EXECUTIVE SUMMARY
Recommendation to approve the purchase of Group Life insurance, Accidental Death insurance,
Long Term Disability insurance, Short Term Disability Claims and Family Medical Leave Act
Administration Services (Agreement #18-7409) from Reliance Standard/Matrix Absence
Management, Inc., for a three-year period effective January 1, 2019, in the estimated annual
amount of $670,994.
OBJECTIVE: To provide a competitive, best value Group Life and Disability insurance program as part
of the County’s comprehensive group insurance package and administer the County’s self -insured Short-
Term Disability, Family Medical Leave (FMLA) and Medical Leave of Absence (MLOA) programs.
CONSIDERATIONS: The Collier County Board of Commissioners through the Risk Management
Division administers a Group Insurance Program for its eligible employees and participating
constitutional officer employees. As part of this program, the County provides two times salary life and
accidental death insurance and a 40% short and long-term disability benefit. Employees can purchase
additional short and long-term disability coverage up to a maximum benefit of 66 2/3% of salary (a 26
2/3% buy-up benefit). The short-term disability benefit is self-funded. All other benefits are fully insured.
Employees may also purchase additional voluntary life and accidental death coverage on themselves and
their dependents. Due to the nature of disability claims, the life insurance, long term disability insurance
and short-term disability administration programs must be integrated and purchased from the carrier
chosen.
The County currently administers the requirements of the Family Medical Leave Act (FMLA) in-house.
The FMLA is a federal law that guarantees certain employees up to 12 workweeks of unpaid leave in any
12-month period for various specified purposes with the right for the employee to return to the same or an
equivalent position after returning from FMLA leave. FMLA also requires that employers covered by the
law maintain the health benefits of eligible workers just as if they were working. The law requires
consistent and close administration with the County’s sick leave bank and disability programs. The law
also imposes significant financial penalties if the requirements of the Act are not followed. As Federal
and State leave requirements have become more complex, the Risk Management Division, Human
Resources and Willis Towers Watson, the County’s benefits brokerage and actuarial firm, believe there is
considerable risk in continuing to administer the requirements of these laws internally. Therefore, in
conjunction with the marketing of the group insurance programs noted above, terms were obtained from
the disability insurers to administer FMLA ensuring compliance with state and federal regulations;
reducing the risk of violations of law; securing protected health information; and ensuring that leave
requests are valid.
The current Group Life and Disability program is underwritten by Aetna and expires on December 31,
2018. In anticipation of this expiration, Willis Towers Watson marketed the program and received
proposals from nine carriers including the incumbent, Aetna.
After reviewing the terms and pricing of each proposal received, the list was narrowed to three carriers
that provided the most competitive financial terms. Aetna was eliminated from the group due to a pricing
increase of 50%, making them uncompetitive. All three carriers guaranteed rates for three years. The table
below shows the price terms quoted.

Staff completed a due diligence review of the three carriers to assess their ability to manage and integrate
the Short-Term Disability and FMLA administration portions with the County’s practices and procedures.
Although there was a marginal $3,738 annual difference in cost between Reliance Standard Life
Insurance Company and the lowest carrier (The Standard), staff unanimously recommended Reliance
Standard Life due to their proven service record and experience with the carrier. The absence and
disability management services quoted by Reliance Standard Life Insurance Company will be
administered and managed by its sister company Matrix Absence Management, Inc., as set forth in the
attached Agreement.
The program will commence effective January 1, 2019.
FISCAL IMPACT: The total estimated annual cost of the Group Life, Disability insurance program,
FMLA and MLOA Administration is $670,994 based upon current reportable payroll. The estimated
additional annual cost to the Board is $118,986, which represents a rate increase due to experience of
$67,535 and costs for FMLA and MLOA services of $51,451. There are sufficient funds available in
Fund 517, Group Health and Life, for this purchase.
GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with this
item.
LEGAL CONSIDERATIONS: This item is approved as to form and legality, and requires majority
vote for Board approval. -SRT
RECOMMENDATION: To approve the purchase of Group Life insurance, Long Term Disability
insurance, Short Term Disability Administration Services, and Accidental Death Insurance and FMLA
and MLOA Administration from Reliance Standard/Matrix Absence Management, Inc., as outlined in the
executive summary, and authorize the Chairman to execute the attached agreement.

