06/09/2020 EXECUTIVE SUMMARY Recommendation to approve the “After-the-Fact” Bureau of Justice Assistance Comprehensive Opioid Stimulant and Substance Abuse Site-Based Program grant application in the amount of $900,000 OBJECTIVE: Engage inmates exiting the jail system in medication-assisted treatment (MAT) to reduce re-incarceration. CONSIDERATIONS: Collier County was one of sixteen (16) counties nationwide participating in the implementation of MAT in correctional facilities. The County was also selected as one o f four (4) demonstration sites to be evaluated further by the Friends Research Institute in February 2020. Collier County was recognized by the Friends Research Institute for our plan to address the Opioid epidemic in correctional settings and our countywide strategic planning efforts to comprehensively address substance abuse and mental health. The purpose of the project is to expand access to MAT for jail inmates during incarceration and as they reintegrate into the community, and to expand access to overdose prevention resources in order to prevent deaths from an opioid overdose. The preparation of this grant application occurred through a collaborative effort with the Collier County Sheriff Office (CCSO) and David Lawrence Center (DLC) through Collier County’s Community and Human Services Division (CHS). Collier County is the lead applicant, and Collier County CHS will perform all grant management duties for the duration of the grant period. The focus of this application is to obtain $900,000 in funding over three (3) years, to achieve the following objectives: • Assist at least 150 unique individuals across three (3) years to access MAT services in the jail. • Implement and form a co-response team (Law Enforcement, EMS, Mental Health Professional, and a peer) to develop a Naloxone Leave Behind Program. • Continue to utilize and expand the Collier County Sheriff’s Office/Community Mental Health Provider Law Enforcement Assisted Diversion (LEAD) program for individuals seeking treatment for their disease. Increase recovery supports and addresses barriers to individualized recovery and community integration. The current Bureau of Justice Administration (BJA) funding opportunity was announced on February 21, 2020, with a submittal date of May 21, 2020. Due to the unique nature of the grant and its collaborative application process between agencies, more time was needed to assemble the application. Collier County CMA #5330 authorizes the County Manager to approve the submittal of grant applications with subsequent Board of County Commissioners (Board) action at the next regularly scheduled Board meeting to ratify the approval as an “After-the-Fact” application. The County Manager approved the grant application on May 21, 2020, and it was timely submitted. As fiscal agent for the grant, Collier County will be responsible for ensuring compliance with the programmatic and financial reporting requirements of the Comprehensive Opioid Stimulant and Substance Abuse Site-Based Program. Should the funding be awarded to the County, the agreement will be brought to the Board for consideration at a future meeting. FISCAL IMPACT: If awarded, funding from the BJA in the amount of $900,000 will be appropriated 16.D.5 Packet Pg. 1107 06/09/2020 within the Human Services Grant Fund (707) via a Budget Amendment at the time of the Board’s acceptance. There is no local match requirement for this grant. GROWTH MANAGEMENT IMPACT: There is no Growth Management impact due to this request. LEGAL CONSIDERATIONS: The Board will have the opportunity to accept or reject the funds if the grant is approved. Located in Southwest Florida, Collier is the second largest county in Florida geographically with 2,305 square miles and is the 161h most populous county, The population estimate in 2019 was 378,488, with most concentrated along the Gulf Coast. In the span of 10 years, Collier County grew from 312,896 permanent residents in 2009 to an estimated more than 380,000 in 2020. Collier County includes the incorporated cities of Naples and Marco Island as well as the agricultural and fishing communities of Immokalee and Everglades City. A sharp contrast in population characteristics exists between the city of Naples, on the Gulf Coast, and Immokalee, a largely migrant farm worker community located inland. In Immokalee, 43% of residents are below the poverty level, while in Naples the poverty level is 9%. Collier County overall has a high number of uninsured adults 22.8% compared to the state population of 16%. The purpose of the project is to expand access to Medication Assisted Treatment for jail inmates during incarceration and as they reintegrate into the community, and to expand access to overdose prevention resources in order to prevent deaths from opioid overdose through LEAD and a Naloxone Leave Behind Program. Statement of the Problem: The lead applicant agency is Collier County Government in partnership with the Collier County Sheriff's Office (CCSO) and the community mental health/substance use