Utilizing a portion of previously awarded FTA Grant Funds 10) Recommendation to award RFQ #09 -5220, Vanderbilt Beach MSTU Roadway Grounds Maintenance to Florida Land Maintenance, Inc. d/b /a Commercial Land Maintenance, Inc. in an estimated annual amount of $61,079. Additional expenditures include repair of an irrigation system damaged by vandalism and landscape refurbishment projects 11) Recommendation to approve the FYI 1 Program of Projects and authorization to submit an application for Federal Transit Administration Section 5307 FYI 1 Grant Funds estimated at $2,578,927. Proposed projects include: bus shelter repair and /or replacement, security cameras & lighting, ADA paratransit services, replacing 2 diesel buses, bus trolley wrap, preventative maintenance and to help fund bus route operations B. Outreach activities focus on life skills with an emphasis on youth leadership and citizenship, volunteer recruitment and training and management 2) Recommendation to approve and authorize the Chairman to sign four (4) releases of lien for deferral of 100% of Collier County impact fees for owner occupied affordable housing dwelling units that were transferred from the developer to the new owner. Properties and Impact Fees include: Lot I Regal Acres ($14,591.26); Lot 6 Regal Acres ($14,591.26); Lot 7 Regal Acres ($14,591.26); Lot 10 Regal Acres ($14,591.26) 3) Recommendation to approve and authorize the Chairman to sign five (5) releases of lien for deferral of 100% of Collier County impact fees for owner occupied affordable housing dwelling units that have been repaid in full. Assistance provided through SHIP or the ADDI in the amount of $10,000 for properties that include: Lot 12, Block 1, Burdale; Independence Phase One Lots 49, 50, 55 and 62; Lot 15, Block 194, Golden Gate, Unit 6; Naples Trace, Unit 202 ($4,255); Lot 10, Collier Village ($8,240); Trafford Pine Estates ($14,300) 5) Recommendation to approve and authorize the Chairman to sign six (6) Satisfactions of Mortgage for owner occupied affordable housing dwelling units that have been repaid in full to Collier County. SHIP Funding Assistance on property that includes: Lot 71, Trail Ridge ($3,000); Lot 114, Liberty Landing ($20,250); Lot 48, Charlee Estates ($23,750); Lot 27, Block A, Habitat Village ($20,000); Lot 2, Block 1, Gulf Acres ($4,640); Lot 2, Block 1, Gulf Acres ($14,702) 6) Recommendation to approve and authorize the Chairman to sign three (3) Mortgage and Note Modification Agreements for Single Family Rehabilitation to correct the total funds disbursed on previously recorded security instruments. Mortgage & Note Modification Agreements for property with Folios #63854840000, #60980320002 and #40351120000 7) Recommendation to approve and authorize the Chairman to sign three (3) lien agreements for deferral of 100% of Collier County impact fees for owner occupied affordable housing dwelling units located in Collier County. Approval of this item will transfer previously approved deferral agreements from developer to owner occupants with a continuing fiscal impact of $45,083.78. For the properties and deferral amounts: Lot 72, Liberty Landing ($15,246.26); Lot 133, Liberty Landing ($15,246.26); and Lot 4, Regal Acres ($14,591.26) Page 12 January 11, 2011 8) Recommendation to approve and authorize the Chairman to sign an agreement in the amount of $258,667 with the Agency for Health Care Administration (AHCA) and Collier Health Services (CHS) to participate in the Medicaid Low Income Pool Program (LIP). Participation in this program will generate an additional $452,907 in Federal matching funds to be paid directly to CHS for services for the most medically needy in Collier County. Expanding comprehensive primary health care services for the medically needy at the Mike Davis Medical Center and area sites 9) Recommendation to approve and authorize the Chairman to sign certifications required by the State of Florida, Department of Children and Families (DCF) in order to be eligible to be awarded 2011 Challenge Grant funding. Grant funds of $63,397 will be used by St. Matthews House, The Shelter for Abused Women and Children and Youth Haven, Inc. 10) Recommendation to accept an additional list of equipment to be purchased with the Health Care and Other Facilities Special Congressional Initiative Earmark from the US Department of Health and Human Services accepted by the County on September 29, 2009, Agenda Item #16D6. Unused funds due to competitive purchasing will be used to purchase additional equipment benefitting local agencies 11) Recommendation to approve and authorize the Chairman to sign an Agreement with the David Lawrence Center to operate a post - adjudication, court supervised, substance abuse treatment program known as the Collier County Drug Court Program. Funding has been provided through Grant #2010 -DC -BX -0016 from U.S. Department of Justice, Bureau of Justice Assistance. No general funds are associated with this project. A two -year contract period ending January 10, 2013 12) Recommendation to approve an amendment to the Homelessness Prevention and Rapid Re- Housing Program (HPRP) Administrative Plan, which provides guidance on the programmatic design, eligibility guidelines and assistance limitations for agencies participating in the HPRP program. The proposed amendment would increase the amount Page 13 January 11, 2011 of medium term HPRP financial assistance a client could receive from the maximum $3,000 to $5,000 per client over an 18 month period. For homeless prevention assistance including housing relocation and stabilization services 13) Recommendation to approve and authorize the Chairman to sign an amendment to a 2009 -2010 Community Development Block Grant (CDBG) Subrecipient Agreement with the Shelter for Abused Women & Children (SAWCC) approved September 15, 2009. The amendment revises Exhibit A, Section B - the Budget, updating the budget to facilitate reimbursement. Amendment #1 to Agreement #B- 09 -UC -12 -0016 14) Recommendation to reject solicitation (ITB) # 11 -5611 for East Naples Community Center Roll Down Shutters and associated proposals that were submitted and authorize the Purchasing Department to re- solicit for services. FYI I impact associated with solicitation is $166,954. 15) Recommendation to approve an award for Invitation to Bid (ITB) # 10- 5568 -R for Fitness Equipment Repair and Purchase to Tropic Gym Tech, LLC.
