EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to sign an amendment between
Collier County Board of County Commissioners and the Area Agency on Aging for
Southwest Florida, Inc., which reflects additional grant funding in the amount of $77,999
for the Older Americans Act Program for 2009.
OBJECTIVE: To amend the 2009 Older Americans Act grant agreement between the Board of
County Commissioners and the Area Agency on Aging. This amendment provides an increase of
$77,999 in additional funding.
CONSIDERATIONS: Collier County Services for Seniors program has been providing support
services to Collier County's frail elderly for over 30 years through the Older Americans Act
program. The services coordinated by staff in the Services for Seniors program allows area
seniors to remain in their homes and age with dignity by providing services such as homemaker,
personal care, respite, emergency alert response, medical supplies and nutritional services which
include meals at congregate dining sites and home delivered meals. Collier County Services for
Seniors has received additional allocations in the following programs under the Older Americans
Act; Title III-B Homemaker, Title III-C] Congregate Meals and Title III-E Caregiver Support, in
the amount of $77,999. Therefore, an amendment to the original agreement must be approved to
accept the additional funding. The original agreement was approved by the BCe on 2/1 0/2009
item 16D7.
The total grant award now amounts to $847,809 and is for the period January 1, 2009 through
December 31,2009. The amendment for the additional funds must be signed by the Chairman.

LEGAL CONSIDERATIONS: This item is not quasi judicial and as such ex parte disclosure
is not required. This item requires majority vote only. This item is legally sufficient for Board

CONTRACT
Collier Coontv Housill2 and Human Senrices
TIDS CONTRACT is entered into between Area Agency on Aging for Southwest Florida,
Inc., hereinafter referred to as the "Agency", and Collier County Hourne: and Human
Senices, hereinafter referred to as the "Vendor". This contract is subject to all provisions
contained in the CONTRACT executed between the Agency and the vendor, Contract No.
OAA 203.09, and its successor, incorporated herein by reference.
1. The purpose of this amendment is to revise the Attachment III, Exhibit-l and Budget
Summary, Attachment VII. The Collier County budget shall increase by the following
amounts:
a. Title 3B service funds increase by $68,000.00,
b. Title Cl funds increase by $3,000.00,
c. Title 3E funds increase by $2,999.00, and
d. Title 3ES funds increase by $4,000.00.
This amendment shall be effective on March 30, 2009. All provisions in the contract and any
Attachments thereto in conflict with this amendment shall be hereby changed to conform with
this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the coop-act.
This amendment and all its attachments are herby made apart of the agreement.
