existing conditions.
TERM OF CONTRACT
The total contract term, if an award is made, is intended to be one hundred-twenty (120) calendar days.
Prices shall remain firm for the initial term of this contract and surcharges will not be accepted in conjunction with this contract. Such
charges should be incorporated into the pricing structure.
All goods are FOB destination and must be suitably packed and prepared to secure the lowest transportation rates and to comply with
all carrier regulations. Risk of loss of any goods sold hereunder shall transfer to the COUNTY at the time and place of delivery, provided
that risk of loss prior to actual receipt of the goods by the COUNTY nonetheless remain with VENDOR.
AWARD CRITERIA
RFQ award criteria are as follows:
For the purposes of determining the winning bidder, the County will select the vendor with the lowest price as outlined below:

Collier County reserves the right to select one, or more than one supplier, award on a line item basis, establish a pool for
quoting, or other options that represents the best value to the County; however, it is the intent to:
• Identify a Single Awardee
The County reserves the right to issue a formal contract or standard County Purchase Order or utilize the County Purchasing
Card for the award of this solicitation.

DETAILED SCOPE OF WORK
1. REQUIREMENTS:
1.1 All work must comply with Collier County Vertical Standards (CCVS) and the Florida Building Code.
1.2 Provide all necessary items, tools, hardware, resources, materials, etc. to complete all project tasks fully and properly.
Install all items per manufacturer’s recommendations and installation instructions.
1.3 Collier County has the right to request that any materials and items being removed or replaced be turned over to the
County.
1.4 Provide all necessary permitting and any documentation, sketches, fees, etc. required for permits. The selected
vendor may utilize owner’s as-built drawings, if available, to complete project.
1.5 The contractor will be required to provide photographs of the progress and successful completion of the work.
1.1 The contractor shall submit specifications, product data and/or shop drawings on all materials, devices, equipment, or
systems being installed on the project for approval by the Project Manager and/or Project Consultant.
1.6 The contractor must warranty the work completed for a minimum of one (1) year following the completion of this
project. Any call-back or remediation work will be completed at no charge to Collier County.
1.7 All work must be performed by properly qualified employees. Credentials may be requested by Collier County.
1.8 The contractor and their subcontractors are required to comply with County Ordinance 2004-52, as amended.
Background checks are valid for five (5) years and the vendor / Subcontractor shall be responsible for all associated
costs. The cost is $40.00 per person for new badges.
1.9 The awarded Contractor will receive a Purchase Order for work requested and may not start work prior to receiving
the Notice to Proceed (NTP) authorization to begin the work.
1.10 Contractor is responsible for blocking off and maintaining any safety perimeters necessary for work. Contractor
shall properly block off construction areas as appropriate until the work item is complete.
1.11 The objective of this project is to restore and repave the referenced site and to improve drainage.
1.12 Any substitution requests need to be added to the Q&A process prior to the close of the RFQ. Quantities listed in
this RFQ are approximate. The Contractor is responsible for completing the work 100% in order to meet the intent
of the project.
1.13 The contractor is expected to be familiar with the standards of the Americans with Disabilities Act (ADA) and
Florida Building Code Accessibility Standards and ensure all work completed is in accordance with these codes and
standards.
2. BASE SCOPE:
The overall intent of this project is to repair, improve drainage, and repave the Immokalee Health Facility parking lot including
ADA improvements and sidewalks in accordance with the attached documents.
Quantities are approximate. Contractor shall include all work necessary to complete the intent of the work as shown
on the construction documents.
The work includes but is not limited to the following:
Mill and remove existing pavement (approximately 11,568 Sq. Yds.) to ensure proper slope and tie into existing

areas, temporary entrances, etc. Ensure area is fully clean and ready for use prior to turnover.

3.2 Include any necessary repairs to adjacent sidewalks or curbs damaged by construction activity. Regrading and sod
placement where necessary and replacement or reinstallation of any improvements, signage, bike racks, parking
stops, markings, etc. affected by the work.
3.3 Schedule / Liquidated Damages / Owner Access
3.3.1 The project shall be substantially completed within ninety (90) days of receiving the authorization to start the
work.
3.3.2 The selected Contractor shall present the owner, within seven (7) days of award, a project work schedule to
ensure smooth coordination with building occupants and other County operations.
3.3.3 The selected Contractor shall coordinate activities with Project Manager and on-site staff to ensure continuous
operations. The contractor may choose to sequence the work, complete certain areas during off hours or any
combination of these methods to ensure uninterrupted use of the facilities on the property.
3.3.4 Should the Contractor fail to substantially complete the project in the time period specified, the owner shall be
entitled to assess, as liquidated damages, but not as a penalty, one thousand two-hundred forty-one dollars
($1,241) per calendar day.
