EXECUTIVE SUMMARY
Recommendation to approve amendments to service agreements and corresponding budget
amendments to reflect actual award of the Community Care for the Elderly, Alzheimer's Disease
Initiative, and Home Care for the Elderly grants for the Collier County Services for Seniors
program from the Area Agency on Aging for Southwest Florida, Inc., dba Senior Choices of
Southwest Florida(Net Fiscal impact$64,742)
OBJECTIVE: To reflect the difference in estimated and actual grant award that will allow uninterrupted
support services to Collier County Services for Seniors (CCSS) frail, elderly clients through the
Community Care for the Elderly (CCE), Home Care for the Elderly (HCE), and Alzheimer's Disease
Initiative(ADI) grant programs.
CONSIDERATIONS: Collier County Services for Seniors (CCSS), managed within the Housing,
Human and Veteran Services (HHVS) Department, has provided support services to Collier County's
qualified seniors for over 32 years through the CCE, HCE, and ADI grants. These grants are funded by
the Florida Department of Elder Affairs (FDOEA), through the Area Agency on Aging of Southwest
Florida, dba Senior Choices of Southwest Florida(Senior Choices). These grants enable CCSS to provide
services to seniors and their caregivers, allowing them to remain in their homes and live with
independence and dignity.
On July 24, 2012, Item 16.D.15, the Board of County Commissioners approved the current service
agreements for CCE, ADI, and HCE which end on June 30, 2014, and allow for two year renewals.
Funding awards for the first year (2012-2013) ended June 30, 2013. In order to prevent a disruption in
service, a budget amendment based upon the previous year's allocation in the amount of$777,105 was
approved on May 28, 2013, item 16.D.14. The proposed amendments to the service agreements for the
second program year (2013-2014) were received from the grantor agency in the amount of$835,142 and
reflect an increase of$58,037.
On May 28, 2013, Item 16.D.14, a budget was approved for client co-payments for the CCE and ADI
grants, based upon prior year collection goals established by Senior Choices. A budget amendment is
required for the difference between estimated and actual co-payment goals established, a$3,500 increase.
The CCE grant has a 10% match requirement of $6,205, which will be partially met through the co-
payments received ($3,000) and results in a net local match impact of$3,205. ADI and HCE have no
matching requirements.
The following table provides a detailed breakdown of the necessary adjustments to the award,co-
payment and required local match budgets set forth in the proposed amendments:

Note: The HCE Basic Subsidy of $22,896.00 is retained by the grantor agency, and therefore is not
included within this table,as it is not a part of the CCSS managed budget.
Approval of the following documents by the County Manager is subject to formal ratification by the
Board of County Commissioners. If the decision by the County Manager is not ratified by that Board,the
document(s) shall be enforceable against Collier County only to the extent authorized by law in the
absence of such ratification by that Board.
FISCAL IMPACT: Budget amendments are required to reflect the difference between the estimated
and actual grant awards, an increase of $58,037.00 for the FY13 CCE, ADI, and HCE grants to the
Human Services Grant Fund (707), Projects 33271, 33272, and 33273. Budget amendments are also
required for increased CCE local match requirement of $3,205, CCE Co-payment increase of $3,000
offsetting the remainder of the 10% match requirement, and for ADI Co-payment increase of$500, to
Human Services Match Fund (708), Projects 33271 and 33723. Local match is available through carry
forward in the Collier County Seniors Fund(123).
GROWTH MANAGEMENT: There is no growth management impact from this recommendation.
LEGAL CONSIDERATIONS: A standard form amendment provided by Senior Choices of Southwest
Florida was used for each agreement. This item is approved as to form and legality, and requires majority
vote for approval.—JAB
RECOMMENDATIONS: Approve contract amendments and attestation statements between Collier
County and the Area Agency on Aging for Southwest Florida, Inc. dba Senior Choices of Southwest
Florida and authorize the budget amendments.

The purpose of this amendment is to add an additional paragraph to Item#4. Contract Amount to read as
follows: The Agency agrees to pay for contracted services according to the terms and conditions of this
contract in an amount not to exceed $104,961 for year two, 2013 –2014, or the rate schedule, subject to the
availability of funds. Any costs or services paid for under any other contract or from any other source are not
eligible for payment under this contract; and revise ATTACHMENT III Exhibit- 1 and ATTACHMENT
VIII.
This amendment shall be effective July 1,2013.
All provisions in the agreement and any attachments thereto in conflict with this amendment shall be and
are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level
specified in the agreement.
This Amendment, three pages, is hereby made a part of this agreement.
