EXECUTIVE SUMMARY
Recommendation to approve Change Order No. 11 to Agreement No. 05-3865 with Jacobs
Engineering, Inc., for restarting the design and permitting of the Collier Boulevard ("CR 951")
expansion from Green Boulevard to the Golden Gate Main Canal, to align the roadway
improvements with the County's recent acquisition of the former Golden Gate Golf Course and the
development of the Collier County Sports Complex, in the total not-to-exceed amount of $726,762;
and to authorize the necessary budget amendment. (Project 68056)
OBJECTIVE: To obtain approval of Change Order No. 11 to Agreement No. 05-3865 with Jacobs
Engineering, Inc., for the CR 951 expansion from the Golden Gate Main Canal to Green Boulevard, to
restart the design process to incorporate changes resulting from the acquisition of the former Golden Gate
Country Club (the "Golf Course") and the development of the Collier County Sports Complex (the
"Sports Complex"), and to authorize any necessary budget amendment.
CONSIDERATIONS: On December 13, 2005 (Agenda Item 10C), the Board approved Professional
Services Agreement No. 05-3865 (the "Agreement") providing for the expansion of CR 951 from Golden
Gate Boulevard to Pine Ridge Road. The design and permitting provide for a six -lane urban roadway
within the existing right-of-way whenever possible.
The County has previously approved ten change orders under the Agreement to: (1) extend the project
limits to the Golden Gate Main Canal, (2) to allow for permit extensions, and (3) to incorporat e other
design changes due to changing requirements (see attached detailed list).
The United States Army Corps of Engineers ("USACE") granted the initial permit (SAJ -2007-04904) on
August 19, 2013, with a five-year expiration. On August 1, 2019, the USACE issued a permit extension,
which expires on August 19, 2020. Similarly, the South Florida Water Management District ("SFWMD")
granted the initial permit (11-03184-P) on November 19, 2012, for five years. To date, SFWMD has
granted five extensions, and the last one expires on December 20, 2021.
During the past 15 years, under the Agreement, the County completed the design and construction of
Collier Boulevard from Green Boulevard to Golden Gate Boulevard, and continually extended the permits
to preserve the environmental status approved initially. However, in 2007, the design of the segment from
Golden Gate Main Canal to Green was only completed to the 60% stage. The economic downturn at that
time resulted in a reallocation of funding for the project. The plans were shelved until funding became
available. The design phase of this segment is currently budgeted to start in FY 2022 with construction to
follow in FY 2024.
On May 22, 2019, staff issued a temporary stop-work order with only two days remaining on the contract
to enable the processing of Change Order #10. On May 28, 2019, (Agenda Item 16.E.4.b), the Board
approved Change Order #10 to extend the Agreement for 330 days, which would allow the Consultant to
apply for another USACE permit extension. The suspension remained in effect until reinstated on April 6,
2020, with 332 days remaining.
The County's Golf Course, Sports Complex, and SFWMD reconstruction of its existing weir structure
impact the project as currently designed. Staff determined that Change Order No. 11 is necessary to keep
the United States Army Corps of Engineers' permit active and to refresh the 60% plans to current
standards and conditions. The design restart would also reevaluate the access management, the traffic
impacts within the corridor, and the public right-of-way requirements due to those proposed
developments. The scope will also include bridge development studies for the replacement of the bridge
located on Collier Boulevard at the Golden Gate Canal at the southern limit of the project, and, a new

bridge at either 27th Avenue SW or 29th Avenue SW to align with the access to the Golf Course. Most
importantly, the status of the current permits must be maintained; their lapse would restart the
environmental assessment process and halt any further roadway expansion. The Consultant requires an
additional 180 days to complete those tasks.
Included in the attached proposed Change Order No. 11 is also the development of a Design and
Construction Criteria Package sufficient for procuring a Design-Build Firm for the CR 951 expansion
from the Golden Gate Main Canal to Green Boulevard project.
Staff negotiated the proposed Scope of Services under Change Order No. 11 for a not -to-exceed amount
of $726,762, with an increase in hourly rates applicable only to Change Order No. 11. The rate increase
was necessary to align with the most current (May 2019), State Occupational Employment and Wage
Estimates, as published by the US Bureau of Labor Statistics for Architectural, Engineering, and Related
Services category. The data shows that since 2005 wages have increased by 21% on average. Another
factor was the cost of construction. In 2005, the FDOT Long Range Estimate to construct a mile of
roadway was reported as $2.2M, compared to 2019, which cost is now estimated as $4.9M.
FISCAL IMPACT: Funds in the amount of $726,762 are required for this Change Order. A budget
amendment, in the amount of $426,100, is required to move funding within Impact Fee Fund (336) from
Project 60129 to Project 68056.
LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote
for Board approval. -SRT
GROWTH MANAGEMENT IMPACT: The requested actions are consistent with Growth
Management objectives and produce no negative impact.
RECOMMENDATION: To approve Change Order No. 11 to Agreement No. 05-3865 with Jacobs
Engineering, Inc., for restarting the design and permitting of the Collier Boulevard (CR 951) expansion
from Green Boulevard to the Golden Gate Main Canal, to align the roadway improvements with the
County's recent acquisition of the Golf Course and the development of the Sports Complex, in the total