The Basic Life and AD&D, Supplemental Life and AD&D, ASO STD and LTD programs have been
with Aetna since 1/1/2016.
The FMLA is currently administered in-house, but the CCG is extremely interested in evaluating the
cost and benefits of outsourcing this responsibility.

Included with this RFP is a copy of the certificates applicable to the coverage in force with Aetna.
It is a requirement of this RFP that there be no less generous provisions in your coverage. Please
confirm you agree to meet, at a minimum, the current policy terms and provisions. If you believe
any of your policy terms or provisions are more generous, please clearly outline those provisions
and why you believe they are superior to what is in place with Aetna today.

January 1, 2019 effective date. Note, if the CCG elects to outsource FML, it may occur at a date
other than January 1, 2019.
Dedicated account management team. The ability to provide an onsite resource will be viewed
extremely favourably, especially if FML administration is included.
It is preferred that all lines of coverage outlined in this RFP will be awarded to one carrier,
including the FMLA, if possible.
Proposal must match the current benefits, including the Basic Life and AD&D offered to Retirees
and the specific benefits offered to the County Manager and County Attorney.
Quoted rates are to be guaranteed for at least three years through 12/31/2021. Carriers may offer
a guarantee extending beyond three years as an option. Extensions beyond three years will be
viewed favourably.
Quoted rates must be final regardless of actual plan enrollment. This means that we expect you
to waive all minimum participation levels, fluctuation levels, etc. for the duration of the rate
guarantee period.
Evidence of Insurability will not be required for in-force life amounts for supplemental or
dependent coverage, including those that are pending with the incumbent carrier. Provide
confirmation.
All basic rates will be non-contributory, fully insured premium rates.
All supplemental and dependent life rates will be 100% contributory.
Proposals should be net of commissions for Basic Life and AD&D, ASO STD, LTD and FMLA.
Proposal should include 20% commission for Voluntary Life and AD&D and 10% commission for
Buy-Up LTD. Include language confirming that rates will not be reduced if commissions are not
included.
Proposal should include a 3 year contract term with a 30 day “without cause” termination clause,
with the option to extend this contract after the 3 year term is exhausted.
Agree to provide an annual executive review of the utilization and performance of the program.
Additional Considerations
Please address the following in your proposal and questionnaire.
Employees are required to use all PTO prior to the onset of STD. This is an issue with their
current STD carrier. Address how you would ensure an employee has exhausted their PTO
prior to the onset of STD.
Are you willing to offer performance guarantees with fees at risk?
Are you willing to offer any implementation credits?
With regard to the STD, please provide an advice to pay option in addition to the current ASO
arrangement, where Aetna cuts the check.

This proposal has been developed to specifically address the needs of Collier County Government. Reliance
Standard Life Insurance Company and Matrix Absence Management have been developing and implementing
customized Integrated Disability programs since 1995, and employers have found that these programs are more than

program, and we start the return-to-work process on day one.

Clients using Matrix for ASO STD and FMLA management are staffed by our Absent Management Specialist (AMS)
team model. AMS strategies integrate LOA administration and STD claim management into a single managed
employee leave event. The AMS model is led by the AMS Claims Examiner (AMS CE), a single individual claim
professional responsible for the concurrent management of STD/FMLA claims. The AMS CE is cross-trained in
products, policies, systems, regulations and procedures. This single point of contact drives fully integrated claim and
leave management decisions with coordinated communications to meet employer needs. Because of the value of
this approach, we have begun the process of transitioning our existing clients onto the AMS Model.

