5. The Chairman's signature line date has been entered as the date of BCC approval of the MC
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip N/A
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 1/12/2021 and all changes made MC
during the meeting have been incorporated in the attached document. The County
Attorney's Office has reviewed the changes,if applicable.

1. Duo to the current termination point of the bank stabilization product In the middle of a residential block,we are requesting a
change order to increase the linear feet of stormwater ditch bank improvements by approximately 600 linear feet so that the
pilot project extends from Pompel Lane to Granada Blvd,
2, In order to reduce project cost,the original scope of the proJoot was downsizod,however,the ease of Installation,appearance
of the abrupt termination point in the middle of the block end terminiation point with the existing unimproved bank were not
anticipated.
3. If this change order is not processed the contractor with HydroTurfZ installation experience will demobilize end terminate the
Installation of the bank stabilization project In the middle of the residential block.By approving this change order we are saving
the cost of future mobilization when the remaining section Is Improved.
