1. Does the document require the chairman's original signature? SRT

3. Original document has been signed/initialed for legal sufficiency. (All documents to be SRT
signed by the Chairman,with the exception of most letters,must be reviewed and signed
by the Office of the County Attorney.
4. All handwritten strike-through and revisions have been initialed by the County Attorney's SRT
Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the SRT
document or the final negotiated contract date whichever is applicable.
6. "Sign here"tabs are placed on the appropriate pages indicating where the Chairman's SRT
signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip SRT
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwarding to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 9/10/13 and all changes made during the SRT
meeting have been incorporated in the attached document. The County Attorney's
Office has reviewed the changes,if applicable.

A RESOLUTION APPROVING SATISFACTION OF LIEN
FOR A CERTAIN ACCOUNT THAT HAS BEEN PAID IN
FULL, INCLUDING INTEREST, ARISING FROM THE
1994 DELINQUENT SOLID WASTE COLLECTION AND
DISPOSAL SERVICES SPECIAL ASSESSMENTS.
WHEREAS, pursuant to Collier County Ordinance No. 90-30, as amended,the Board on
August 22, 1995, adopted Resolution No. 95-475 authorizing the recording of notices of liens for
the delinquent solid waste collection and disposal services special assessments for 1994; and
WHEREAS, Collier County Ordinance No. 90-30, as amended, requires the Board to
approve by Resolution and record in the Official Records Satisfactions of Lien on all accounts
that have been paid in full.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY,FLORIDA,that:
Pursuant to Collier County Ordinance No. 90-30, as amended,the Board recognizes full payment
and receipt of the 1994 Service Year Solid Waste Collection and Disposal Special Assessments
for the account numbered below, whereupon a lien has been recorded on real property pertaining
to the following accounts:

The Satisfaction of Lien, referencing the account identified herein is hereby approved and
the Chairwoman is hereby authorized to sign on behalf of the Board and the Clerk is directed to
