EXECUTIVE SUMMARY
Recommendation to provide after- the -fact approval for the attached Assistance to Firefighters
Grant application that was submitted to the Federal Emergency Management Agency for the
purchase of three replacement ambulances for Emergency Medical Services in the amount of
$575,850.
OBJECTIVE: To receive after - the -fact approval from the Board of County Commissioners to submit
an Assistance to Firefighters Grant (AFG) application to the Federal Emergency Management Agency
(FEMA) to fund the purchase of three replacement ambulances for Emergency Medical Services.
CONSIDERATIONS: On July 2, 2012 the County Manager approved the electronic submittal of an
AFG grant application, in accordance with Collier County CMA #5330 which authorizes the County
Manager to approve the submittal of grant applications with subsequent BCC action at the next available
Board meeting to ratify the approval as "after -the- fact."
The Assistance to Firefighters Grant program typically provides only a month for development and
submittal of grant applications. This year the program opened on June 11th with a deadline of July 6th, too
late to meet the deadline for the June 26th Board Meeting.
This grant was submitted by EMS to request funding to purchase three ambulances that are on the Fleet
Replacement Schedule, because even with the funding of four ambulances subject to Board approval of
the FY13 budget, there are still an additional three on the replacement schedule for FY13 and two
additional scheduled for replacement and two for refurbishment in FY14.
The total funds requested for this project was $575,850, $460,680 to be funded by the grant and a match
of 20 %, $115,170 to be funded by the County in EMS Fund 493. If awarded, staff will come back to the
Board to request approval to accept the grant.
FISCAL IMPACT: There is no Fiscal Impact at this time.
LEGAL CONSIDERATIONS: This item has been reviewed by the County Attorney, is legally
sufficient, and requires majority support for approval. -JAK
GROWTH IMPACT: There is no Growth Management Impact resulting from this action.
RECOMMENDATION: That the Board of County Commissioners approves the after -the -fact
submittal of an Assistance to Firefighters Grant application to fund the purchase of three replacement
ambulances for Emergency Medical Services.

SUBJECT: County Manager review and approval of an Assistance to Firefighters Grant Application
to the Federal Emergency Management Agency in the amount of $460,680.
Collier County Emergency Medical Services is submitting an application on behalf of Collier County to
request funding for three replacement ambulances. The grant request will be $460,680 and, if awarded,
EMS would provide a 20% match of $115,170 towards the total $575,850 necessary to fund the purchase
of these trucks. The 20% match will come from EMS Fund 493.
This grant program typically provides only a month for development and submittal of grant applications.
This year, the program was announced on June, 11, 2012 with a deadline of July 6, 2012. Furthermore,
since there were no BCC meetings scheduled prior to or during the application period, your review and
approval of the application is requested followed by after- the -fact approval at the July 24, 2012 BCC
meeting.
This application is subsnitted online and does not require the submittal of any signed documents. Your
approval will allow Artie Bay in EMS to submit the application via the FEMA online application portal.

Collier County EMS is the sole provider of emergency medical transport services for all of Collier County, Florlda. Collier Is the
largest county in the state, encompassing 2025 square miles, with a combination of urban, suburban and rural areas. Collier
County Is located on the southwest coast of Florida with a permanent population of 321,520 (based on the 2010 census).
Approximately 20,000 workers increase the population each'day. In addition, the moderate *climate, miles of beaches, three
airports and numerous marinas attract a multitude of visitors each day, swelling the population level to nearly 400,000 (Collier
County Planning Department estimate) during peak season. This agency responded to 36,185 calls for emergency medical
services In FY 2011 and provided 23,920 transports. Emergency medical services are provided to the County via 166 full -time
Paramedics and EMTs, along with 11 field supervisors.

