5. The Chairman's signature line date has been entered as the date of BCC approval of the JAK
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's JAK
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip N/A
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 9/10/19 and all changes made during the

RESOLUTION NO. 2019 - 1 57
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA APPOINTING MICHAEL M.
RUBENSTEIN, AND REAPPOINTING SUZANNE BRADACH, MICHAEL
J. PRIOLETTI, AND GARY D. LIND, TO THE HOUSING FINANCE
AUTHORITY.
WHEREAS, Collier County Ordinance No. 80-66 established the Housing Finance Authority of
Collier County, Florida(Authority) pursuant to Chapter 159, Part IV, Florida Statutes; and
WHEREAS, Ordinance No. 80-66 provides that the Authority shall be comprised of five members
with not less than three of the members knowledgeable in the fields of labor, finance or commerce; and
WHEREAS, four seats on this Authority will expire on September 11, 2019; and
WHEREAS, the Board of County Commissioners previously provided public notice soliciting
applications from interested parties; and
WHEREAS, the Authority has provided the Board of County Commissioners with its
recommendations for appointment and reappointment.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS
OF COLLIER COUNTY, FLORIDA, that MICHAEL M. RUBENSTEIN, representing the category of
finance, is hereby appointed to the Housing Finance Authority, to a four-year term which shall expire on
September 11, 2023; and
BE IT FURTHER RESOLVED that SUZANNE BRADACH, MICHAEL J. PRIOLETTI, and
GARY D. LIND, are hereby each reappointed to their respective seats on the Authority, to new four-year
terms expiring on September 11, 2023.
THIS RESOLUTION ADOPTED after majority vote on the 10''' day of September, 2019.
