COMMUNITY CARE FOR THE ELDERLY PROGRAM
COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices
of Southwest Florida ("Agency") and Collier County Board of County Commissioners, ("Recipient"), amends agreement
CCE 203.12.
The purpose of this amendment is to increase Collier County allocations by $11,479.00 affecting Year Two, 2013-2014,
and revise ATTACHMENT III EXHIBIT—I and ATTACHMENT VIII,Budget Summary.
Additionally,this amendment amends the Standard Contract with additions of subsections 7.2.5 through 7.2.8.
Line denotes completion of above summary
The Standard Contract is hereby amended to read as follows:
7.2.5 As defined in Sections 119.0701 and Section 119.011(2), Florida Statutes,the Recipient shall keep and maintain
public records that ordinarily and necessarily would be required by the public agency in order to perform services.
7.2.6 The Recipient shall provide the public with access to public records on the same terms and conditions that the
public agency would provide the records, and at a cost that does not exceed the cost provided in Chapter 119,
Florida Statutes, or as otherwise provided by law.
7.2.7 Public records that are exempt or confidential and exempt from public records disclosure requirement are not to be
disclosed except as authorized by law.
7.2.8 All requirements shall be met for retaining public records and transfer, at no cost to the agency all public records
in possession of the Recipient upon termination of the contract and destroy any duplicate public records that are
exempt or confidential and exempt from public records disclosure requirements. All records stored electronically
must be provided to the public agency in a format that is compatible with the information technology systems of the
public agency.
This amendment shall be effective on August 8, 2013. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.

Florida Statutes, or as otherwise provided by law.
7.2.7 Public records that are exempt or confidential and exempt from public records disclosure requirement are not to be
disclosed except as authorized by law.
7.2.8 All requirements shall be met for retaining public records and transfer, at no cost to the agency all public records
in possession of the Recipient upon termination of the contract and destroy any duplicate public records that are
exempt or confidential and exempt from public records disclosure requirements. All records stored electronically
must be provided to the public agency in a format that is compatible with the information technology systems of the
public agency.
This amendment shall be effective on August 8, 2013. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