Over 80% of the Collier County EMS budget Is to cover personnel costs. The remaining 20% is utilized to cover
operating costs and capital purchases to replace worn equipment. The budget is largely dependent (41 %) on ambulance
fee revenues. With the majority of the county being agricultural, there Is a large influx of both a'migrant population and
Indigent patients. As such, Collier County experiences a high volume of write -offs for ambulance services. in addition,
ambulance fee collections are tatting a negative turn due to the trend by insurance companies to short -pay claims
(payments substantially less than billed), unemployment and people's loss of group Insurance, and the economy in
general. Most Individuals will choose to keep their lights on and feed their families before paying for an ambulance ride.
As the revenues decrease, the offsetting funds must come from ad valorem taxes, which are already strained due to the
growth of the area and demands on the infrastructure of the County. Especially significant is a referendum in the State of
Florida to reduce property taxes that has crippled county agencies from obtaining necessary funding through ad valorem
taxes. In addition, and common throughout the United States, property values have tumbled and Collier County has
experienced an $87 million loss in revenue since FY08. For the past four fiscal years, the department's operating budget
has been reduced from the prior year, totally eliminating fundirig for replacement of worn out equipment.
Collier EMS has been unable to secure alternative funding for this project. We are unable to apply to the State of Florida
for grants because only projects for new or expanded services are eligible under.that program. As indicated above, the
only way we will be able to fund this project is through outside financlal assistance, such as the AFG program.

Proiect Deserintion;
This grant request is respectfully subnutted to provide $4460,680 for three medium duty ambulances
comprised of Ford F550 chassis and a Horton Box. Collier County will match 20% ($115,170) for a
total of $575,850 to fiend the purchase of this equipment. These vehicles would be purchased through
the "Florida Sheriffs Bid for Fire Rescue Vehicles and Other Fleet Equipment." These ambulances are
compliant with KKK- A -1822E and Collier EMS' driver /operator training utilizes DOT Emergency
Vehicle Operators Course (EVOC) National Standard Curriculum.
Safety of our crews, patients and the general public are first and foremost. In June 2011 one of our
trucks experienced a complete electrical failure during a transport. Not only did the truck have to be
towed to motor pool for repair, but another ambulance had to be summoned to resume the transport of
the patient. The next day, this same truck was again transporting a patient when the engine failed. Once
again, delay in transport and another ambulance taken from its zone to continue the transport. Recently
another truck experienced a power failure (again during transport) and was out of service for five days.
In the past 12 months, we have. experienced 8 instances of apparatus failure either enroute to a call or
during transport. Breakdowns are particularly problematic for its because our County is so large and
transports often initiate in very remote areas, many miles from our local hospitals. Breakdowns of any
type can have adverse outcomes for critically ill or injured patients. We are especially sensitive to this
danger because a truck broke down a few years ago while transporting a patient from a remote location
and although a replacement unit was dispatched and completed the patient transport, she eventually died
from injuries she had suffered. In addition to the patient risks, breakdowns affect coverage in other
zones as another unit is dispatched to replace a broken -down ambulance. Because our crews are spread
so thin and often have long waits at hospitals, particularly during the heavy visitors' season, any
equipment failure puts a strain on the entire system and consequently the public may suffer slower
response times to emergencies, in addition, there is a- liability to the County for worn out or unsafe
equipment.
Collier County is served by 83 miles of interstate 75 (Alligator Alley). Approximately 68 of these miles
are crossing the state through national preserves and everglades swamplands,, According to the Florida
Department of Transportation, from 2008 to 2011, the annual average traffic count went from 9 million
to 9.5 million vehicles. During that time period, Collier County EIS responded to about 3600 calls per

year on that highway system, with about 350 per year in the remote areas of 1 -75 between Collier and
Broward counties,
Cost/Benefit:
Collier EMS currently has a� fleet of 33 ambulances. In 2007 we began purchasing Horton ambulances
with boxes that are better quality and more rugged to allow them to be remounted on new chassis at least
once; in effect doubling the lifespan of the box and reducing the replacement costs of new ambulances.
The remounting process is scheduled to begin in FY14. The County's Fleet Management Department
has recommended replacement of ambulances each year, but the department has been unable to keep up
with these replacements due to budget constraints. Tile department currently has seven old and high
mileage ambulances that are recommended for replacement. All but two have in excess of 200,000

