should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!

GRANT AGREEMENT—MI20
CONDITIONS:
1. This grant agreement is between the Community Foundation of Collier County (Foundation) and the Collier
County Board of Commissioners (Grantee).
2, The grant of$10,500 to be used solely for fulfillment of: Shade Structure for Tiger Tail Beach Playground with
acknowledgement plaque as detailed in deliverables of your grant request.
3. The grant period is from 2/6/2020—2/6/2021
4. Grantee will return this form signed as an acknowledgement of the grant award. Grantee warrants and
represents that its receipt of this grant will not adversely affect the Grantee's status as a public charity under
Section 501(c)(3)of the Internal Revenue Code.
FINANCIAL REPORTING:
5. Grantee will furnish to the Foundation any information concerning a change, or proposed change, in the (a)
Grantee's legal or tax status, (b) Grantee's executive or key staff responsible for achieving the grant purposes,
and (c)Grantee's ability to expend the grant for the intended purpose.
6. If Grantee's tax-exempt status changes, the Foundation reserves the right to request all remaining grant funds
returned immediately.
7. Any funds not used, or committed for the specific purpose of the grant, must be returned to the Foundation
unless otherwise authorized in writing.
8. Grantee must provide a written report on after the grant period describing conclusions, progress, and status of
grant objectives including how funds were expended to attain the objective.
9. The Grantee agrees to give the Foundation reasonable access to the grantee's files and records for the purpose
of making such financial audits,verifications,and investigations as it deems necessary concerning the grant,and
to maintain such files and records for a period of at least four years after completion ortermination of the project.
MARKETING AND PROMOTION:
By accepting this grant,your organization agrees to the following:
10. As a grant recipient,your organization agrees to acknowledge the Community Foundation of Collier County in
written and digital media communications, public announcements, publications or collateral materials
concerning any and all programs or initiatives supported by this grant. CFCC generally does not sponsor events
and should not be acknowledged on event invitations and materials, unless otherwise specified by CFCC.
Acknowledgements should read:Supported by a grant from the Community Foundation of Collier County.
11. Please use the attached media kit and Community Foundation logo (please remove other versions of our logo
from your flies). If you have any questions about marketing this grant or use of the logo, please contact Cindi
Withorn:cwithorn@cfcollier.org.
12. Send a press release, upon Collier County Board acceptance of the check, to the local media outlets. The
Grantee allows the Foundation to review and approve the content of any proposed publicity concerning this
grant prior to its release. Please pre-approve your press release with Cindi Withorn:cwithorn@cfcollier,org.
13. Provide updates of your program's progress on your social media outlets,such as Facebook,Twitter,
newsletters and your website. Remember to tag the Community Foundation on twitter @CFCCFL and
@communityfoundationcolliercounty on facebook.
14. Suggestions:Contact your Naples Daily News to submit an op-ed,write a story about the grant funded program
and submit it to your media outlets, and/or highlight your funded grant program at an event.

15. The Grantee will facilitate, where reasonably possible, requests from the Foundation to conduct on-site visits
that includes meeting 2 clients and requests to provide high resolution photos when requested. Such on-site
visits will be coordinated with the Grantee.
16. In addition,the Grantee allows the Foundation to reference and promote the Grantee,Its mission,the grant(s)
and/or use the content of any communication from the Grantee regarding the grant(s) (i.e. thank you letter
and/or final narrative report) in any and all media by the Foundation (i.e. including but not limited to written,
digital and social media to illustrate how grant was received and/or used by Grantee).
PROGRESS&FINAL REPORTS:
17. Provide a six month progress report,and a final report on your grant outcomes and expenses to the Foundation
and include a copy of all marketing or promotional materials.

COMPLIANCE:
18. The Foundation reserves the right to discontinue,modify or withhold any payments under this grant award or to
require a total or partial refund of any grant funds if, in the Foundation's sole discretion,such action is necessary:
(a) because you have not fully complied with the terms and conditions of this grant; (b) to protect the purpose
and objectives of the grant or any other charitable activities of the Foundation; or (c) to comply with the
requirements of any law or regulation applicable to you,the Foundation, or this grant.
19. This agreement, any alterations, variations, changes, modifications and/or waivers of this Agreement shall be
valid when they have been reduced to writing, signed by the Grantee and attached to the original Agreement.
Please signify your agreement to the foregoing terms and conditions by having a duly authorized officer sign and
return a copy to dgrundeman@cfcollier.org or mailing to our office(see below).
