EXECUTIVE SUMMARY
Recommendation to approve the after - the -fact submittal of a grant application for
the U. S. Smokeless Tobacco Polaris Ranger Donation Program for Emergency
Medical Services.
OBJECTIVE: To receive after- the -fact approval from the Board of County
Commissioners to submit a grant application to the U. S. Smokeless Tobacco Polaris
Ranger Donation Program. This is a national program to provide off-road utility vehicles
to assist in emergency response.
CONSIDERATIONS: On December 17, 2008, the County Manager approved the
electronic submittal of a grant application, in accordance with Collier County CMA #5330
which authorizes the County Manager to approve the submittal of grant applications with
subsequent BCC action at the next available Board meeting to ratify the approval as "after -
the- fact."
This program was announced on November 4, 2008 with a deadline of January 9, 2009.
There was only a 30 day window between receipt of the announcement and the deadline
for the December 16d' BCC meeting to allow for the research and development of this
application. In addition, the grant writer for EMS was committed to other higher priority
projects during this period. Due to the time constraint, it was necessary to request the
approval of the Manager to submit this application to meet the deadline.
Should this application be successful, the awarded utility vehicle would be used for search
and rescue, emergency response during public events and as a support vehicle for rehab
during wildfires and other occurrences requiring emergent care. it is expected that
approximately 65 to 75 of these vehicles will be awarded in 2009. There are no matchin-,
funds required.
FISCAL IMPACT: There is no Fiscal Impact.
LEGAL CONSIDATIONS: This item has been reviewed and approved by the County
Attorney's Office, is not quasi- judicial and requires no ex parte disclosure, requires only a
majority vote for approval, and is otherwise legally sufficient for Board action. —SRT
GROWTH IMPACT: There is no Growth Management Impact resulting from this
action.
RECOMMENDATION: That the Board of County Commissioners approves the after -
the -fact submittal of the grant application for the U. S. Smokeless Tobacco Polaris Ranger
Donation Program for Emergency Medical Services.

emergency medical services for all of Collier County, Florida. EMS provides service to the City of Naples, City of Marco Island and Everglades City,
as well as all of the unincorporated areas of the county. Collier County stretches from the Gulf of Mexico on the west to the Broward County line on
the east and from Lee County to the north all the way to Monroe County to the south, and includes the Florida Everglades and Big Cypress Swamp.
Consequently, a large portion of the seance area is heavily - wooded and /or low -lying wetlandslgresslands.

permanent population of 251,000 (based on 2000 census). Approximately 20,000 workers
increase the population each day. In addition, the moderate climate, miles of beaches, three
airports and numerous marinas attract a multitude of visitors each day, swelling the population .

Corer EMS is charged with Emergency Service Function 9, which Is search and reme for the county. Consequently, search and rescue operations
would be a primary purpose. Hunters, campers and hikers can become disoriented In the woods and swamps of to area. In addition, children may
wander offend became lost. Having a medic available on these operations could hasten the search and possibly improve a survivcn's medical
outcome. Also, because there are four small airports serving the county, as well as the large international airport in neighboring Lee County, such a
vehicle would be valuable for search and rescue operations when a plane crashes In a wooded area and subsequently being able to get the parent
out of an area in which an ambulance could not venture.
Another important purpose for this vehicle would be to provide quick response to medical emergencies during events. Corer County and the
municipalities within the county host hundreds of events each year, including art Taira; nationally televised golf and tennis toumamehtts, swamp buggy
raoss and monster truck rallies; various festivals and the county fair. These events draw anywhere from 1,000 to over 70,000 people during the
` course of the event A list of many of these events, along with attendance estimates is attached.

Being charged with search and rescue and not having a vehicle to traverse the vast and rugged terrain of the county oadainly impedes our aogy, to
perform this function. The polaris Ranger, property equipped for medical emergencies, would greatly enhance our service to tie community. Due
to the size of an ambulance, it Is very difficult, if not impassible to maneuver through crowds to attar d a patent It is certainty Impossible to utilize an
ambulance for search and rescue. In addition, during the dry season (usually between January through April or May) there are many brush and
forest fires throughout the county, somaWnes involving thousands of acres and taking perhaps a week or longer to bring under control, The Pola s
Ranger would be an ideal vehicle to serve in fire rehab. Medics would be available closer to the scene and provide mono immediate oxygen and
other treatment to Wghters as well as any civilians who may have been overtone with smoke or Injured in to ire. The availability of this vehicle
would also free up other resources for hospital transports. Copies from a presentation documenting one of the severe broshfires of the past Is