treatment provider, David Lawrence Center (DLC). Based on the population size of our county, Collier County is applying under Subcategory lb. An analysis of the current population of Collier County indicates the jail population demographics do not mirror those of the general population in the County. The jail population is 25% Black, while Blacks account for 7.4% Countywide and 68% of the County is White (non -Hispanic) the jail population is 50% and the community to jail population for Hispanic/Latino is almost identical at 25%. Collier County operates two jail facilities with a combined inmate population of 1304. Approximately 42% of the Collier County Jail inmate population have self -identified as using illegal substances at intake. It is estimated that the number of inmates suffering from a substance use disorder is higher than the number who self -identified, as inmates tend to not be honest about substance use, due to fear of additional criminal charges. In addition, jail administration has identified an additional, 31 % of the jail as suffering from a mental health and/or a substance use or co-occurring disorder. However, the percentage of inmates with mental health and substance use problems has significantly increased year over year and has become an alarming trend, The Collier County Sheriff's Office has collaborated with the High Intensity Drug Trafficking Areas (HIDTA) program to collect accurate overdose information for Collier County. In 2019, there were a total of 448 overdoses and 34 fatalities in Collier County, this was an increase from 2018. Of those 448 overdoses, NARCAN® was deployed a total of 282 times; 179 were single deployments and 103 multiple deployments. From January 1, 2020 through April 21, 2020, Collier County has reported 138 overdoses, 20 of which were fatal. There have been 105, NARCAN® deployments through March 2020. The most alarming statistic is the fatality rate for 2020 has increased by 56%. This is the largest increase seen since data has been collected. Without immediate intervention and the implementation of MAT, Collier County is at an enormous risk of more than tripling overdose deaths in 2020. Implementation of a MAT team will reduce unnecessary detox at the time of arrest and potentially eliminate the risk for overdose upon release from jail. In addition the implementation of a Leave Behind Program will assist in a fatality reduction. Through the Strategic Planning process Collier County and its judicial, healthcare, mental health & substance abuse and law enforcement partners determined the need to map the system of care in support of the Countywide plan, The Sequential Intercept Model was used as a conceptual framework to organize targeted strategies for justice -involved individuals with serious mental illness and substance use disorders, identified the need for expanded MAT 2 services and increased law enforcement assisted diversion (LEAD) strategies to address increasing opioid use and overdoses, particularly those related to individuals in the criminal justice system. The model (Figure 1) depicts the six primary points or intercepts where an individual would typically progress through the criminal justice system. At each of these points are opportunities to intervene and "intercept" the person, moving or diverting them from the justice system to treatment system. Collier County has employed Crisis Intervention Training for 15 years and provides intensive training to law enforcement, emergency services, fire and all other first responders. Collier County has a Central Receiving Facility under design with construction to begin in 2022. This facility will serve as a "one -stop" drop-off center for law enforcement and first responders for those with minor crimes that might have otherwise been brought to jail or a hospital. This facility will serve adults in need of detoxification on a locked unit as well as serve those in a mental health crisis. Intercept 1: To combat re -incarceration and promote treatment as an alternative before criminal activity, CCSO, in partnership with DLC, developed the Law Enforcement Assisted Diversion (LEAD) initiative. This program works with members of the community by promoting safety, coordinating treatment options for health- related problems, and developing alternative -system responses independent of the justice system. This program allows law enforcement to redirect certain drug -related activity to community - based treatment instead of jail or prosecution. By diverting these individuals, the program can save lives, save families, and improve public safety. This program is available for individuals looking for help and assistance with their disease and is a voluntary program. To address recent increases in the number of opioid overdoses in the community, a co -responder program (consisting of law enforcement, Emergency Medical Services, and DLC, partnered with a peer recovery specialist), has been designed to meet individuals where they are in their recovery and attempt to link them to treatment. This