7. In most cases(some contracts are an exception),the original document and this routing slip JAK
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwar 'rig to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aw a of your deadlines!
8. The document was approved by the BCC on 9/10/ 3 and all changes made during the JAK
meeting have been incorporated in the attached document. The County Attorney's
Office has reviewed the changes,if applicable.

MEMORANDUM OF UNDERSTANDING
BETWEEN
THE COLLIER COUNTY BOARD OF COUNTY COMMISSIONERS
AND
THE EARLY LEARNING COALITION OF SOUTHWEST FLORIDA, INC.
This Second Extension of Memorandum of Understanding (also referred to as the
"Extension") is entered into this jo\--‘,\ day of Se-exti, -, 2013, between the Collier
County Board of County Commissioners, hereinafter referred to as "COUNTY" and the Early
Learning Coalition of Southwest Florida, a Nonprofit Corporation registered under the laws of
Florida Chapter 617, operating under the laws of the State of Florida and, hereinafter referred to
as the "COALITION."
This Second Extension shall begin October 1, 2013, and end September 30, 2014, unless
terminated by either party as provided for herein.
1. The Memorandum of Understanding, as previously extended, a copy of which is
attached hereto as Exhibit A, is hereby extended to September 30, 2014.
2. All terms and conditions of the Memorandum of Understanding, as previously
extended, shall continue to be in full force and effect, except for the following changes:
A. Payments to the COALITION during the Extension Term shall be on a
reimbursement basis for work completed and/or services delivered for a total amount
not to exceed $75,000.00, that will cover the match for the cost of care for eligible
Collier families in the School Readiness program;
B. During the Extension Term the COALITION will provide the following services to
children and families residing in Collier County: Resource & Referral, Eligibility and
Enrollment, Reimbursement to Child Care providers, Children's Developmental
Screenings, Provider Monitoring, Inclusion, Infant & Toddler and Training.
C. During the Extension Term, the COALTION will invoice the COUNTY for monthly
expenditures incurred beginning in October 2013 through September 30, 2014, for the
preceding month's expenditures in a form acceptable to the County;

e. To provide the appropriate supporting back-up documentation for clients enrolled,
including a quarterly report showing the client demographics with the number of Collier
County residents impacted by the program;Iist of employers of clients receiving school
readiness services; and other appropriate evidence of client movement toward
economic self sufficiency. The Coalition shall maintain all other records required by the
County in connection with the School Readiness program and have them available for
the County's review and audit upon request.
f. To cooperate with the COUNTY as needed to fulfill the terms of this agreement.