Thera have been several Instances where having the use of the Polaris Ranger would have benefited our ability to respond to emergencies. There
have been several plane crashes at one of the small airports that is located in a heavily- wooded area. Fortunately, there were survivors. Sul these
waking, wounded survivors treaded in different directions and had to be located on foot The use of this vehicle net only would have slowed for
quicker location of the survivors, but also allowed us to transport them to an area where they could receive more sophisticated treatment and if
necessary, be transported to a hospital.
There have been numerous emergencies during some of the crowded events that take place every year, Several of those have been cardiac related
and the ability to nave through the crowds quickly and provide basic Re support, which is so critical In those early minutes of a cardiac emergency,
would certainly enhance our response and ensure a better outcome for the patient
As indicated earlier, search and rescue is a function of EMS and we are seeing a greater frequency of Atalxeimer's patients leavng their homes or
caretakers, wandering off and becoming lost. Several have been found in wooded locations. Luckily, these patients have survived, but again, the
Polards Ranger would have provided us a better means of searching these areas and hopefully reduce the likelihood of hypothemde, dehydration,
insect Was or even more life- threatening scenarios that might occur. In a most recent occurrence, an elderly, mentally confused patient wandered
out of a facility, resulting in a massive search by al and on foot. The patient was not located for three days, The availability of the Polaris Ranger
would have greatly benefited Ihis search.
Collier County is connected to the east coast of Florida via Interstate 75 (Alligator Alley). Motor vehicle accidents are frequent and much of this
Interstate runs through remote areas of the county, where a deep canal runs parallel to the road. Approximately sic months ago an accident
occurred, launching the vehicle nearly all the way across the canal. The survivors were able to exit the vehicle and dimb the bank on the opposite
side of the canal. Had the Polaris Ranger been available to us, it could have crossed the canal at an access point and Oransporled the accident
victims to safety. Other than bringing in and launching a boat, the only other option available in this circumstance Is to swim the victims back to the
other side. This area, incidentally, is heavily populated with alligators (thus tie name Alligator Alley). Obviously an expedient route to safety is a
high priority for the safety of these motor vehicle aocident Actims.

Neatly 80% of the Collier County EMS budget Is to cover personnel costs. The remaining 20% is ublized to cover operating costs and capital
purchase to room wom equipment The budget is largely dependent on revenue. With the majority of the county being agricultural, there is a
large influx of both a migrant population and Indigent patients. As such, Collier County experiences a high volume of wdle -offs for ambulance
services, In addition, ambulance fee collections are taking a negative turn due to the trend by insurance companies to short-pay claims (payments
substantially less than that billed), unemployment and people's lose of group Insurance, and the economy in general. Most individuals will choose to
keep their fights on and feed their families before paying for an ambulance ride. As the revenues decrease, the offsetting funds must come from ad
valorem taxes, which are already strained due to the growth of the area and demands on the Infrastructure of the County. Especially al fficant b
the passage of a referendum in the Slate of Florida lo reduce property taxers that will crople county agencies from obtaining necessary funding
through ad valorem taxies. In fact the budget for FY09 required a two percent reduction over the FY08 budget and it Is antiolpatsd that even further
reductions will be required for FY10. This in itself vdll seriously hamper any efforts to replace wom and unsafe equipment, let alone expand
resources.

Program Grant Application,
The U.S. Smokeless Tobacco Polaris Ranger Donation Program was created for the purpose of
providing off -road utility vehicles to eligible organizations to assist in emergency response.
Approximately 65 -75 vehicles are expected to be awarded in 2009,
Should this grant application be successful, the vehicle would be used for search and rescue,
emergency medical services at festivals and fairs, and as a support vehicle during wildfires and other
natural events requiring emergency medical services.
The enclosed application is due to U.S. Smokeless Tobacco's Operation Ranger on January 9, 2009.
Because higher priority projects required the attention of the grant writer for Emergency Management,
and the program only allowed one month for development of the grant application, staff was unable to
devote the necessary time for research and development of the grant application until this month. Due
to this time constraint, it is necessary to request your approval of the application to be followed by
after -the -fact approval of the Board of County Commissioners during the January 13th meeting.
