Attached, you will find two original release agreements as referenced
above (Agenda Item #16E5), which was approved by the Board of
County Commissioners on Tuesday, January 13, 2009.
The Minutes & Records Department has kept the final original for the
Board's official record.

Original document has been signed/initialed for legal sufficiency. (All documents to be signed by the
Chairman, with the exception of most letters, must be reviewed and signed by the Office of the
County Attorney. This includes signature pages from ordinances, resolutions, etc. signed by the
County Attorney's Office and signature pages from contracts, agreements, etc. that have been fully
executed by all parties except the BCC Chairman and Clerk to the Board and possibly State
Officials.)

In most cases (some contracts are an exception), the original document and this routing slip should be
provided to Sue Filson in the BCC office within 24 hours of BCe approval. Some documents are
time sensitive and require forwarding to Tallahassee within a certain time frame or the BCe's actions
are nullified. Be aware of our deadlines!

As you may be aware, the Florida Attorney General, along with the Florida Department of ~c-
Financial Services and the Office of Insurance Regulation, entered into a settlement agreement #~~
with Willis Group Holdings Limited. Our records show that your organization as a customer of '.~.; 7,
Willis Group Holdings Limited at some point between 1999 and 2004 is entitled to a share of C-,'

I We have utilized Willis Group Holdings Limited data for the premiums paid by its
Florida governmental entity policyholders during the relevant time period to determine the pro
rata share of the settlement proceeds for each governmental entity.

EXHIBIT A
RELEASE
BACKGROUND: By virtue of a settlement, Willis Group Holdings Limited (WILLIS)
has paid a sum of money for the benefit of its Florida government entity policyholder clients to
resolve the investigation. conducted jointly by the Florida Department of Financial Services
., ("DepllI1ment"), the Florida Department of Legal Affilirs, Office of the Attorney General, and
' the Office of Insurance Regulation regarding undisclosed compensation allegedly received by
WILLIS from insurers on accountS such as yours. WILLIS has admitted no wrongdoing or
liability in the settlement. Please be advised that only those Florida governmental entity
policyholder clients who execute this Release are entitled to receive payments under the
settlement.
RELEASE: For and in consideration of the sum of $ I 55.55, and other good and valuable,
consideration in hand paid by WILLIS to the Department for distribution to the Releasor, the
Releasor, by executing this Release for itself and its respective divisions, subdivisions, officials,
officers, executors, attorneys, administrators, agents, successors, and assigns, hereby waives,
releases, remises, forgives, acquits" and forever discharges WILLIS, including any parent,
subsidiary, or affiliated company of WILLIS, and their respective predecessors, successors,
assigns, former and current officers, directors, agents, representatives, officials, attorneys, and
employees of and from all manner of actions, cause and causes of action, suits, damages,
whether compensatory, consequential or punitive, awards, claims, demands, and judgments
whatsoever, whether sounding in contract, quasi-contract or tort, in law or in equity, or any right
or participation in any class action, which Releasor now has against WILLIS or which it ever
had, or which Releasor's successors or assigns hereafter can, shall or may have regarding
WILLIS's receipt of undisclosed compensation from insurers through the date of the execution
of this Release. Releasor acknowledges that, in making this release, WILLIS does not admit any ,
liability to Releasor.
