COMM AIMM: The Older -Americans Act (OAA) Program is a federal program initiative
that provides assistance to older persons and caregivers and is the only federally supportive services
program directed solely toward improving the lives of older people. These f vWs are received by Collier
County from the Area Agency on Aging of Southwest Florida, Inc dba Senior Choices of Southwest
Florida.
The current agreement for the OAA Program will en& on December 31, 2012., Historically, Housing,
Human and Veteran Services, Services for Seniors receives the continuation agreements. from the grantor
agency during the latter part of December for the upcoming grant cycle. Jn order to avoid disruptions in
services, the grantor has extended the existing grant agreements by sixty day days or until a new
agreement is executed, whichever comes fast. Furthermore,? the grantor has guaranteed that funding will
be receive&JA an amount not less than the FY12 allocation, When final agreements are received, budget
amendments will be prepared and presented to the Beard of County Commissioners to reflect any changes
W the final allocation.
Approval of this item will extend the agreement for the OAA program which includes various grants, by
sixty days, approve budget amendments to reflect funding for FY 13 in the amount of $042,226.00, and
authorize the contim>ed payment of ,grant expenditures under. the aforementioned programs. The
following table provides a detailed breakdown of the award amounts and .the required, local match by
grant.

LEGAL QNMERATIONS The underlying Agreement (concerning which a nest for amendment
to reduce funding is also on this agenda) expressly contemplates an extension mutual assent of the
parties. This item has been reviewedsby the County Attorney, is legally sufficien4 and requires majority

AREA AGENCY ON AGING FOR SOUTHWEST FLORIDA, INC.
STANDARD CONTRACT AMENDMENT
OLDER AMERICANS ACT PROGRAM TITLE III
Collier County Board of Commissioners
This AMENDMENT is entered into on this J I th day of December by and between the Area Agency on
Aging for Southwest Florida. Inc.. hereinafter referred to as the "Agency," and Collier County Board of County
Commissioners, hereinafter referred to as the " Contractor," oblled vdysmedasthe "Parties"
RECITALS:
WHEREAS, on February 28, 2012, the Parties enter into the Standard Contract for the Older American's Act
Program Title III for social services to be provided to Collier County's frail and elderly seniors (hereinafter referred
to as "Contract "); and
WHEREAS, on October 9, 2012 the Parties entered into a a Second Amendment to the Contract; and
WHEREAS, Section 3 of the Contract provides that the agreement is to expire on December 31, 2012; and
/'N WHEREAS, the funding source for a successor agreement for senior services is not yet available to the
Agenda from the Florida Department of Elder Affairs;
WHEREAS, the Parties are satisfied with the arrangement that they currently enjoy under the Contract so
that both parties wish to extend the Contract to February 1, 2012 while they await notification of funding of a
successor contract.
NOW, THEREFORE, in consideration of the foregoing Recitals, and other good and valuable consideration, the
receipt and sufficiency of which is hereby mutually acknowledged, the Parties agree as follows:
1. The foregoing Recitals are true and correct and are incorporated by reference herein.
2. Both Parties agree to extend the contract to February 1, 2012.
All other provisions of the Contract are to remain in effect and are to be performed at the level specified
in the agreement.
