3. Original document has been signed/initialed for legal sufficiency. (All documents to be
signed by the Chairman,with the exception of most letters,must be reviewed and signed RB
by the Office of the County Attorney.
4. All handwritten strike-through and revisions have been initialed by the County Attorney's N/A
Office and all other parties except the BCC Chairman and the Clerk to the Board
5. The Chairman's signature line date has been entered as the date of BCC approval of the Ce/�rJ
document or the final negotiated contract date whichever is applicable. `(

signature and initials are required.
7. In most cases(some contracts are an exception),the original document and this routing slip RB
should be provided to the County Attorney Office at the time the item is input into SIRE.
Some documents are time sensitive and require forwardin to Tallahassee within a certain
time frame or the BCC's actions are nullified. Be away f your deadlines!
8. The document was approved by the BCC on 04/09/201 (enter date)and all changes RB
made during the meeting have been incorporated in the attached document. The
County Attorney's Office has reviewed the changes,if applicable.

RESOLUTION NO. 13- 8 2
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA FINALLY ACCEPTING THE PLAT
DEDICATIONS GRANTED BY THE PLAT OF BROOKS VILLAGE,
ACCORDING TO THE PLAT THEREOF RECORDED IN PLAT BOOK
49,PAGES 27 AND 28.
WHEREAS, on January 15, 2008, the Board of County Commissioners of Collier
County, Florida approved the plat of Brooks Village for recording; and
WHEREAS, Section 10.02.04 B.5.l.i of the Collier County Land Development Code
requires the acceptance of plat dedications by Resolution; and
WHEREAS, the County has development improvement plans which include Tract `R-1'
according to the plat, which was dedicated to Collier County as future public road right-of-way
with responsibility for maintenance; and
WHEREAS, the plat also dedicates certain drainage, lake maintenance and conservation
easements to Collier County, with no responsibility for maintenance.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that the plat dedications according to
the plat of Brooks Village as recorded in Plat Book 49, Pages 27 and 28, are finally accepted.
BE IT FURTHER RESOLVED AND ORDERED that the roadway and drainage
improvements within Tract `R-1' will be constructed and maintained by Collier County as public
road right-of-way.
This Resolution adopted after motion, second and majority vote, this 941 day of
Aprit_, 2013.
