FIRST AMENDMENT TO AGREEMENT No. 19-7500
FOR
SENIOR FOOD PROGRAM
THIS AMENDMENT, made and entered into on this (date), by and
between G.A. Food Services of Pinellas County, Inc. d/b/a G.A. Food ervice, Inc. (the "Contractor")
and Collier County, a political subdivision of the State of Florida, (the "County"):
WHEREAS, on May 14, 2019 (Agenda Item No. 16.D.6), the County entered into an
Agreement with Contractor for home delivered and congregate meal services for the Senior Nutrition
Program; and
WHEREAS, the parties desire to amend the Agreement to modify Exhibit A -Scope of
Services and Exhibit B-Fee Schedule to: 1) include a 10% match requirement of the federal funds
received, 2) provide for a 3% price increase to unit prices to meals effective July 1, 2020, and 3) update
invoicing requirements.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein
contained, it is agreed by the parties as follows:

Scope of Services
Meal Payment
Department will pay the vendor for the actual number of meals received and accepted by Department
during the billing period.
A weekly invoice from the vendor will reflect the, following:
1. Total number of meals ordered.
2. Amount charged per meal.
3. Credit(s) for rejected whole meals or portion of meals that were below acceptable temperature
which were not replaced within the one (1) hour of rejection.
4. Amount of total invoice.
5. 10% Match deducted from the total invoice
6. Final Invoice Amount
7. � Purchase Order number must be included on all invoices
MATCH REQUIREMENT: The Contractor shall provide a match of at least ten percent (10%) of the
federal funds received. The Contractor's match will be made in the form or cash, general revenue
administrative funds and/or in -kind resources, or in the form of a set-off from the amount represented in
the Contractor's invoices.

No part of this agreement may be assigned or subcontracted by the vendor without prior written
approval by Division. If approved subcontracting shall be strictly 15% mark-up on cost. Pricing