team has identified the need to develop a leave -behind Naloxone program, These co -responder initiatives have been found to reduce overdose fatalities in other communities across the nation and have been proven to be the "gold standard" of overdose interruption in numerous communities. Intercept 2 includes robust screening protocols currently in place in the Collier County Jail to review inmates' histories of substance use and mental health challenges in order to make appropriate referrals to diversion programs. Intercept 3: Our current system does not allow for the administration of MAT in our jail and is an identified gap in service. Our jail cannot prescribe methadone but has worked over the last 12 months to establish a memorandum of understanding (MOU) with the local methadone clinic to come into the jail and dispense medication for its patients. Under this grant, in combination with state grants, the jail will provide naltrexone and buprenoiphine. In addition, Collier County offers three distinct Treatment Courts, which include Drug Court, Mental Health Court (MHC) and Veterans Treatment Court (VTC). All three are adult criminal courts, with MHC and VTC serving felony and misdemeanor defendants, and Drug Court serving only felony defendants. Drug Court currently serves approximately 50, MHC serves approximately 30, and 4 VTC serves approximately 12. These programs enjoy tremendous support and buy -in from the various stakeholders, and from the community. Intercept 4 has been the area identified as a gap in our service delivery system. Currently those coming into the jail on MAT are not receiving services nor are those who are interested in MAT being provided the support at time of release. Our critical service gaps include the necessary MAT implementation and community transition. Intercept 5: Re-entry is critical and as part of our data collected during the COAP planning grant has provided the support for the County to move forward with a diversion and implementation of MAT for this target population. Obstacles to community reintegration for individuals released from jail include history of repeated drug arrests, and lack of access to employment benefits, housing and treatment services, Due to this overwhelming concern and increased overdoes and fatalities, Collier County began researching implementation of medication -assisted treatment in our jail setting and transitioning inmates to MAT services the community. The most effective treatment for opioid use disorder is the use of FDA -approved medications (methadone, buprenorphine and extended - release injectable naltrexone), a treatment commonly known as medication -assisted treatment (MAT). During the last 9 months, Collier County has been a participant in the Bureau of Justice Assistance's Comprehensive Opioid Abuse Program (COAP) "Building Bridges" grant. A team of 5 delegates in conjunction with other County agencies, has developed a MAT implementation plan for the Collier County Jail and community. Throughout the planning period, the team has collected data on the need for MAT in the jail setting. The data elements collected include: inmates' history of naloxone deployment and/or non -fatal overdose, whether a withdrawal management protocol needed to be initiated in the jail, whether the inmate was receiving MAT at time of arrest or in the previous 12 months. Given the significant number of inmates requiring a detox protocol during our data collection period, the need for MAT to be implemented in the jail has become increasingly evident. Data has been collected in Collier County from a six-month period from August 2019 to January 2020 which indicated that at the time of booking, 49 individuals reported receiving Buprenorphine, 6 reported receiving Vivitrol, and 88 were receiving methadone at the time of their arrest. An additional 39 inmates reported prior treatment with MAT in the last 12 months. An additional 116 inmates indicated that they would be interested in starting a MAT program, During the last quarter of 2019, 432 new jail intakes (22Va) inmates were on a detox protocol. Data analyzed between August 1, 2019 and February 29, 2020 indicates that 333 persons who were incarcerated, were identified as having co-occurring disorders. Collier County has found that inadequate transition planning contributes to individuals with substance use and co-occurring disorders who enter the jail in a state of crisis returning to the streets still in crisis. Many of these individuals have a history of trauma, a lack of engagement with community support systems, and unemployment, When these needs are left unaddressed, recidivism and other negative community outcomes are more likely to occur. Collier County providers often sees these same inmates end up in emergency rooms, back in jail, or in psychiatric inpatient or detox units --all with major financial costs for the community. The following are Collier County priorities for 2020-2024: ➢ Increase use of evidence -based practices for treatment of addictions in order to