hospitals, there is even greater wear and tear on the engines than the mileage reflects. One source
indicates that 60 minutes of idling time is equivalent to between 80 and 120 minutes of driving time.
Also, because of their age and type, these trucks are not suitable for the remount plan. In fact, the
manufacturer of these older trucks has since gone out of business. Even if this had not been the case, the
welds on the boxes are broken and their general condition does not make them conducive to a remount
plan.
Cost, of course, is always a factor. Over the past several years, maintenance and repair costs have
skyrocketed. It has become a financial burden keeping these old units operational. All of the trucks
have exceeded their cost in maintenance and repair costs, some by almost twice their purchase price. In
addition, repair and -maintenance resources that are being spent on these units could be better utilized on
other -more viable vehicles.
Should this grant not be funded, Collier EMS is in jeopardy of not having sufficient transport vehicles to
keep. up with our call volume, postponing treatment and transports, which may ultimately result in
negative outcomes for our patients, as well as possibly limiting our capacity foxmt►izi ;l -pi d:situaiio s;
Statement of Effect:
If awarded this grant through the assistance of the U. S, Department of Homeland Security, Collier
County EMS will be able to assure that our citizens and visitors have reliable transport vehicles to
achieve acceptable response times and favorable patient outcomes. As indicated previously, tourist
season increases our call volume significantly, requiring extra units be put into service. in addition,
extra units may be required during hurricane season, which runs from June through November.. It is.
imperative that each and every truck is ieady and able to roll when extra units are placed into service
during a severe storm or any other type of emergency. Measurable results would be fewer vehicle
failures, fewer interrupted responses and transports as well as lower maintenance costs. Collier E1v1S
does not track responses by specific trucks but rather by medic unit. Numbers of responses vary greatly
based on zone demographics. For example, in 2011 one of our units in a remote, sparsely populated

zone responded to 287 calls, while another unit in a densely populated urban zone responded to 2,060
calls.
Additional Information:
As indicated previously, our County is experiencing critical budget reductions. Even though our Fleet
Department has recommended a replacement schedule for our ambulances, we have been unable to
accomplish this goal due to budget constraints. Our capital budget to purchase worn out medical
equipment has been completely eliminated for FY 2012 and the operating budget has been cut for four
consecutive years. No pay raises have been given, In fact, several years ago the union members voted
to not take 'a scheduled COLA adjustment that was contractually obligated in an effort to avoid layoffs
which would have occurred due to the budget constraints. The safety of our crews and patients is being
threatened by vehicles that are experiencing increased mechanical failures due to age and wear and tear.
The award of this grant will allow us to remove the most problematic of our fleet and replace them with
reliable vehicles that will last twice as long due to our remount program, improve response times and
provide safe and reliable transports for our patients.

3. Drug -Free Workplace (Grantees other than individuals)
As required by the Drug -Free Workplace Act of 1988, and implemented at 44CFR Part 17, Subpart F, for grantees, as
defined at 44 CFR part 17, Sections 17.615 and 17.620:
(A) The applicant certifies that it will continue to provide a drug -free workplace by:
(a) Publishing a statement notifying employees that the unlawful manufacture, distribution,
dispensing, possession, or use of a controlled substance is prohibited in the grantee's
workplace and specifying the actions that will be taken against employees for violation of
such prohibition;
(b) Establishing an on -going drug free awareness program to inform employees about:
(1) The dangers of drug abuse in the workplace;
(2) The grantees policy of maintaining a drug -free workplace;
(3) Any available drug counseling, rehabilitation and employee assistance
programs; and
(4) The penalties that may be imposed upon employees for drug abuse
violations occurring in the workplace;
(c) Making it a requirement that each employee to be engaged in the performance of the
grant to be given a copy of the statement required by paragraph (a);
(d) Notifying the employee in the statement required by paragraph (a) that, as a condition of
employment under the grant, the employee will:
(1) Abide by the terms of the statement and
(2) Notify the employee in writing of his or her conviction for a violation of a
criminal drug statute occurring in the workplace no later than five calendar days
after such conviction.
(e) Notifying the agency, in writing within 10 calendar days after receiving notice under
subparagraph (d)(2) from an employee or otherwise receiving actual notice of such
conviction. Employers of convicted employees must provide notice, including position title, to
the applicable DNS awarding office, i.e. regional office or DHS office.
(f) Taking one of the following actions, against such an employee, within 30 calendar days of
receiving notice under subparagraph (d)(2), with respect to any employee who is so
convicted:
(1) Taking appropriate personnel action against such an employee, up to and
including termination, consistent with the requirements of the Rehabilitation Act
of 1973, as amended; or
(2) Requiring such employee to participate satisfactorily in a drug abuse
assistance or rehabilitation program approved for such purposes by a Federal,
State, or local health, law enforcement or other appropriate agency.
(g) Making a good faith effort to continue to maintain a drug free workplace through
implementation of paragraphs (a), (b), (c), (d), (e), and (f).

Section 17.630 of the regulations provide that a grantee that is a State may elect to make one certification in each Federal