List of Change Orders for Project 68056 – Collier Blvd (CR 951) expansion from Green Blvd
to the Golden Gate Main Canal:
On May 22, 2007, under item 16B.9, the Board approved Change Order No. 1 to extend the project
limits to include final design up to Green Boulevard and 60% design from Green Boulevard to the
Golden Gate Main Canal in anticipation of a future design-build project for this segment.
The Board approved Change Order No.2 on October 28, 2008, under item 16B. 2 to align accesses,
drainage expansion, and right-of-way modifications with the project due to development
requirements along Pine Ridge Road and White Boulevard.
Change Order No. 3, required scope change to once again realign the project geometry to
development improvements within the corridor. Also, the County decided to expand Green
Boulevard to the east, which required a new bridge across the CR 951 canal and a realignment of
Green Boulevard to the west.
Change Order No. 4 increased the time to complete the various design changes necessitated by the
previous change orders.
Change Order No. 5 required the refinement of the project's geometry due to development impacts.
The approved change order captured the details of this action.
Change Order No. 6, required modification to the roadway lighting and roadside swales design
due to Florida Power and Light transmission and distribution facilities. It was deemed cost-
prohibitive to relocate the massive power transmission poles along the west side of the corridor.
On September 9, 2014, under item 16A. 29 the Board approved Change Order No. 7, required to
provide a safer and operationally functioning roadway transition to the existing segment south of
Green Boulevard. The 60% design of the section from Green Boulevard to the Golden Gate Main
Canal was no longer in the 5-year work program due to lack of funding; therefore, the prior
segment required a suitable transition back to existing features.
Change Order No. 8 increased the contract time and budget for post-design services during
construction.
On January 28, 2015, the construction commenced for Collier Boulevard (CR 951) from Golden
Gate Boulevard to Green Boulevard, and on October 27, 2017, completed the improvements at the
cost of $21,403,299.87.
On March 27, 2018, under item 16.E.6.a, the Board approved Change Order No. 9, to allow the
Consultant to extend the Environmental Resource Permit (ERP) and the United States Army Corps
of Engineer's (USACE) Permit. The segment from Green Boulevard to the Golden Gate Canal
does not have the right-of-way to treat and attenuate roadway stormwater; therefore, this segment
was conceptually approved under the original permits. The permits cannot be allowed to expire as
this would restart the permitting process and jeopardize the improvement of this segment. The

ERP was granted an extension until December 20, 2021, and the USACE was extended until
August 19, 2018.
Change Order No. 10 extended the USACE permit, which now expires August 19, 2020, and no
further extensions will be granted.

As requested, Ardaman & Associates, Inc. (Ardaman) is pleased to submit the following proposal for
geotechnical engineering services for a proposed new bridge at 29th Avenue SW over the CR 951
Canal, a proposed bridge replacement of Bridge No. 030174, and subsurface soil exploration for the
canal along 39th Street SW in Collier County, Florida.
It is our understanding that a Design-Build contracting method is being considered to materialize this
project and that information is needed so design-build firms bidding the project are offered with
geotechnical data to build these structures.
During our phone conversation on 04/07/2020, you indicated that the final location of the proposed
new bridge has not yet been decided. This proposal has been prepared assuming that the new
bridge will be located at 29th Avenue SW. We also understand that the existing 142-ft long Bridge
No. 030174 will be replaced with an 8-lane bridge. Both structures will span over the CR 951 water
canal. Additionally, you have requested that we include up to 5 soil borings for the canal along 39th
Street SW. Other details about the proposed bridges were unknown at the time of writing of this
proposal.
Using the Florida Department of Transportation (FDOT) guidelines for exploration as addressed
in the 2020 Soils and Foundation Handbook, we have prepared a summary of anticipated tasks
and services as follows:
Obtain Collier County Test Boring and ROW Permits.
Our service includes staking the borings and calling Sunshine State One-Call of Florida
(811).

