NOTICE: ALL PERSONS WISHING TO SPEAK ON ANY AGENDA ITEM

COLLIER COUNTY ORDINANCE NO. 2003-53, AS AMENDED, REQUIRES
THAT ALL LOBBYISTS SHALL, BEFORE ENGAGING IN ANY LOBBYING
ACTIVITIES (INCLUDING, BUT NOT LIMITED TO, ADDRESSING THE
BOARD OF COUNTY COMMISSIONERS), REGISTER WITH THE CLERK TO
THE BOARD AT THE BOARD MINUTES AND RECORDS DEPARTMENT.
REQUESTS TO ADDRESS THE BOARD ON SUBJECTS WHICH ARE NOT ON
THIS AGENDA MUST BE SUBMITTED IN WRITING WITH EXPLANATION
TO THE COUNTY MANAGER AT LEAST 13 DAYS PRIOR TO THE DATE OF

2. Update on Hurricane Charley and its affect on Collier County - Dan Summers,
Collier County Emergency Management Director
Update includes:
A. Request for mandatory evacuation for Coastal High-Hazard Collier
County due to the potential of high storm surge - evacuation to start @
10 a.m. in order to have the coastland secure by 9 p.m.;
B. Shelters that are opening at 3 p.m. are: Gulf Coast High School,
Oakridge Middle School, Corkscrew Middle School, and Immokalee
High School (others are set for reserve);
C. Request to waive the EMS Transport Fees (one-time out and back-in is
their responsibility)
Resolution 2004-245: To waive EMS Transport Fees "outgoing" for
Preregistered (w/Emergency Services) residents with health conditions for
transport to shelter - Adopted 4/0;
And Declaring Mandatory Evacuation of Coastal High-Hazard Collier
County - Adopted 4/0;
Update w/National Hurricane Center at 5 p.m. and Emergency Management
to update County at 6 p.m. (same Commissioners will be available);
John Torre, Director of Communications and Customer Relations will be
spokesperson for County Emergency Management Department

Declaration of Extraordinary Circumstances that Commissioners Fiala and
Coletta be voting members of this Emergency Meeting - Approved;
Commissioner Henning is the Acting-Chairman in the absence in person of
the Chairman and the Vice-Chairman.
4. Commissioner Coletta's request to non-coastal residents to offer help, shelter and
assistance to Coastal High-Hazard Collier County residents

RESOLUTION NO. 2004-245
RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS
FOLLOWING ITS DECLARATION OF A STATE OF LOCAL
EMERGENCY FOR ALL TERRITORY IN COLLIER COUNTY
FURTHER PROVIDING FOR WAIVER OF SPECIFIED EMS
FEES AND ORDERING MANDA TORY EV ACUA TION OF
RESIDENCE AND PROPERTIES IN COUNTY COASTAL AREAS.
WHEREAS, on August 11, 2004 the Board of County Commissioners of Collier
County, Florida (the "Board") in Emergency session adopted ProclamationlResolution
2004-244 declaring a state of emergency for all territory within the legal boundaries of
Collier County including all incorporated and unincorporated areas; and
WHEREAS, The Board, having duly met again in chambers on Thursday,
August 12, 2004 and being further informed that Hurricane Charley has the potential for
causing extensive damage to public utilities, public buildings, public communication
systems, public streets and roads, public drainage systems, commercial and residential
building and areas and posed an imminent threat to the health, safety and welfare of the
citizens, residents and visitors of/to Collier County.
NOW THEREFORE, IT IS RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, in Emergency Session,
this Ith day of August 2004, that:
1. The Board waives the base fee(s) for convalescent transports by Collier
County EMS for evacuation only to approved shelter(s) pursuant to EMS practices and
procedures for "Registered Special Needs" persons.
2. The Board activates mandatory evacuations for all coastal areas including
Copeland, Chokoloskee, Everglades City, Goodland and Isles of Capri.
This Resolution adopted after motion, second and majority vote favoring same.
