Incentive Program.
OBJECTIVE: To receive approval from the Board �f County Commissioners (BCC) for minor
and routine contract and budget amendments to the Community Care for the Elderly (CCE),
Home Care for the Elderly (HCE), Alzheimer's disease Initiative Program (ADI) and Nutrition
Services Incentive Program (NSIP) grants.
CONSIDERATIONS: The Services for Seniors I
and Veteran Services Department (HHVS), has 1

$43,602 to $34,602. Senior Choices notified HHVS the amendment must have an effective date
of August 9, 2012, the date the Board of Senior Choices approved the proposed amendment.
The amendment for the NSIP contract with Senior Choices is associated with one (1) budget
amendment reflecting a decrease in funding of $9,00 .
FISCAL IMPACT: The only fiscal impact is to the NSIP grant program. The allocation for
NSIP grant program resides in Fund 707, Human Se ices Grants, FYI 1-12 NSIP project 33182.
This allocation is being reduced by $9,000.00 through a budget amendment. There is no match
requirement, and no fiscal impact to General Funds. '
LEGAL CONSIDERATIONS: The proposed amendments are proposed to be effective July 1,
2012 for the CCE, HCE, and ADI programs and August 9, 2012 for NSIP program. These items
are legally sufficient for Board action and require a majority vote. - JBW
GROWTH MANAGEMENT IMPACT: There is no growth management impact.

referred to as the "Parties." The term Recipient for this purpose may designate a vendor, subgrantee or subrecipient, the
status to be further identified in ATTACHMENT III, Exhibit -2 as necessary.
WITNESSETH THAT:
WHEREAS, the Agency has determined that it is in need of certain services as described herein; and
WHEREAS, the Recipient has demonstrated that it has the requisite expertise and ability to faithfully perform such
services as an independent Recipient of the Agency.
NOW THEREFORE, in consideration of the services to be performed and payments to be made, together with the
mutual covenants and conditions hereinafter set forth, the Parties agree as follows:
1. Purpose of Contract
The purpose of this contract is to provide services in accordance with the terms and conditions specified in this
contract including all attachments and exhibits, which constitute the contract document.
1.1 AGENCY MISSION STATEMENT
Mission: To help aging adults, 60 and older, achieve greater independence through awareness of resources and
access to qualified service providers.
Vision: To be the recognized leader in supporting older adults and their families with access to trustworthy
resources and services in their communities while empowering them to live with independence and dignity.
Values: The Agency believes the residents of Southwest Florida are entitled to:

Lead agencies and local service providers as partners and stakeholders in Florida's aging services network are

2. Incorporation of Documents within the Contract
The contract will incorporate attachments, proposal(s), area plan(s), grant agreements, relevant Department
handbooks, manuals or desk books, as an integral part of the contract, except to the extent that the contract
explicitly provides to the contrary. In the event of conflict in language among any of the documents referenced
above, the specific provisions and requirements of the contract document(s) shall prevail over inconsistent
provisions in the proposal(s) or other general materials not specific to this contract document and identified
attachments.
3. Term of Contract
This is a two -year contract shall begin on July 1, 2012 or on the date on which the contract has been signed by
the last party required to sign it, whichever is later. It shall end at midnight, local time in North Fort Myers,
Florida, on June 30, 2014,
4. Contract Amount
The Agency agrees to pay for contracted services according to the terms and conditions of this contract in an
amount not to exceed $661,357.00 for year one, 2012 - 2013, or the rate schedule, subject to the availability of
funds. Any costs or services paid for under any other contract or from any other source are not eligible for
payment under this contract.

THIS, CONTRACT is entered into between the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices of
Southwest Florida ("Agency ") and Collier County Board of County Commissioners, ( "Recipient"), and collectively
referred to as the "Parties." The term Recipient for this purpose may designate a vendor, subgrantee or subrecipient, the
status to be further identified in ATTACHMENT III, Exhibit -2 as necessary.
WITNESSETH THAT:
WHEREAS, the Agency has determined that it is in need of certain services as described herein; and
WHEREAS, the Recipient has demonstrated that it has the requisite expertise and ability to faithfrilly perform such
services as an independent Recipient of the Agency.
NOW THEREFORE, in consideration of the services to be performed and payments to be made, together with the
mutual covenants and conditions hereinafter set forth, the Parties agree as follows:
1. Purpose of Contract
The purpose of this contract is to provide services in accordance with the terms and conditions specified in this
contract including all attachments and exhibits, which constitute the contract document.
1.1 AGENCY MISSION STATEMENT

access to qualified service providers.

5. Renewal
By mutual agreement of the Parties, in accordance with s. 287.058(1)(g), F.S., the Agency may renew the
contract for a period not to exceed three years, or the term of the original contract, whichever is longer. The
renewal price, or method for determining a renewal price, is set forth in the bid, proposal, or reply. No other
costs for the renewal may be charged. Any renewal is subject to the same terms and conditions as the original
contract and contingent upon satisfactory performance evaluations by the Agency and the availability of
funds.
6.1 if this contract contains federal funds this section shall apply.
6.1.1 The Contractor shall comply with the provisions of 45 CFR 74 and /or 45 CFR 92, and other applicable
regulations.
