EXECUTIVE SUMMARY
Recommendation to authorize the Chairman to sign the amendment between Collier
County Board of County Commissioners and the Area Agency on Aging of Southwest
Florida, Inc. and approve budget amendments to reflect the actual award amount for
the Older Americans Act for 2008.
OBJECTIVE: To amend the operating budget of the Older Americans Act to reflect the actual
FY08 allocation of the Older Americans Act.
CONSIDERATIONS: Collier County Services for Seniors program has been providing in-
home support services to Collier County's frail elderly for over 30 years through the Older
Americans Act program. An amended agreement has been received to reflect allocations for
the Title III-B and Title III-E Caregiver Support programs.
The total grant award is in the amount of $749,116 for the period January I, 2008 through
December 31,2008. Of the total grant award, $566,313 is the direct funding portion of the
County for case management, in-take screening, transportation and nutrition. The remaining
$182,803 will be retained by the grantor agency to pay for service delivery to Collier County
residents. The following table lists the funding amounts that will be received by the County
and what will be retained by the grantor agency:

The original agreement was approved by the Board on January 15,2008 as item 16D37. That
agreement included only the Older Americans Act Title C-l and Title C-2 Nutrition Program
components of the award. Funds for the nutrition program have already been approved and
budgeted. This amendment will now incorporate allocations for the Title III-B and Title III-E
Caregiver Support programs. In order to allow for continuous service to clients in the Title
III-B and Title Ill-E programs, funds were budgeted at the previous year's funding level.

However, partial funding of these programs will be retained by the grantor agency, thereby
reducing the county budget for these programs. The following budget amendments are

FISCAL IMPACT: The total funding in the amount of $749,116 is through the State of
Florida grants and aids appropriation administered by the Area Agency on Aging of
Southwest Florida, Fund 123-155972 will be reduced by $110,823.
LEGAL CONSIDERATION: This amendment has been reviewed by the Office of the
County Attorney and is sufficient for Board action.

STANDARD CONTRACT
Collier County Housing and Human Services
THIS CONTRACT is entered into between the Area Agency on Aging for Southwest
Florida, Inc., hereinafter referred to as the "agency", and the Collier County Housing and
Human Services, hereinafter referred to as the "recipient." This contract is subject to all
provisions contained in the MASTER CONTRACT executed between the agency and the
recipient, Contract No. 203M. I 08, and its successors, incorporated herein by reference.
The purpose of this amendment is to:
1, Increase contract amount adding Titles IIill and IIIE services.
2. Revise Rate Summary, Attachment III.
3. Include the Budget Summary, Attachment lV.
Section II, paragraph A and C are hereby revised to read:
A. Contract Amount:
To pay for services according to the conditions of A TT ACHMENT I in an
amount not to exceed $749.116.00, subject to the availability of funds.
C. Source of Funds:
The costs of services paid under any other contract or from any other source are
not eligible for reimbursement under this contract. The funds awarded to the
recipient pursuant to this contract are in the state grants and aids appropriations
and consist of the following:

Section 373 Caregiver Support Services for (J) family caregivers; (2) grandparents and older
individuals who are relative caregivers shall include:
. Assistance to caregivers in gaining access to services
. Information to caregivers about available services
. Counseling, organization of support groups, and caregiver training in the areas of
health,
nutrition and financial literacy.
. Respite care
. Supplemental services as available to complement the care provided by caregivers.
The recipient agrees to perform the services of this contract in accordance with all
federal, state, and local laws, rules, regulations and policies that pertain to Older
Americans Act funds, as well as the current Department of Elder Affairs Home and
Community-Based Services Handbook and the Department of Health and Human
Services, Office of Assistant Secretary, Administration on Aging, current Program
Instruction, AoA-PI-01-02.
Attachment I, Section III, paragraph D is hereby revised to read:
D. The recipient agrees to implement the services and rates and the distribution of
funds as detailed in the Rate and Budget Summaries, Attachment III and
Attachment IV, of this contract. Any changes in the amounts identified on the
Rate and/or Budget Summaries require a contract amendment.
This amendment shall be effective on January 1,2008. All provisions in the contract and
any Attachments thereto in conflict with this amendment shall be hereby changed to
conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be
performed at the level specified in the contract and the Master Agreement.
This amendment and all its attachments are herby made a part of the agreement.