eliminate/reduce overdoses (fatal and non -fatal). ➢ Increase law enforcement -related opportunities for early intervention, diversion, and improved care for high -risk, high -needs persons who are incarcerated or at risk of arrest or Baker Act through LEAD. 0 ➢ Implement, enhance, and sustain coordinated reentry/reintegration services and processes using MAT for those with OUD. ➢ Expand, improve and sustain the justice system response to persons with mental health and substance use disorders in or at risk of involvement in the criminal justice. Collier County is requesting funding to implement an in jail MAT and community MAT care team to fill our gaps in intercept 4 and intercept 5, reduce overdose deaths and enhance treatment and recovery post release. Without the funding through this award, those arrested persons will not receive the necessary supports and will detox in the jail setting without medication and not be connected to treatment services. The County currently has a state funded criminal justice award for 1.2M over three years and is in the process of re -applying. Should the County be awarded state funds again, these funds will be used to enhance the funding received under this grant. The state grant will fund the medication and jail and community personnel. The funds under this award will support the medical services. b. Program Design and Implementation Collier County and its partners plan to implement a diversion and treatment program to include LEAD, a Leave Behind Program and access to MAT. Effective agency partnerships and collaborations have been identified as vital to the national opioid response. Partnerships such as diversion programs, medication administration programs, and data sharing efforts, with multi- disciplinary dimensions, such as embedding social workers and mental health professionals in law enforcement organizations can make all the difference in a community. Law Enforcement Assisted Diversion (LEAD]: The CCSO Mental Health Intervention Team (MHIT) currently oversees the LEAD Program and would like to expand to increase outreach through a Leave Behind Program. The LEAD Program was developed in late 2018 and has assisted individuals in need of substance use treatment into care at Community Mental Health and Substance Abuse provider agency. To date, the program has been successful in getting the individuals requesting help into treatment. The CCSO MHIT team is comprised of a Law Enforcement Deputy and a LCSW. The MHIT team is tasked with addressing a vast array of duties in the county that include but are not limited to: interacting with individuals with severe and persistent mental health/addiction issues, crisis callouts, court duties, and community presentations just to name a few. Once an individual is identified they will be connected to community supports and for those in need of MAT connected with the team and also provided education on Naloxone and provided a kit and training, if needed. MAT: National and international professional bodies consider medical treatment for opioid use disorder (OUD) to be the evidence -based best practice for treating OUD. To combat recidivism and opioid use Collier County will implement an in -jail and community -based MAT program for all inmates who had been receiving MAT at time of arrest. The use of this evidenced based treatment approach will provide recovery support while incarcerated and upon release. It is the intent of the Collier County Jail personnel to systematically screen all patients for substance use disorder (SUD) and signs of withdrawal using validated screening instruments to determine which patients have an OUD. The Collier County Sheriff's Office contracts with Armor Correctional Health for inmate health care. At the time of booking, each inmate is assessed using an Armor Correctional Health Patient Intake Health Screening Form (PT-051). In order to gain a better understanding of how many inmates booked into the Collier County Jail suffer from a substance use disorder, the PT-051 has been expanded since August of 2019. Nursing staff has been identifying inmates who report receiving medication -assisted treatment (MAT) at time of arrest or who reported receiving MAT in the previous 12 months. For those who were on MAT at the time of their arrest, information is collected regarding the type of medication and the name of the provider dispensing their medication. In addition, information regarding prior use of NaIoxone is collected, along with the number of overdoses the inmate has experienced. Armor Correctional Staff will be using the Texas Christian University Drug Screen V (TCUDS-V) screening tool. The use of a validated, effective screening and assessment tool promotes favorable treatment engagement and outcomes. Together with collateral records (e,g„ physical/mental health status, urine test results, arrest records, etc.), screening and assessment tools will help inform the treatment placement, planning, and decision -making process as well assisting the Collier team to optimize specialized