Drill two Standard Penetration Test (SPT) borings for the proposed new bridge, one at each
side of the structure. Each boring will be advanced to a maximum depth of 100 feet or
shallower if adequate bearing material is encountered. The SPT borings will be continuous
in the top 15 feet and at 5-foot intervals thereafter. The borings will be located at opposite
bridge corners as utilities and ground conditions allow. We will locate the boring for the
west side of the proposed bridge in the existing CR 951 roadway requiring maintenance of
traffic (MOT) by Index 603 to shut down one lane of traffic. We anticipate 1 day of MOT.
Drill two Standard Penetration Test (SPT) borings for the proposed bridge replacement,
one at each side of the structure. Each boring will be advanced to a maximum depth of
100 feet or shallower if adequate bearing material is encountered. The SPT borings will
be continuous in the top 15 feet and at 5-foot intervals thereafter. The borings will be
located at opposite bridge corners as utilities and ground conditions allow. We will locate
the borings in the corners accessible to our drilling equipment therefore we don’t anticipate
the need of maintenance of traffic (MOT).
Drill up to five Standard Penetration Test (SPT) borings for the canal along 39th Street SW.
Each boring will be advanced to a maximum depth of 15 feet. The SPT borings will be
continuous in the top 10 feet and at 5-foot intervals thereafter. The borings will be located
along the canal as utilities and ground conditions allow. We will locate the borings in the
east side of the canal therefore we don’t anticipate the need of maintenance of traffic
(MOT).
Groundwater levels at the time of fieldwork will be recorded.
Collect soil samples from the SPT borings to aid in soil classification and to determine the
engineering properties of the soils. Soil index tests will include moisture content, percent
fines, sieve analysis, organic content, and Atterberg limits. Additionally, soil samples will
be obtained from each boring for soil corrosion series tests and a water sample obtained
from the canal for water corrosion series (pH, resistivity, chlorides and sulfates).
Provide the horizontal coordinates of the borings as given by a handheld GPS locator,
which is typically accurate to ± 10 feet, depending on field conditions. Obtaining any
elevation measurements at the boring locations is beyond the scope of this proposal.
Engineering and technical support services will also be required to analyze the data and to
prepare an engineering report.
Use Standard FDOT reporting procedures for bridge structures consisting of a
Geotechnical Structures Report for each one of the two bridges.
Based on our knowledge of the project to-date, the attached summary of services and corresponding
fees will be necessary to meet our goals. We have prepared a separate summary of services and
estimated fees for the proposed new bridge at 29th Ave. SW over the CR 951 canal and the proposed
widening of Bridge No. 030174. We will invoice in accordance with the attached schedule for actual
services rendered. Our estimated fees are $20,000 for the proposed new bridge, $22,000 for the
bridge widening, and $7,500 for the subsurface soil exploration for the canal along 39th Street SW.
Our total estimated fees under this proposal are $49,500. If initial findings indicate that additional
services are necessary, then we will contact you for authorization.

format. Hard copies of the report can be provided upon request at the express courier service costs
if requested.
Our timeframe to complete our field work and prepare the reports for the proposed bridges is
approximately 3 months from notice to proceed.
Please return a copy of the attached Project/Proposal Acceptance form complete with client name
and signature to this office as our authorization to proceed. The party whose signature appears
on the acceptance form will be invoiced for our services.

RWA, Inc. is pleased to submit the following proposal for land surveying services associated
with the development of the subject project. Outlined below is our understanding of the
project profile and the assumptions we have used to develop our scope and associated fees in
response to your request for proposal.
PROJECT PROFILE
•Jacobs (client) will be designing and preparing improvement plans for the bridge crossings
Collier Blvd. First crossing is at 29th Ave SW, the second crossing is at the existing bridge at
the Golden Gate Main Canal.
•Client desires to retain the services of RWA, Inc. (Consultant) and proceed with the Project
as described within this proposal.
•All surveying activities shall be performed under the direct supervision of a Florida
licensed Surveyor, and performed in accordance with the State of Florida
Standards of Practice Chapter 5J-17, Florida Administrative Code, as set forth by
the Board of Professional Surveyors and Mappers, pursuant to Section 472.027,
Florida Statues.
PROJECT ASSUMPTIONS
•The horizontal datum and vertical datum were established during the original work
project. The horizontal datum is NAD 88, Florida West Zone, and the vertical datum is
NAVD 88. RWA has both horizontal and vertical cont rol along the corridor. We will make
comparison to that control shown the plans set.
•This proposal includes performing all services described within on a one-time basis.

SCOPE OF SERVICES
1.0 Topographic Surveys for Two Crossings
1.1. RWA shall prepare Topographic Surveys and cross sections for each of the
project areas. This shall also include locations of any existing improvement,
evidence of exiting utilities, roadways, bridges, etc.
1.2. RWA shall prepare Topographic Surveys in the area of potential lane shifts or
realignments.
2.0 Reimbursable Expenses
Expenses for copies, blueprints, reproduction services, overnight or express
delivery and, services not specifically specified herein shall be reimbursable to
RWA, Inc.
PROFESSIONAL SERVICE FEES
The professional service fee estimates for the associated scope of services are listed below.

ACCEPTANCE AND AUTHORIZATION TO PROCEED
Period for Acceptance
This Proposal/Agreement is open for acceptance by Client through June 30, 2020. After the
acceptance date, the proposal may be withdrawn or subject to modification by RWA, Inc.
Acceptance
If this Proposal/Agreement satisfactorily sets forth Client’s entire understanding of the
agreement, please sign one copy of this agreement in the space provided and return it to
RWA, Inc. as authorization to proceed with the work.
Owner/Client Authorization
I hereby certify that the undersigned is the Agent to the Owner of the property that is the
subject of this proposal. I hereby certify that the undersigned has the authorizing authority to
execute this contract for professional services. I hereby authorize the performance of the
services as described herein and agree to pay the charges resulting thereby as identified

Code and Fee Schedule of RWA, Inc.
