6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's LM
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip LM
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 04-28-2020 and all changes made
during the meeting have been incorporated in the attached document. The County
Attorney's Office has reviewed the changes,if applicable.
9. Initials of attorney verifying that the attached document is the version approved by the
BCC,all changes directed by the BCC have been made,and the document is ready for the
Chairman's signature.

TERMINATION OF EASEMENT
WHEREAS, the BOARD OF COUNTY COMMISSIONERS OF COLLIER
COUNTY, FLORIDA, AS THE GOVERNING BODY OF COLLIER COUNTY, AND AS
THE EX-OFFICIO GOVERNING BOARD OF THE COLLIER COUNTY WATER-
SEWER DISTRICT, ("Easement Holder") is the owner of record of that certain Deed of
Utility Easement, ("Easement") dated February 11, 2019, and recorded on March 27,
2019, in Official Records Book 5611, Page 1668, of the Public Records of Collier
County, Florida.
WHEREAS, the Easement Area is located on land owned by NATIONAL
RETAIL PROPERTIES, LP, a Delaware limited partnership, ("Grantor"), and more
particularly described in Exhibit "A," attached;
WHEREAS, the Grantor of the Easement granted said Easement to Easement
Holder in connection with the conveyance of utility facilities;
WHEREAS, the Easement should never have been conveyed, as said utility
facilities are to be owned and maintained by Grantor;
WHEREAS, the Easement Holder did not accept the Easement;
WHEREAS, the Easement Holder desires to formally terminate the Easement;
and
WHEREAS, the Easement Holder and Grantor are agreeable to terminating the
Easement.
NOW, THEREFORE, in consideration of TEN DOLLARS ($10.00) and other
valuable consideration, receipt of which is hereby acknowledged, the Easement Holder
does hereby relinquish, abandon, and terminate its interest in the Easement and
releases the Easement, effective upon the recording hereof in the Public Records of
Collier County, Florida.
