CONSIDERAno~s: On Apri127, 2010, the Board adopted Resolution No. 20,.
the 2010-2011 Action Plan (Item 16DI5), and authorized its submission to the U.S.' t of
Housing and Urban Development (BUD). A listing and description of proposed projects, the
amount of funding for each project, and the'Sub~ent Agreement template for each type of grant
were also included in the Action Plan submission.
On August 6,2010, HUD provided the FY 2010-2011 entitlement funding letter and agreements to
the County in the amount of $3,376,051. The Board of County Commissioners (Bee) accepted
receipt of this entitlement funding on September 14, 2010 (Item 16DI2). The. individual
subrecipient agreements and project scopes mustbe approved and signed by the Cbai1man.

A. PRo.JECI" SCOPE
On April 27*, 2010, the Board of County Commissioners approved the HUD Action PI8D allocating
ftmding for this initiative. Community Development Block Grant (CDBG) funding will be used for the
Anthony Parle improvements within the City of Naples.
In the Action Plan for this program the intended outcome of this funding is to provide pass-through
funding for the enhancement of two (2) neighborhood perks within a low/moderate income area in the
City of Naples. Enh8Dccments will include: paving IIDd curbs, sod replacement and additional pIantings,
~ and iIrigation improvements to the Anthony Park. and Cambridge Pcny Parle area adjaoem to
Sdl Avawe North, by inBtalling a minimum often (10) parking spaces.
The funding will cover all costs associated with Engineering design, drainage, landscaping, paving.
inigation and hardscaping.
The detailed project scope will be contained in the bid specifications oftbe vendor awarded the project's
engineering and/or construction contracts. The project construction contract will include details sufficient
to document the number, amount, and costs associated with all activities.
The project activities will meet the u.s. Department of Housing and Urban Development's national
objective to benefit low and moderate income area.
B. BUDGET
Collier County Housing, Human and Veteran Services is providing One Hunrlred Twenty Thousand Four
Hundred Thirty Four Dollars and 00/100 ($120.434.00) in CDBG funding for the project scope described
above.

On April 2"-, 2010, the Board of County Commissioners approved the HUn Action Plan
allocating ftmding for this initiative. Home Investment Partnerships Program (HOME) fuDding
will be used for the purchase 8Dd n:babili1ation of foreclosed homes within Collier County.
In the Action Plan it is stated that the County, as a participating jurisdiction (Pl). must invest
15% or set aside at least 15 percent (15%) of ita total HOME allocation to projects that are
owned, developed or sponsored by Community Housing Development Orpni7Jrtions
(CHDOs), and which resuh in the development ofbomcowncrship or rental UDits.
Florida Non-Profit Housing, Inc. as a CHDO, will use tbcsc funds to acquire aDd rcbabiIitate a
minimum of ODe (I) property within Collier County. Rehabilitation activities/items may
include but DOt be limited to: repair and/or rqJlacement of the following: roofs, floors, ceiliDg.
waIls, kitchens, bathrooms, structural items, doors, windows, if\,<ZI,I-tion, HV AC, electrical,
plumbing systems, fixtures, appliances, landscapiDg. irription systems, pttinti~. driveways,
pest control, termite treatment, permitting, labor, matcria1s, supplies and other related items.
The detailed project scope will be contained in the schedule of values of the vendor. The
contnlct will include details sufficient to document the nlmlber, amount and costs associated
with all activities.
Any unexpended funds that may be available after a minimum of ODe (I) home is purclwed"
rehabilitated, and occupied, may be used toward the purchase and rehabilitation of one (1)
additional home. Reimbursement for the pwdwe of 811 additicmal home will not exceed
$65,000.00, and rehabilitation for the same additional home will not exceed 520,000.00.
The project activities will meet the u.s. Department and Housing and Urban Development's
national objective to provide decent housing.
B. BUDGET-
Collier County Department of Housing, Human and Vetel8n Services is fundina ONE
TWELVE rnOUSAND TWO HUNDRED and THIRTY DOLLARS ($112,230.00) in HOME
funds for the CHDO Set-Aside project scope described above. The required 17'At,..hil'lg funds of
