Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the CMG
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's CMG
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip CMG
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on January 14,2020 and all changes made CMG /A is not
during the meeting have been incorporated in the attached document. The County 'option for
Attorney's Office has reviewed the changes, if applicable. this line.

EXTENSION AND AMENDMENT OF AGREEMENT BETWEEN
FLORIDA GULF COAST UNIVERSITY BOARD OF TRUSTEES AND
THE COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS EXTENSION and AMENDMENT AGREEMENT is made and entered into this
day of January,2020,by and between Collier County,a political subdivision of the State of Florida
("County"),and the Florida Gulf Coast University Board of Trustees.
RECITALS:
WHEREAS, the Parties entered into a Service-Learning Agreement dated January 24,
2017, a copy of which is attached to this Extension and Amendment, wherein the County agreed
to provide opportunities for FGCU students to complete service-learning activities for a term of
two years with two 1-year renewals;and
WHEREAS, the Parties entered into an Extension Agreement on January 22, 2019 to
extend the Agreement through January 24, 2020;and
WHEREAS, both Parties wish to extend and amend the Service-Learning Agreement on
the terms and conditions set forth below.
WITNESSETH:
NOW, THEREFORE, in consideration of the promises and covenants contained herein,
and other good and valuable consideration exchanged amongst the Parties, the Parties agree as
follows:
1. All of the above RECITALS are true and correct and are hereby expressly
incorporated herein by reference as if set forth fully below.
2. The Term of the Service-Learning Agreement is hereby extended through January
24,2021,then for an additional four years following the extension through January 24,2021. The
four-year term shall start on January 24,2021 and extend through January 24,2025. Accordingly,
Collier County will continue to provide service-learning opportunities for FGCU students through
January 24, 2025.
3. The provisions of Paragraph 1, Additional terms, of the Service-Learning
Agreement are hereby amended as follows:
The initial term of this Agreement shall be two years from the date executed below
(January 24, 2017). This Agreement may be renewed for two additional one-year
terms with a written amendment signed by both parties. Following the initial
Agreement term, this Agreement is valid for an additional four years (January 24,
2021-January 24, 2025). The Agreement may be further extended for two additional
one-year extensions with an amendment in writing executed by both parties.

4. Excepting the above, all other terms and conditions of the Agreement continue in
full force and effect.

hereunder set their hands and seals on the date and year first above written.
