EXECUTIVE SUMMARY
Recommendation that the Board of County Commissioners provide after-the-fact approval of the
amendments between Collier County 'Board of County Commissioners and the Area Agency on
Aging for Southwest Florida, Inc., which reflects additioual grant funding in the amounts of
$60,000 for the Community Care for the Elderly (CCE) program and $8,668 for the Alzheimer's
Disease Initiative (ADI) program for FYIO-11.
OBJECTIVE: To amend the FY] 0-11 Community Care for the Elderly and Alzheimer's Disease
Initiative grant agreements between the Board of County Commissioners and the Area Agency
on Aging of Southwest Florida. Inc. These amendments provide an increase of $60,000 in
additional funding for the Community Care for the Elderly program and an increase of $8,668 in
funding for the Alzheimer's Disease Initiative program.
CONSIDERATIONS: Housing, Human and Veteran Services, Services for Seniors Program has
been providing support services to Collier County's frail elderly for over 30 years through the
Community Care for the Elderly Program. The services coordinated by staff in the Services for
Seniors program allows area seniors to remain in their homes and age with dignity by providing
services such as homemaker, personal care, respite, emergency alert response, and medical
supplies. The Housing, Human and Veteran Services. Services for Seniors Program has received
additional allocations in FYIO-11 for the Community Care for the Elderly program and
Alzheimer's Disease Initiative program in the amounts of $60,000 and $8.668, respectively.
Therefore, amendments to the original agreements must be approved to accept the additional
funding. The original agreements were approved by the Board on June 22, 2010, Item 16D6.
The total grant awarded to the Community Care for the Elderly program and Alzheimer's
Disease Ittitiative program now amounts to $713,779 and $129,158, respectively for the period
July 1,20]0 through June 30, 201]. In order to receive the additional funding, the amendments
were required to be signed by May 3], 2011. As a result, the amendments for the additional
funding have been signed by the County Manager as per CMA# 5330 and require after-the-fact
approval by the Board of County Commissioners.
FISCAL IMPACT: The funding for the Community Care for the Elderly program was increased
by $60,000 and increased by $8,668 for the Alzheimer's Disease Initiative program. No
additional matchittg funds are required.
LEGAL CONSIDERATIONS: This item is legally sufficient and requires a majority vote for
approval. - JBW
GROWTH MANAGEMENT IMPACT: There tS no growth management impact from this
recommendation.
RECOMMENDATION: That the Board of County Commissioners provide after-the-fact
approval of the grant amendments with the Area Agency on Aging for Southwest Florida, Inc.

County Manager Review and Approval of amendments to Community Care for
the Elderly (CCE) and the Alzheimer's Disease Initiative (ADI) grant agreements
sponsored by Area Agency on Aging for Southwest Florida, Inc. in the amount of
$68,668.
The Collier County Housing, Human, and Veteran's Services department is requesting your
approval of two amendments to two grant agreements to increase funding. HHVS Services for
Seniors program has been providing support services to Collier County's frail elderly for over 30
years through the Community Care for the Elderly (CCE) program. The services coordinated by

amendments followed by after the fact approval by the Board of County Commissioners.

A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF
COLLIER COUNTY, FLORIDA, REPEALING AND SUPERSEDING
RESOLUTION NO. 96-268 AND AUTHORIZING THE COUNTY
MANAGER OR HIS/HER DESIGNEE TO EXECUTE CERTAIN
STANDARD SOCIAL/SENIOR SERVICES GRANT APPLICATIONS
AND CONTRACT DOCUMENTS INCLUDING AMENDMENTS IN
COMPLIANCE WITH CMA #5330, GRANT COORDlNA nON.
WHEREAS, the Board of County Commissioners (Board) has the authority to apply for
and receive social/senior services grants including the Older American's Act Program (OAA),
Community Care for the Elderly Program (CCE), Alzheimer's Diseasc Initiative Program (ADI),
Medicaid Waiver Agreement, the Home Care for the Elderly program (HCE). and USDA
Nutritiona] Supplement Incentive Program; and
WHEREAS, in order for Collier County's social/senior services grant documents to be
submitted in a timely fashion, the Board adopted Resolution No. 96-268 authorizing the County
Manager or hislher designee to execute the required standard grant documents; and
WHEREAS, Collier County CMA #5330, Grant Coordination, provides authorization for
the County Manager or hislher designee to execute certain grant documents and return to the
Board of County Commissioners for after-the-fact approval; and
WHEREAS, the Board wishes to delegate signature authorization to the County Manager
or hislher designee for the above-referenced social/senior services grant programs and as it
relates to grant applications, standard contract documents, and major and minor amendments
provided that said submissions and contracts reqUire no additional Collier County matching
funds.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISS]ONERS OF COLLIER COUNTY. FLORIDA. that the County Manager or hislher