interventions. Collier County has developed a dedicated MAT Care Team, through funding from BJA/ IRR, that consists of staff members based both in the jail to assist those on MAT during their incarceration and community -based team members to assist those being released on MAT as they transition back to the community. Collier County will use the grant funds, through subrecipient agreements to fund the local Iaw enforcement agency and jail administrator (CCSO) and the community behavioral health treatment provider (DLC). The Collier County jail currently has a contractual agreement with Armor Correctional Health Services, the jail medical/mental health services vendor. The grant funds will be used to support the cost of in -jail MAT medical personnel and a discharge planner as well as the cost of medical personnel to support services in the community through the identified sub -recipient agreement. It is the intent of the Collier County MAT program to begin and work with a small cohort of participants. This allows change to be initiated, assessed and improved before expanding to a larger number of participants. Collier County will be using a "Medication First" model. The implementation of this evidence -based model will allow jail medical staff to ease inmate suffering by managing withdrawal symptoms and assist to reduce drug craving. Our approach will focus on four main principles (a) receiving medication quickly, (b) maintaining medication levels without arbitrary tapering, (c) individualized psychosocial services, and (d) discontinuation only if worsening condition. Phase #I: The following steps represent the "engagement" of the inmate with OUD in services: a. A brief screening occurs at the time of booking with the Armor nursing staff. Those inmates who report current MAT use are immediately engaged with the MAT nursing personnel. b. Inmate receives the TCUDS-V screening and urine screen. Jail MAT Care Team (JMCT) meets with the inmate and develops a plan for medication administration and services while in the jail c. Inmate is engaged with Armor medical staff to implement treatment and a Discharge Planner to begin preparation for Reentry. Phase #2: Jail MAT Care Team nursing staff will obtain proper releases of information in order to obtain documentation from the MAT provider in the community and verification of the inmate's medication and dosage. The Jail MAT Care Team Advanced Practice Registered Nurse (APRN) will then meet with the inmate to prescribe MAT medication, The goal is to have the inmate receive MAT within 48 hours of intake. When applicable, inmates on Vivitrol will continue to receive Vivitrol injection provided/coordinated through their community provider. Inmates on Methadone will receive services thorough a MOU agreement with the Methadone provider and those on/receiving Suboxone/buprenorphine will have medications prescribed by 10 and dispensed by Jail MAT Care Team Medical Staff. Medications are dispensed by qualified staff members and our medical/nursing staff who will closely watch reactions to the medications for quick possible dosage adjustments or discontinuation. Medication levels are closely monitored on a continuous basis by means of urinalysis testing and the dosage is adjusted as needed over time as the patient progresses through the different phases of treatment. The Collier County Jail currently offers a voluntary residential treatment program through partnership with DLC, Project Recovery (PRP), for inmates with substance use disorders. Referral to PRP is available and encouraged for those clients receiving MAT. Phase 03: JaiI Reintegration and Reintegration Discharge Specialists will meet with the inmates on MAT prior to their release to develop a discharge plan. This team will include trained staff who will conduct Overdose and Naloxone Distribution training with the individual and the natural supports prior to discharge. The inmate will be provided two Naloxone kits. In addition, during the 34-day period prior to release, MAT Care Coordination team members complete a discharge plan with the inmate. Research has shown that recovery support case managers and peer specialists bridge the gap between institutions and communities. These case managers and peer specialists arc available to meet with individuals before release and remain available for support and assistance for up to a year after release. Among other duties, recovery support case managers may accompany released individuals when they first enter treatment programs, meet with medical providers, or engage in other recovery related activities. It is the intent of the Collier MAT team, through a "warm handoff," that each inmate being discharged will be linked to Community MAT Care team members. The inmate will work closely with the Community MAT Care Coordinator (CMCC) and Peer Specialist (PS) who are employed by DLC. This team will ensure 11 comprehensive and holistic assessment of post -discharge needs and referrals to various community providers for services such as continued MAT and