Summary of Proposed Services
Event Reporting
We don’t process absence claims – we manage them, just as we would an injury or illness. As an Absence
Solutions® customer, your employees can choose either telephonic reporting or our web-based claim-filing tool,
eFiling. Both intake options will be customized to meet the needs of Collier County Government. It is through this
type of collaborative partnership that our intake specialists can identify an employee in a “teachable moment” and
direct them to the appropriate resources provided by Collier County Government, even if the health risk is not the
specific reason for the employee’s leave. During claim intake, the customized questions and reminders on the intake
statement can solicit information that would assist in the identification of potential referrals and include information
related to the specific program applicable to the employee’s situation.
Additional benefits of our intake services include:

All reporting is to an Absence Solutions® employee – it is never subcontracted
All intake calls are recorded and kept for one year
Employees calling during peak call volume times have the option of requesting a priority callback
Translation services available for over 150 languages
Finally, we believe so strongly in our process and our ability to have a positive impact on employee durations, we
make our telephonic and web-based filing options available 24 hours a day, 7 days a week, for all of our customers.
Customized Leave Packets and Next Steps
We see our services as an extension of your human resources department. Our goal is to relieve your staff of the
burden of the administrative tasks related to employee leaves. One way we accomplish this goal is to customize the
leave packet to meet the unique employee benefit goals and objectives of Collier County Government. Our standard
procedure is to be in contact with the employer, the employee and the provider, during the first business day of a
reported absence.
At event reporting, the employee provides a summary of the incident
The employees customized leave packet is sent within 24 hours.
A client checklist which outlines the employee’s roles and responsibilities is included in the packet, along with a
medical certification form and expected time line.
Each packet is customized to include any other forms, documents, and letters required by Collier County
Government.
Triage
Immediately upon completion of the intake interview, all claims are subject to our automated duration review
process. Using industry guidelines and standards based on diagnostic information and expected duration; claims are
reviewed and assigned to resources based on complexity. In addition, we can use Collier County Government-
specific criteria to determine which additional cases go to your dedicated in-house nurse case manager.
Clinical Case Management – Medical Director Model
An automated duration review system completes initial screening of cases. However, your claim examiners and
dedicated nurse case manager evaluate all claims that are flagged as complex or that meet a predetermined criteria

so we can establish a plan of action. Our medical directors are specialists in their respective fields and work with our
nurse case managers to review and manage all complex claims.
We use best practice escalation criteria to determine additional claims that should be referred to the nurse case
manager and/or medical directors based on claim facts. Our claim staff can also make referrals based on their own
knowledge of the employee and the facts surrounding the disability event. In addition, your dedicated nurse case
manager will:
Manage clinical aspect of complex cases
Management based on primary and secondary diagnosis
Referral to Health and Wellness Programs

This is NOT a Tracking System
Just as our name suggests, what we offer is a comprehensive Absence Solution. Our processes allow us to identify
patterns of utilization, which are outside the prescribed certification times. No leave will be approved without a
certification that supports the time off. All intermittent leaves are reviewed every 60 days to analyze day of the week
and other patterns that could go undetected. Your dedicated claim examiner will personally intervene to encourage
your employees to schedule appointments outside of working hours and make a timely return-to-work. Our claim

eClaims – An employer application that provides information on all aspects of an individual claim, including

eAccounts – An employee application, used to give employees information about their open claims
eReports – An employer application that allows self-service access to a range of standard and ad-hoc reporting
across the entire Collier County Government program.
eReports designer – Includes a sub-application which permits the user to design, generate and save
customized ad-hoc reports using any data elements captured within eReports.
Also included in our eServices applications is the new Absence Radar, which is a set of tools that will help you
manage employee absence. This tool includes:
Status Notifications- One consolidated email for all your employees absences that will keep you up-
to-date. These emails inform you of any new or updated information about absences on your team.
Absence Radar Calendar- This calendar tool provides a detailed, interactive view of employee
absences over a12-month period, allowing you to review previous absences and plan around future
ones. The information on your calendar will be customized for your role and responsibilities within
your company.
This web-deployed suite of applications offers enriched communications, real-time claim activity, and on-demand
program analysis and information transfer. Put simply, Collier County Government will be armed with the data
necessary to make informed decisions about necessary adjustments to your plans as well as information on current

