A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, AUTHORIZING ITS CHAIRMAN TO
EXECUTE A FLORIDA HIGHWAY DEPARTMENT OF
TRANSPORTATION BEAUTIFICATION GRANT.
WHEREAS, many roadside areas and median strips within Department of
Transportation rights of way must be maintained and attractively landscaped;and
WHEREAS, the Board of County Commissioners for Collier County, Florida (the
'Board")beautify and improve various rights of way by landscaping within Collier County;and
WHEREAS, the Board wishes to approve and authorize its Chairman to execute a
Florida Department of Transportation Beautification Grant.
NOW,THEREFORE,BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA that:
Section 1. The Board hereby authorizes its Chairman to execute a Florida Department of
Transportation Beautification Grant (Form 650-050-10).
Section 2. The Clerk of Courts is hereby directed to send a copy of this Resolution to the
Department of Transportation along with the executed Florida Department of Transportation
Beautification Grant .
PASSED AND DULY ADOPTED by the Board of County Commissioners of Collier
County, Florida, after motion, second, and majority vote favoring same this on this 424-1'1day

Part I
GENERAL INSTRUCTIONS: Statutory requirements for the Beautification Grant Program are found in Section 339.2405,Florida
Statutes. Within Rule Chapter 14-40,Florida Administrative Code,are sections dealing with the grant application process,grant award
process, and funding, construction, and maintenance of beautification projects. These documents can be reviewed on the FDOT
website,jitto://www.fdot.Rov/designsupport/highwavbeautification/.
When a decision is made to apply for a Beautification Grant,please immediately notify the District Grant Coordinator. A directory of
Grant Coordinators is posted on the program website. The Grant Coordinator will provide guidance, and help ensure that no
Department projects are in conflict with the grant project. Grant applications for landscape projects in conflict with other Department
projects will not be accepted.
SUBMISSION: Please complete all items in this application. A complete grant application includes digital copies of the following:
® 1)One illustrative plan proposal.The Illustrative proposal can include existing site photos,sketches or renderings,and
any narrative and labels to accurately describe the project.The plan must be legible when printed at 11"x17"size.Only one
page will be accepted.
® 2)Form 650-050-10,Application Form,Part I
® 3)Form 650-050-10,Application Form,Part II(Submit a pdf of the excel document)
® 4)A copy of the signed resolution
® 5)Letter of Authorization from the Department
® 6)Optional letters of support
To be eligible for a grant,digital copies of the above 6 items are to be submitted online by October 1,5pm EST.(If October 1 falls on
a weekend or holiday,the deadline will be the next following business day.) Late or incomplete applications,or any addendums or •
changes will not be accepted.
Grant applicants should contact the District Grant Coordinator at least one week prior to the deadline to gain access to the online
system that will accept the online submission of applications.
•
DIGITAL FILE ORGANIZATION: Include only the 6 files listed above in the online submission. If multiple pages are to be included,
package the files together into one file. For instance, if multiple letters of support are to be submitted, scan and bundle all of the

Dear Pam,
The Collier County has requested authorization from the Department to proceed with a Beautification
Grant Application for the year of 2021 for the proposed landscape improvements for SR-84 (Roadway ID
#03001000)are from Mile Post 0.000(at the US 41 intersection)to Mile Post 1.005 (at the Airport
Pulling Road intersection).
Upon examination of District One's five year Work Plan we have found no proposed FDOT projects,
roadway or landscape,that will conflict with the proposed beautification project.
This Letter of Authorization allows Collier County to proceed with their Beautification Grant submittal
for the year of 2021. Please retain a copy for your records. A copy of this letter will be required to be
submitted with your grant application.

Dear Mr. Richard,
On the behalf of Collier County Road Maintenance Division (CCRMD), I am requesting for you to provide an authorization
from FDOT for us to proceed with the application process to qualify for a 2021 Beautification Grant. If awarded, the
Beautification Grant will assist with funding for the proposed landscape improvements on Davis Boulevard (SR-84) from
US 41 to Airport Pulling Road that will be delineated in the formal application. The landscape improvements project limits
for SR-84 (Roadway ID#03001000)are from Mile Post 0.000 (at the US 41 intersection)to Mile Post 1.005 (at the Airport
Pulling Road intersection).
CCRMD humbly requests for you to please review the District One's Five-Year Work Plan as to identify any potential
conflicts with our proposed landscape beautification project.
Upon your determination of'no conflict',please at your earliest convenience issue CCRM D a formal Letter of Authorization
allowing us to proceed with the abovementioned project's FDOT Beautification Grant Application process for the fiscal
