This grant is set to expire on December 31, 2018. The State has authorized and extension for an active 5339 JPA from FY14 for twenty-five(25)ADA improvements in the Immokalee area that expires December 31, 2019. The extension was needed because the County was conducting an evaluation of the routes in the area which resulted in significate realignment of the routes and the removal of some of the stops. The route modifications have been implemented,effective November 2017 and staff would like to consolidate the bus stop improvements contemplated with this grant with the improvements approved for the FY 14 grant(Contract if G0015). The bus stop locations for shelter and ADA improvements have been identified and surveys of these locations are about to begin. In order to timely complete the defined ADA improvements and install the five shelters,we are respectfully requesting that FY15/16 Section 5339 Grant under Contract if GOA60 be extended to December 31, 2019. In accordance with Section 18.00 of the Joint Participation Agreement, a time extension may be requested. Please complete, sign, date and return this form to me. It is requested that the Joint Participation Agreement for the subject project be extended until December 31, 2019. The reason for the Time Extension request must be included in a letter to the Department from the Director/Manager/CEO of the organization. A detailed summary of the reason for the Time Extension and a schedule for completing the project must be included. Time Extensions will not be granted for billing purposes if the project is complete.
