EXECUTIVE SUMMARY
Recommendation to approve Change Order No. One to address redesign changes and add sixty-
nine (69) days to the final completion date of Agreement No. 18-7286, "Goodlette Frank Road at
Fleischmann Boulevard Roadway and Signalization Improvements," which had an expiration
date of September 3, 2019 (Project No. 60172.6).
OBJECTIVE: To complete the project to lengthen the northbound and southbound left turn lanes in the
median of Goodlette-Frank Road at the Fleischmann Boulevard/Zoo Entrance, add a right turn lane for
southbound traffic onto westbound Fleischmann Boulevard and upgrade the existing concrete strain pole
traffic signal to a steel mast arm structure by adding sixty-nine (69) days to the final completion date.
CONSIDERATIONS: On September 11, 2018, the Board awarded Invitation To Bid No. 18-7286
"Goodlette Frank Road at Fleischmann Boulevard Roadway and Signalization Improvements" (Project
No. 60172.6), to Florida Safety Contractors, Inc., for $888,697.31 (Agenda Item 16A.19). On October 22,
2018, staff issued a Limited Notice to Proceed and Purchase Order No. 4500189576 to Florida Safety
Contractors, Inc., with services commencing on May 6, 2019, and the final planned completion date of
work being September 3, 2019, for a total of 120 days. Currently the project has been on a work
suspension since September 19, 2019. The revised final completion date will be determined by the date
that work commences again.
On August 20, 2019, staff initially proposed a Change Order adding 39 days of project time and revising
the final completion date to October 12, 2019, but that Change Order was not presented to or approved by
the Board. While that proposed Change Order was being processed further construction delays relating to
Collier County Public Schools fiber lines occurred, and it was determined at least an additional 30 days of
project time is needed (for a total 69 days) to complete the project.
Although the Engineer of Record prepared the plans based upon as built information provided by the
utility providers, and despite soil borings and Subsurface Utility Engineering exploration, staff did not
discover unforeseen underground obstructions until construction began. As a result, staff is requesting
that the Board approve Change Order No. One adding sixty-nine (69) days to the agreement from
September 3, 2019, to account for the extended time the Engineer of Record needed to design alternative
solutions for avoiding the utility conflicts and preparing a subsequent sidewalk redesign.
FISCAL IMPACT: Project allowance funds in the amount of $1,418.24 will be reallocated to pay for
additional work required. Funds already encumbered in Gas Tax Fund 313 Project 60172.
LEGAL CONSIDERATIONS: This item is approved as to form and legality, and requires majority
vote for Board approval.-SRT
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated with this
action.
RECOMMENDATION: To approve Change Order One to address redesign changes and add sixty-nine
(69) days to Agreement No. 18-7286, "Goodlette Frank Road at Fleischmann Boulevard Roadway and
Signalization Improvements" (Project No. 60172.6) from September 3, 2019, to complete the Project, and
to authorize the Chairman to sign the attached Change Order.
