Recommendation to approve Amendment #2 to the Letter of Agreement with Quest
Diagnostics, Inc. to provide follow up lipid and A1C testing services in support of the
Wellness Based Incentives Program in an estimated amount of $10,410.
OBJECTIVE: To provide follow up lipid and Al C testing in support of the Wellness Based
Incentives Program.
CONSIDERATIONS: On January 1, 2009 the Risk Management Department implemented a
behavior based wellness incentives program into the Collier County Group Health Plan. The
purpose of the program is to engage employees to participate in various wellness related
activities or "qualifiers" to prevent illness and to manage chronic disease. Employees are
enrolled in one of three plans with progressively better benefits (Basic, Select, or Premium)
based upon their completion of these qualifiers. The qualifiers include the completion of a
personal wellness profile; the gathering of weight, height and waist measurements; a complete
blood analysis; age and gender based screenings; and participation in diabetes management
and smoking cessation programs (if applicable). The participation rate by employees has
consistently exceeded 90 %.
Quest Diagnostics, Inc. provides the blood analysis, biometric measurement and wellness
reporting portion of the program. Once the testing is completed, the results are sent to the
onsite Health Advocate and incorporated into the member's Personal Wellness Profile report.
Since the inception of the program, an emphasis has been placed on identifying and managing
diabetes. In 2012, an additional emphasis is being placed on managing cardiac health. The
Wellness staff has developed two educational and medical management programs to address
diabetes and lipid management which require a second follow up test in addition to the initial
test. The purpose of this second test is to measure the progress of the member and to
clinically support the educational components of these programs. It is recommended that these
additional tests be incorporated in the existing Quest test panel.
Amendment #2 to the current agreement with Quest Diagnostics identifies the two additional
test panels and the associated cost per test. These tests include a Lipid Panel and an Al C
test. The blood draws will be managed through the MedCenter onsite clinic. Thus, no
additional office visit costs will be incurred.
The commencement date of Amendment #2 is effective March 1, 2012.
FISCAL IMPACT: The cost per test is $34.50 for the Lipid test and $24.00 for the Al C test. It
is estimated that approximately 180 employees will need to complete the Lipid test and 175
employees will need to complete the Al C test. Total annual cost is expected to be $10,410.
Funds are budgeted within Fund 517, Group Health and Life Insurance to fund the program.
GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with
this item.
LEGAL CONSIDERATIONS: This item has been reviewed and approved by the County
Attorney's Office, is legally sufficient for Board action and only requires a majority vote for
approval —SRT.
RECOMMENDATION: It is recommended that the Board of Commissioners approves
Amendment #2 to the Letter of Agreement with Quest Diagnostics, Inc. and authorizes the
Chairman to execute the amendment.

SECOND AMENDMENT OF AGREEMENT BETWEEN
Collier County Government AND
QUEST DIAGNOSTICS INCORPORATED
THIS SECOND AMENDMENT is effective as of the 1st day of March, 2012 by and between Quest
Diagnostics Incorporated, with a principal place of business located at 10101 Renner Blvd., Lenexa, KS
66219 ( "Quest Diagnostics "), and Collier County Government, 3301 East Tamiami Trail, Bldg. D, Naples,
FL 34112.
WHEREAS, the parties have previously entered into a 2011 Letter of Agreement & Event Scheduling
Memo for Blueprint for Wellness Services on the 12 «' day of July, 2010, and a First Amendment dated
March 21, 2011, for the performance of Blueprint for Wellness Services, (collectively, the "Agreement");
and
WHEREAS, the parties, by this Second Amendment, desire to amend the Agreement by adding 2
additional testing panels.
NOW, THEREFORE, for and in consideration of the covenants and agreements made herein and of the
mutual benefits occasioned by the terms of this Second Amendment, the presence of which are mutually
acknowledged and confessed, the parties each agree as follows:
The following two additional testing panels shall be added to the Agreement:
1. Lipid Panel - $34.50 per participant
2. Hemoglobin Ale Test — $24.00 per participant
Executed in duplicate originals on the dates set forth the below by duly authorized representatives of the
parties.