treatment, options for housing and supportive employment services. Phase #4: Individuals returning to the community after incarceration face a myriad of challenges, including securing stable housing, employment and income; complying with community supervision requirements; accessing transportation; and rebuilding social connections. When these needs go unmet, the likelihood of accessing and remaining on MAT in the community diminishes. Studies demonstrate that the elevated risk of opioid-related overdose death among people released from jails and prisons is increased through disrupted social networks, poverty, disruptions in health care access, stigma and an exacerbation of underlying mental illness and SUD. People released from jails or prisons are four times more likely to be homeless and individuals without stable housing reported the highest levels of drug use after release from a correctional facility. Data has shown that the lack of transportation was cited as the top reason individuals did not connect to MAT services post -release among individuals who received MAT while incarcerated. The Community MAT Care Coordinator will work with community treatment providers to establish the inmates' follow-up appointment with treatment providers, with a goal for this appointment to occur within 24-72 hours of discharge. The Community MAT Care Coordinator will also establish connection with the local community behavioral health pharmacy, Genoa Healthcare, who partners with DLC to provide access to MAT medications. Those discharged on methadone will return to the methadone provider and will continue to be followed by the Community MAT Care Coordinator and/or Peer Specialist to ensure continuation of treatment 12 services. As each participant progresses through the program, sustainability for continued care and a transition plan will be developed and modified with the support of the MAT Care Team. A medical MAT team will be established through the grant to provide MAT administration. Each participant will receive regular visits with the prescriber. Between the follow-up appointments the client may be assessed by the MAT nursing support staff. During office visits, urine drug screens to test for other substances and for the presence of buprenorphine or its metabolites will be administered. Other lab testing may be recommended such as liver function test or pregnancy testing for female clients. In the event a client tests positive for illicit opioids, the client may receive increased buprenorphine dose, increased medical visit frequency or motivational interviewing/peer support. The MAT Care Team includes a peer specialist. These individuals have lived experience of individuals in recovery, as well as that of their families, is an essential element of successful programs for individuals with substance use disorders. Peer recovery coaches have an important role on the MAT team. Peer recovery specialists are people who have lived experience with substance use and SUD who are trained to assist others with their recovery. Peer recovery coaches offer knowledge that treatment staff often lack based on life experiences. An increasing number of MAT programs in correctional settings have integrated peer supports within MAT teams. The CMCCT Peer Specialist will facilitate re-entry into the community alongside the CMCCT Care Coordinator. It is the intent of the Community MAT Care Team to provide up to 6 to 12 months of support and linkage with outpatient treatment services, however services will be based on individual need and may continue beyond this timeframe. MAT will be prescribed by the community facility and will be consistent with its purpose of being a risk -reduction and craving- 13 reducing component of the program. It is important to note that medication will not be contingent upon the individual receiving or participating in behavior therapies or other services. All MAT Care Coordination team members will be trained in motivational interviewing (MI) techniques. MI techniques help to engage and retain participants in treatment, supports self - efficacy, and uses shared decision -making to identify goals. Phase #5: When a client is ready for discharge from the program, they have reached the maintenance phase. The client is functioning well on a steady dose of MAT medication with little or no cravings and little or no illicit drug use. This phase will continue indefinitely for most clients since Iong-term maintenance is recommended due to a high risk for relapse. The overall goals of the project include: (a) appropriate medication dosing; (b) assisting those served in making continued improvement in functioning and quality of life; (c) facilitating a warm handoff to maintain and continue treatment; (d) decreasing frequency and severity of relapse/return to use;(e) retain clients in treatment; (f) reducing recidivism; (g) expanding local initiatives; (h) improving the accessibility and effectiveness of treatment services. c. Capabilities and Competencies The key project partners, Collier