EXHIBIT A -1 Contract Amendment # 1 to Contract # 09 -5126 16E4
"Annual Contract for the Purchase and Renting of All Collier County Uniforms"
This amendment, dated 43r , 20 1 ". to the referenced agreement shall be by and
between the parties to the original Agreement, Unifirst Corporation (to be referred to as
"Contractor") and Collier County, Florida, (to be referred to as "Owner ").
Statement of Understanding
RE: Contract # 09 -6126 - Annual Contract for the Purchase and Renting of Collier County Uniforms
In order to continue the services provided for in the original Contract document referenced
above, the Contractor agrees to amend the above referenced Contract as per the modified
language below:
IV. Term: The contract shall be for a one (1) year period, commencing on date of award by the
Board of County Commissioners, and terminating one year from that date in 2010. The County
reserves the right to renew said contract with consent of the Contractor for an additional thfee
five (5} one -M year contract periods under the same terms and
conditions. No additional extension periods are available unless approved by the Board
of County Commissioners.
New Term: Contract Finalization: two weeks prior to the contract end date, a notification
letter will be sent to UniFirst including a pick up date scheduled for all uniforms and
facility service products (flexible to meet both the County and UniFirsD. County
departments would have the option to purchase uniforms; the pricing in the bid for the
garments would be used.
Note: Language deleted has been struck-t-hmugh. New language has been underlined.
All other terms and conditions of the agreement shall remain in force.

authorized person or agent, hereunder set their hands and seals on the date(s) indicated below.
