OBJECTIVE: To obtain. approval froni the Boar~of County Commissioner to recognize
revenue from the Community' Foundation of Collier County to be . used to -provide services to
enhance the health, safety and welfare of seniors. inlmmokalee. . .
-, CONSIDERATIONS: -The lmmokalee Initiative of theColnmunityFoundation ofeollier
County in conjunction with th~ Women:s Initiativehas received a grant in the amount of$25,QOO
from the Agua Fund. - These funds will serve a two-part purpose. An amount equal to $12,500
will be uSyd by the. Immokalee Initiative. to hire a consultant to prepare a needsassessmeIlt for
Collier County.. tha.t will provide uP. dated _demo ographic. info. rm.atio.n, identification.. o..f heeded
services. and' the gaps, a well. as an assessment of seniors who are not being reached that have
needs. The second $12;500 of the award has been given to' the Collier County Housing and
Hl;1manServices Department to provide services to seniors in Immokalee with the greatest need.
. The Housing and Huinan Services Department has provided the CQmmunity Foundation With a
list()feligible services that wi II be.providedwith the fUnding.
GROWTH. MANAGEMENT: There is no growth management. impact rromthis
recommendation.
FISCAL IMP ACT: The FY09 budget in the Services for Seniors fund 123 will increase by
$ 12,500.

The Community Foundation of Collier County has been working on a
collaborative effort regarding senior services in our Community, The Women'g
Initiative has spearheaded the effort to bring social service providers together to
identifY needs and gaps in services, A Leadership Council has been formed by
community stakeholders to identifY the outcomes of this collaboration,
The IrnmokaleeJnitiative of the Community Foundation in conjunction with the
Women's Initiative has received a grant from the Agua Fund, lnc, in the amount
of $25,000. The purpose of the grant is twofold, first, it will provide funding of
$12,500 to hire a consultant to prepare a needs assessment for Collier County
that will provide updated demographic information, identification of needed
services and the gaps, as well as an assessment of seniors who are not being
reached that have needs. This funding will be leveraged with funds from the
Area Agency on Aging for the assessment.
The second part of the grant is for $12,500 to provide services to seniors
with the greatest needs in Irnmokalee, The needs in Irnmokalee are
critical due to the fact that one in four seniors live below the poverty
leveL In addition, 61 % have a disability and 64% do not live with a
spouse, Many are the primary caregiver for grandchildren; 45% of the
grandparents who live in households with children under the age of 18
are responsible for the grandchild.
The Community Foundation has partnered with the Collier County
Housing and Human Services DepaTtment to assess the most critical
needs of their senior clients, Attachment 1 outlines the services that will
be provided to seniors in Irnmokalee, We appreciate your help in
delivering these services to those most in need,

ELIGffiLE SERVICES
Introduction
The Senior residents in Irnmokalee face many fmancial challenges on a daily basis. Medicare now requires clients to pay
a co-pay when filing needed prescriptions. With many SeniOlS in need of multiple prescriptions each month, the co-pay
requirement has placed a financial burden on these vulnerable citizens. Additionally. many doctors who provide care for
Medicare patients are located in Lee County. The Collier Area Transit system does not provide transportation to Lee
County, further burdening the Seniors in Immokalee, One goal of this program is to provide basic household supplies, gas
and food cards and other needed household items to Seniors in need in Immokalee.
Many clients receive only home delivered meals and are now so frail that they require additional services to remain in
their home, However, due to demand, the waiting list for these services may be over a year long, Many of these clients
are alone with little or no family support. Given the importance of communication, the second goal of this program is to
provide funds for emergency alert systems for clients in need.
--- _. The downturn in the local ~d national economy bas forced providers of service to cut back and elimirurte once common
practices. Home health aides no longer provide the cleaning supplies needed to service the elderly in Irnmokalee.
Cleanliness is an important part of assisting Seniors who desire to age in their home, With many clients incontinent, the
need increases for increased laundry supplies and funds to pay for laundry machines and laundry supplies. The third goal
of this program is to assist clients with laundry supplies and funds for laundry machines to keep the Seniors clean and
safe. If a home is especially in need of cleaning, an outside cleaning service may be hired.
Finally, this program intends to address the variety of individual needs of the Seniors in Immokalee. The program is
intended to meet those needs by assessment and, therefore, it is difficult to predict precisely what items will be needed.
Eligible items may include, but not be limited tn, cleaning services, fmniture such as beds, mattresses, bedding, tnwels,
shelving, pet supplies, Ensureā¢, incontinence products, or any other product a Case Manager determines needed for the
health, safety and welfare of the client being assisted.

The total budget for this program is Twelve Thousand Five Hundred and OOtlOO Dollars ($12.500.00), Funds will be