County Board of County Commissioners, Collier County Sheriff's Office and the David Lawrence Center, have a proven history of collaboratively developing and managing criminal justice diversion and treatment programs in Collier County. These include: 1) an adult Mental Health Court in operation since 2007; 2) an adult felony Drug Court, active since 2000; 3) a Veteran's Treatment Court, active since 2015; 4) the Forensic Intensive Reintegration Support Team (FIRST) jail reintegration case management program, active since 2010; 5) Crisis Intervention Team (CIT) training since 2008; b) Project Recovery 14 Program (PRP), a short-term (45-90 day), residential substance treatment program for incarcerated inmates since 1994. (a) Collier County Community and Human Services (CHS). The Collier County Board of County Commissioners (BoCC) will act as the applicant, primary grantee, and fiscal agent for the collaborative project. CHS, a department of the BoCC, is an experienced grantee for the criminal justice and behavioral health programs. The CHS is a past recipient of the Bureau of Justice Assistance Drug Court Enhancement grant 2017-2020. CCHHS is a current grantee for the Florida Department of Children & Families' Criminal Justice Mental Health & Substance Abuse Reintegration grant since 2010. The CHS office will appoint a project coordinator whose responsibilities will include: 1) provide continued oversight of the Grant Program; 2) continue to convene the partners for regular collaboration meetings; 3) collect and compile data from partner agencies as required by the grant; 4) submit required programmatic and financial reports to the grantor. The coordinator will ensure compliance with the statutory and mandatory requirements of the grant program requirements and serve as the primary point of contact with BJA. CHS will also establish the agreements with all participating entities outlining their scope of work, program parameters and initiatives. CHS will also be responsible for data collection. (b) Collier County Sheriff's Office (CCSO). The CCSO LEAD program team is overseen by the CCSO Mental Health Bureau (MHB). The MHB is supervised by a Certified Law Enforcement Lieutenant (who has a master's degree in Mental Health Counseling) and other members include a Certified Law Enforcement Sergeant, a contracted David Lawrence Center licensed clinical social worker and two CCSO support staff. Jail MAT program will be overseen by the Collier County Jail Administrator, who is the Chief of Corrections, who oversees the Corrections Operations and the Corrections Support Division. The MAT discharge planner is 15 a civilian appointee who reports the Jail Administrator's team. Each of these divisions is run by a deputy at the rank of Captain or Director. The Corrections Support Division oversees, but is not limited to, the contracted health services for inmates and the Inmate Programs and Reentry Sections. The Collier County jail health services contract is with ARMOR. ARMOR provides a Medical Director, On -Site Physician, ARNP and a RN Health Services Administrator and S RNs and several LPNs. The health services provider will be responsible for completing the TCUD assessment and prescribing and dispensing the Bupenorphine and Naltrexone. ARMOR will collaborate with the local Methadone provider who will come into the jail to dispense. (c) David Lawrence Center (DLC). DLC provides comprehensive mental health and substance use treatment services for all of Collier County, including inpatient, outpatient, residential and community based -prevention and treatment services. Evidence -based programs include (but are not limited to); trauma specific -services, supported housing, MAT and other addiction treatment services. Forensic services department includes Drug Court, Mental Health Court, community -based competency restoration and the jail community reintegration program. Grant funds will support the MAT treatment providers at DLC through an advanced practice registered nurse (APRN) credentialed to provide MAT, and a nurse to assist the team. DLC case managers will assist with benefit application processes and will ensure linkage to primary care and medical follow up for each participant. Peer recovery specialists provide specialized recovery coaching to MAT recipients. All DLC programs are available to participants. DLC will bill Medicaid, Medicare, and the state contract for community -based treatment costs for individuals when possible. The Care Coordination "wraparound" model includes "enhancement funds" which help pay for items that are necessary to facilitate community integration, including housing, transportation and medications. `t DLC is accredited by The Joint Commission (TJC) and has decades of experience administering millions of dollars in annual federal, state, and local government grants and contracts. DLC maintains an exemplary record of meeting or exceeding expectations of each of its grantor and contractor organizations. DLL's centralized Access Center provides assessments and crisis