Food Services of Pinellas County, Inc. d/b/a G.A. Food ervice, Inc. (the "Contractor") and Collier County, a political subdivision of the State of Florida, (the "County"): WHEREAS, on May 14, 2019 (Agenda Item No. 16.D.6), the County entered into an Agreement with Contractor for home delivered and congregate meal services for the Senior Nutrition Program; and WHEREAS, the parties desire to amend the Agreement to modify Exhibit A -Scope of Services and Exhibit B-Fee Schedule to: 1) include a 10% match requirement of the federal funds received, 2) provide for a 3% price increase to unit prices to meals effective July 1, 2020, and 3) update invoicing requirements.
