Recommendation to approve and authorize the Chairman to sign the contract amendment and
Attestation Statement between the Area Agency on Aging for Southwest Florida, Inc. dba Senior
Choices of Southwest Florida and Collier County to' extend the grant period by ninety ,days,
approve budget amendments to reflect estimated funding for the FY12 Older Americans Act
Program in the amount of $919,008, local match funding from carry forward in Servers for
Seniors Fund 123 in the amount of $102,111.11 and authorize the continued payment of grant
expenditures.
OBJECTIVE: To approve and authorize. the Chairman to sign the contract amendment and Attestation
Statement to extend the grant period by ninety days,` approve budget amendments to reflect funding for
the FY12 Older Americans Act program in the amount of $919,000, local match fiuxiing from carry
forward in Services for Seniors Fund 123 in the amount of $102,111.11 and authorize the continued
payment of grant expenditures to avoid disruptions in services.
CONSIDERATIONS: The Older Americans Act (OAA) Program is a federal program initiative
that provides assistance to alder persons and caregivers and is the only federally supportive services
program directed solely toward improving the lives of older people. These funds are received by Collier
County from the Area Agency on Aging of Southwest Florida, Inc dba Senior Choices of Southwest
Florida. The contract period is January 1, 2012 through December 31, 2012.
The current agreement for the OAA Program will end on December 31, 2011. Historically, Housing,
Human and Veteran Services, Services for Seniors receives the continuation agreements from the grantor
agency during the latter part of December for the upcoming grant cycle. In order to avoid disruptions in
services, the grantor has extended the existing grant agreements by ninety days or until a new agreement
is executed, w_ hichever_ comes first. Furthermore, the grantor has guaranteed that funding will be received
in an amount not less than the FYI l allocation. When final agreements are received, budget amendments
will be prepared and presented to the Board of County Commissioners to reflect any changes in the final
allocation.
Approval of this item will extend the agreement for the OAA program which includes various grants by
ninety days, approve budget amendments to reflect funding for FY12 in the amount of $919,000, and
authorize the continued payment of grant expenditures under the aforementioned programs. The

r-� FISCAL IMPACT: Budget amendments are required to recognize grant revenue in the amount of
$919,000 for the FY12 OAA Program to Human Services Grant Fund (707), Projects 33187,
33186, 33184, and 33188. Budget amendment are also required for the local match requirement
of $102,111.11 to Human Services Grant Match Fund (708), Projects 33187, 33186, 33184, and
33188. Local match is available through carry forward in the Services for Seniors Fund (123).
LEGAL CONSIDERATIONS: The underlying Agreement (concerning which a request for amendment
to reduce funding is also on this agenda) expressly contemplates an extension by mutual assent of the
parties. This item has been reviewed by the County Attorney, is legally sufficient, and requires majority
vote for approval. JAK
GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with this
item.
RECOMMENDATION: To approve and authorize the Chairman to sign the contract amendment and

This AMENDMENT, entered into by Area Agency on Aging for Southwest Florida, Inc. (Agency) dba Senior Choices of
Southwest Florida and Collier County Board of County Commissioners, (Contractor), amends Contract OAA 203.11.
The purpose of this amendment is to (1) extend this contract for 90 days or until a new contract is executed, whichever
comes first.
This amendment shall be effective January 1, 2012.
All provisions in the agreement and any attachments thereto in conflict with this amendment shall be and are hereby
changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