support on a walk-in basis, 24/7. DLC is the sole Baker Act -receiving facility in Collier County. (d) The primary grantee partners are all members of the Collier County Public Safety Coordinating Council (PSCC). The group has collaborated on strategic plans together since 2010. The Planning Council and subcommittees will facilitate improved coordination of all of the current diversion programs offered in Collier County and provide direction for future development and sustainability. Each of the partner agencies, has demonstrated its long-term commitment to the project. This commitment is demonstrated through completion of an interagency MOU, participation in the Criminal Justice Mental Health Substance Abuse (CJMHSA) Planning Council and strategic planning, and through its ongoing operations of several local centralized coordination projects. Council member input is essential to ensure the programs are meeting the needs and expectations at both the policymaking and service delivery levels. The PSCC will review the grant implementation and data at regularly scheduled meetings. The Collier County grant staff will prepare an annual summary report to the PSCC and identify areas to improve performance and discuss the areas of success. In addition, the grantee partners,will meet as a group on a quarterly basis to review the progress of the program and address any issues associated with the project. These partners will also serve as members of the PSCC subcommittee. 17 (e) Community Partnerships: Collier County has an extensive availability of resources for those persons involved in this project. These include but are not limited to: (A) Mental Health Court, Veteran's Treatment Court and Adult Drug Court; (B) access to integrated, co-occurring capable treatment provider (DLC); (C) judicial and multi interagency leadership; (D) Supported Employment and Supported Housing, Recovery Residences, SOAR (SSI/SSD Outreach, Advocacy, and Recovery (E) support from broad community including homeless services/shelters (St. Matthew's House, Hunger and Homeless Coalition of Collier County) and National Alliance for Mental Illness (NAMI)-Collier County; (F) Florida Assertive Community Treatment (FACT) Team (operated by DLC) (F) The Willough of Naples treatment for co- occurring disorders; (G) PATH (Projects for Assistance in Transition to Homelessness) program at CASL; (H) New Seasons Opioid Treatment Program (OTP). The project will engage advocates, peer specialists, family members, and responsible partners. MAT Care Team staff members will participate in community awareness meetings, including the Collier County Drug Response Team, to provide opportunities for education, outreach and support related to opioid overdose prevention and access to evidence -based treatment services. d. PIan for Collecting Data Required Collier County will collect a set of data elements for every incarcerated individual who participates in the LEAD and MAT programs. The LEAD program will create a simple report that will include biographical data and circumstances surrounding the contact of the individual who takes advantage of the opportunity to utilize the LEAD Program. The MAT program team will create reports that indicate all inmates who received MAT while in jail, to include type of medication, length of stay, history of overdoses and community connections/appointments made. Collier County will utilize Homeless Management Information System (HMIS) and the jail data 18 base (WINGS) to track individuals served by the grant. Collier County will collect the participant data on a monthly basis from the subrecipients to evaluate performance, participants served during the project and post discharge. The County currently utilizes data searching system, LexisNexis, to locate persons post discharge to enable long range tracking as a measurement of our success. Relevant data will be readily available for monitoring and evaluation purposes. Currently, data is being collected and shared and includes easily identified metrics that have not created a significant burden to staff. Medical data, for example, is already collected and stored in electronic health records or paper charts. Data related to the person's incarceration is already stored in the corrections department's records. Collier County has also recently adopted and countywide behavioral health Strategic Plan. Data sharing is a key priority in the County's Strategic Plan, and has been highlighted as a community need by the Board of County Commissioners. It is the intent for all stakeholders to work together to determine what types of additional data and measures are relevant. Where identifiers are used to track individuals across systems, agreements will be essential to enabling the sharing and integration of data. Findings from these analyses will be shared in a collaborative manner, so that all agencies involved may benefit from the information shared and collectively strategize to make systemic improvements. All results will be shared on a regular basis with the PSCC.
