Subject: Service Development Grant 436923-1-84-01 JPA G0575 Extension
Dear Mr. Felter:
Collier County currently has funding through Service Development Grant 4369234-84-01 JPA G0575 for
the development and implementation of a Mobile Application and the development of a marketing plan.
This grant is set to expire on December 31,2018 and we are writing to request an extension of the FY15/16
JPA to December 31, 2019.
The Agreement was executed December 1, 2015 and it took some time for staff to develop the scope for
the procurement process due to the research and coordination required to ensure that the proposal
submitted covered the desired features for the mobile application. We are currently finalizing the
procurement process and expect to have a contract in place with the selected contractor by September
2018.
The schedule for implementation is approximately 159 days and the Marketing for this project will require
111 days of the project implementation. The project will be fully implemented by July 2019. This
extension will allow time for the development, testing, implementation and marketing of the mobile
application. The extension would also provide for time to sufficiently close out the grant.

Project Description: Development and Implementation of a Mobile Application

The time allotted for the completion of services or goods on the subject agreement dated December
15, 2016 and any supplementals thereto will expire on December 31, 2018.
In accordance with Section 18.00 of the Joint Participation Agreement, a time extension may be
requested. Please complete, sign, date and return this form to me.
It is requested that the Joint Participation Agreement for the subject project be extended until
December 31, 2019.
The reason for the Time Extension request must be included in a letter to the Department from
the Director/Manager/CEO of the organization. A detailed summary of the reason for the Time
Extension and a schedule for completing the project must be included. Time Extensions will
not be granted for billing purposes if the project is complete.
