1. Original document has been signed/initialed for legal sufficiency. (All documents to be
signed by the Chairman, with the exception of most letters, must be reviewed and signed ~\
by the Office of the County Attorney. This includes signature pages from ordinances, C!J
resolutions, etc. signed by the County Attorney's Office and signature pages from
contracts, agreements, etc. that have been fully executed by all parties except the BCC
Chairman and Clerk to the Board and ossibl State Officials.)
2. All handwritten strike-through and revisions have been initialed by the County Attorney's N/A
Office and all other arties exce t the BCC Chairman and the Clerk to the Board
3. The Chairman's signature line date has been entered as the date ofBCC approval of the
document or the final ne otiated contract date whichever is a licable.
4. "Sign here" tabs are placed on the appropriate pages indicating where the Chairman's r&

5. In most cases (some contracts are an exception), the original document and this routing slip "
should be provided to Sue Filson in the BCC office within 24 hours of BCC approval. \V
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCe's actions are nullified. Be aware of our deadlines!

RESOLUTION NO. 2009-216
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS
OF COLLIER COUNTY, FLORIDA, TO APPOINT JILL KOBE TO
THE PELICAN BAY SERVICES DIVISION BOARD.
WHEREAS, Collier County Ordinance No. 2002-27, as amended, which established the
Pelican Bay Services Division Board, provides that the Board shall consist of nine (9) members
with a residential interest within the unit and two (2) members who are representative of the
commercial and business interests within the unit; and
WHEREAS, there is currently a vacancy on this Board for the commercial interest category;
and
WHEREAS, the Board of County Commissioners previously provided public notice
soliciting applications from interested parties; and
WHEREAS, the Pelican Bay Services Division Board has provided the Board of County
Commissioners with its recommendation for appointment to their Board.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that Jill Kobe, representing the
Commercial category, is hereby appointed to the Pelican Bay Services Division Board to fulfill
the remainder of the vacant term, said term to expire on March 31, 2013.
This Resolution adopted after motion, second and majority vote on this the 15TH day of
September, 2009.
