7. In most cases(some contracts are an exception),the original document and this routing slip CD
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 06/25/2019 and all changes made during CD

GRANT AGREEMENT—Trees for Immokalee Parks
CONDITIONS:
1. This grant agreement is between the Community Foundation of Collier County(Foundation)and Collier County
Board of County Commisioners(Grantee).
2. The grant of$10,000 to be used solely for fulfillment of:Trees for Immokalee Parks as detailed in deliverables
of your grant request.
3. The grant period is from May 1,2019-Dec.1,2019
4. Grantee will return this form signed as an acknowledgement of the grant award. Grantee warrants and
represents that its receipt of this grant will not adversely affect the Grantee's status as a public charity under
Section 501(c)(3)of the Internal Revenue Code.
FINANCIAL REPORTING:
5. Grantee will furnish to the Foundation any information concerning a change, or proposed change, in the (a)
Grantee's legal or tax status, (b) Grantee's executive or key staff responsible for achieving the grant purposes, E
and(c)Grantee's ability to expend the grant for the intended purpose.
6. If Grantee's tax-exempt status changes, the Foundation reserves the right to request all remaining grant funds v
returned immediately.
7. Any funds not used, or committed for the specific purpose of the grant, must be returned to the Foundation g
unless otherwise authorized in writing. (9
8. Grantee must provide a written report on after the grant period describing conclusions, progress, and status of m
grant objectives including how funds were expended to attain the objective.

@communityfoundationcolliercounty on facebook.
14. Suggestions:Contact your Naples Daily News to submit an op-ed,write a story about the grant funded program
and submit it to your media outlets,and/or highlight your funded grant program at an event.

15. The Grantee will facilitate,where reasonably possible,requests from the Foundation to conduct on-site visits
that includes meeting 2 clients and requests to provide high resolution photos when requested. Such on-site
visits will be coordinated with the Grantee.
16. In addition,the Grantee allows the Foundation to reference and promote the Grantee,its mission,the grant(s)
and/or use the content of any communication from the Grantee regarding the grant(s) (i.e. thank you letter
and/or final narrative report) in any and all media by the Foundation (i.e. including but not limited to written,
digital and social media to illustrate how grant was received and/or used by Grantee).
