EXECUTIVE SUMMARY
Recommendation to approve and authorize the Chairman to sign an amendment to the
ARRA 203.09 agreement and attestation statement with the Area Agency on Aging for
Southwest Florida, Inc. and recognize a reduction in funding in the amount of $15,000.
These federal funds are provided by the United States Department of Health and Human
Services under the American Recovery and Reinvestment Act of 2009.
OBJECTIVE: To approve and authorize the Chairman to sign an amendment to the ARRA
203.09 agreement and attestation statement with the Area Agency on Aging for Southwest
Florida, Inc. to reflect a decrease in the allocation of ARRA funds in the amount of $15,000.
CONSIDERATIONS: On June 9, 2009, the Board of County Commissioners signed an
agreement, agenda item 16D 11, with the Area Agency on Aging for Southwest Florida, Inc. to
accept an allocation of ARRA funds in the amount of $68,901.84 designated for Collier County
to operate the initial three (3) months of nutrition program activities. In August 2009, the
agreement was amended to extend the agreement to September 30, 2010 and increase the grant
by $51,609.96, bringing the total grant amount to $120,511.80. Amendment #4 reflects a
decrease in the Congregate Meals program in the amount of$15,000.
This ARRA Senior Meals grant ends on September 30, 2010. There are sufficient funds
currently available under this grant to serve existing clients until the end of the grant. Once the
ARRA grant ends on September 30, clients served under ARRA will be transferred to the Older
Americans Act Congregate Meals program. This transfer will be performed administratively by
Housing, Human and Veteran Services staff and clients will not experience any interruption in
servIces.

FISCAL IMPACT: The ARRA Congregate Meals program in Fund 125 ARRA Grants will
decrease by $15,000.
GROWTH MANAGEMENT IMPACT: There is no growth management impact from this
recommendation.
LEGAL CONSIDERATION: This item has been reviewed and approved by the County
Attorney's Office. This item is legally sufficient for Board action.-JBW
RECOMMENDATIONS: To approve and authorize the Chairman to sign the amendment and
attestation statement, and approve the necessary budget amendment.

STANDARD CONTRACT
AREA AGENCY ON AGING
Collier County Housing, Human and Veteran Services
This AMENDMENT, entered into by Area Agencv on Aging for Southwest Florida. Inc..
hereinafter referred to as the "Agency," and Collier County Housing. Human and Veteran
Services. hereinafter referred to as the "recipient," amends contract ARRA 203.09.
The purpose of this amendment is to
A) Amend the contract amount by the following:
1) In Collier County, the C1 allocation decreased by $15,000.00.
B) Revise and replace Attachment III - Exhibit 1 attached.
C) Revise and replace Attachment VII, Budget Summary attached.
This amendment shall be effective August 12,2009.
All provisions in the agreement and any attachments thereto in conflict with this amendment
shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at
the level specified in the agreement.
This amendment and all of its attachments are hereby made a part of this agreement.

COLLIER COUNTY HOUSING & HUMAN SERVICES
THIS CONTRACT is entered into between the Area Agency on Aging. hereinafter referred to as the
.. Agency," and Collier County Housing and Human Services., hereinafter referred to as the
"Contractor"; and collectively referred to as the "Parties." The term contractor for this purpose may
designate a vendor, subgrantee or subrecipient, the status to be further identified in KIT ACHMENT
III, Exhibit-2 as necessary.
WITNESSETH THAT:
WHEREAS, the Agency has determined that it is in need of certain services as described herein; and
wHEREAs, the Cont.nlctor has demonstrated that it has the requisite expertise and ability to faithfully
perform such services as an independent contractor of the Agency.
NOW THEREFORE, in consideration of the services to. be performed and payments to be made,
together with the mutual covenants and conditions hereinafter set forth, the Parties agree as follows:
1. Purpose of Contract
The purpose of this contract is to provide services in accordance with the terms and conditions
specifted in this contract including all attachments and exhibits, which constitute the contract
document.

The contract will incorporate attachments, proposal(s), area plan(s), grant agreements, relevant
Department of Elder Affairs handbooks. manuals or desk books, as an integra! part of the
contract, except to the extent that the contract explicitly provides to the contr'ar).. In the event
of conflict in language among any of the documents referenced above, the specific provisions
and requirements of the contract document(s) shall prevail over inconsistent provisions in the
Proposal(s) or other general materials not specific to this contract document and identified
attachments.
3. Term of Contract
This contract shall begin on May I. 2009 or an the date on which the contract has been signed
by the last party required to sign it. whichever is later. It shall end at midnight, local time in
Tallahassee, Florida, on September 30.2010.
4. Contract Amount
The agency agrees to pay for contracted services according to the terms and conditions of this
contract in an amount not to exceed $68,901.84, ortbe rate schedule, subject to the availability
of funds. Any costs or services paid for under any other contract or from any other source are
not eligible for payment under this contract.
5. Comnliauee with Federal Law
