Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the JAK
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's JAK
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this JAK
routing slip should be provided to the County Attorney Office at the time the item is
input into MinuteTraq. Some documents are time sensitive and require forwarding
to Tallahassee within a certain time frame or the BCC's actions are nullified. Be
aware of your deadlines!

RESOLUTION NO. 2019 -1 5
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, APPOINTING JOAN LOUISE
HOCHSCHILD AND REAPPOINTING CONSTANCE L. BETTINGER TO
THE LIBRARY ADVISORY BOARD.
WHEREAS, Collier County Ordinance No. 88-71, confirmed the establishment of the
Library Advisory Board, which advises the Board of County Commissioners on the operations
and proposed budget for the Collier County Library System; and
WHEREAS, Ordinance No. 88-71, as amended, provides that the Advisory Board shall
consist of five members; and
WHEREAS, two seats on this Advisory Board expired on December 31, 2018,
representing Commission Districts 1 and 3; and
WHEREAS, the Board of County Commissioners previously provided public notice
soliciting applications from interested parties; and
WHEREAS, the Library Advisory Board has provided the Board of County
Commissioners with its recommendations for appointment.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that JOAN LOUISE
HOCHSCHILD, resident of Commission District 1, is hereby appointed to the Library Advisory
Board to serve a four-year term expiring on December 31, 2022; and
BE IT FURTHER RESOLVED that CONSTANCE L. BETTINGER, resident of
Commission District 3, is hereby reappointed to the Library Advisory Board to serve a four-year
term expiring on December 31, 2022.
THIS RESOLUTION ADOPTED upon majority vote on this 22nd day of January 2019.