The Absence Solutions® proactive approach continues throughout the life of the claim, up to and including the
return to work process. Our return to work services include;
A call to your employee five days prior to their scheduled return to work.
A reminder of any Collier County Government-specific RTW policies such as:
Required RTW note from attending physician
Mandatory drug testing requirement
Reminder to check in with Collier County Government designated personnel
Return to work status notification to Collier County Government confirming actual return-to-work date
On the scheduled return to work date we will contact the employee and/or supervisor to confirm employee has
resumed his/her work assignment.
Employer and Employee Satisfaction
Consistently meeting our own high standards and yours is key to our success. Customer satisfaction surveys are
sent to your employees after intake and at the close of every claim. Results will be specific to Collier County
Government and will be delivered by your assigned account manager, who will assist you in interpreting these results
and identifying areas of improvement. Matrix company-wide scores have never fallen below 95% customer
satisfaction.

A dedicated account manager will serve as your single point of contact and assumes total responsibility for the

Term Definitions
Takeover Existing STD Claims
We can at your option take over and seamlessly continue the management of employees currently receiving STD
benefits. We would typically require additional information to ensure no disruption to your employees
Takeover Existing FMLA Claims
We can at your option take over and seamlessly continue the management of employees currently on leave. We
have the ability to take over historical data and usually recommend at least 24 months of data.
STD Appeal
Matrix will agree to assist the client with denied claims on appeal. This will include one written recommendation to
the client based on the information available within the timeframes specified within the ASO agreement.
Banking Services
Matrix establishes a unique bank account on behalf of the client that does not commingle funds with any other
bank accounts. Arrangements for reimbursement of claim monies must be in place whereby Matrix is reimbursed
for the claims monies paid out on the client’s behalf. These arrangements are established at the inception of the
program, based on the client’s preferred method of reimbursement (ACH Credit or ACH Debit) and frequency of
reimbursement (usually Monthly) to adequately fund claim liabilities. Matrix requires a pre-funding amount to
cover the maximum expected claim payments issued during your selected reimbursement period.
Payroll Services
Matrix will ensure the withholding of Federal income taxes, Social Security taxes, and Medicare taxes for disability
payments if Matrix is issuing checks on ASO business. The taxes and W2s are handled under Matrix’s FEIN.
ACH Credit/ACH Debit
With ACH Credit the client funds, or reimburses, the bank account for the amount of claims paid out under the
ASO/STD plan through a wire transfer or other Electronic Funds Transfer of the required funds.
With ACH Debit, the client grants Matrix permission to fund, or reimburse, the bank account for the amount of
claims paid out under the ASO/STD plan by having Matrix draw from a specific designated client-owned account.
Benefit Deduction Services
A client-selected service which allows employee benefit deductions to be taken from disability benefit checks, with
the exception of 401(k) or other pre-tax withholding.
Additional LOA Tracking
In addition to FMLA administration, we have the ability to track and manage Jury, Bereavement, Military,
Personal, and Medical Leaves. We also provide an Advise to Pay with a PTO bank, but do not track PTO or
Vacation time as a standard part of our Absence Management Program.

especially when the employee is used to paying premiums through payroll deduction.
We can offer a fully customizable solution to ensure employees maintain the insurance they had while at work.
We offer flexibility and streamlined collection of insurance premiums from employees on unpaid leave. You
control the content and timing of all letters and notices.
How Premium Billing Works: 1. Employee goes out on unpaid leave; 2. Customized communications and
payment coupons are sent from Matrix to the employee; 3. Employee makes monthly benefit payment; 4. Matrix
reconciles the payment and reports out to the employer; 5. Employers and employees have immediate access to
payment information via easy web access; 6. Matrix advises the employer when payments stop, so you can
determine continuation of benefits.
Non-Standard Data Feed
Matrix would consider a standard data feed to be one that is a single-source file feed in our preferred format. A
non-standard data feed would be one with multiple sources, outside of our standard format and/or with
customized field definitions.
