Parents recently took 4 students each from Naples, Lely, and Barron
Collier High Schools to a National Conference on Drug Abuse in Atlanta

Conference and approximately 3000 students attended this years con-
ference. She stated that a group of experts in varying fields of
expertise spoke at the conference and some were invited to Naples next
year to speak at the various schools. She also advised that Nancy
Reagan was one of the speakers. Mrs. Howard then introduced Tina
Ryner, Jackie Lloyd, Justin Graham, and Trae Trimble. Each of these
students gave a brief presentation of what they received from
attending the Pride r.onference.

Collier County is higher than either Lee or Sarasota Counties with
1434 deliveries in 1986. She stated that 37% of those births were to
medically indigent women, as opposed to 32% for Lee County and 21% for
Sarasota County. She noted that Collier County has 4 obstetricians
with no midwives; Lee County has 18 obstetricians with 7 midwives;
and Sarasota County has 14 obstetri¢ians with 5 midwives.

that by dividing the number of 1986 births by the number of obstetri-

ciðns, Collier County will have 444 births per year, or 37 births per
month, per obstetrician. She noted that the American College of
Obstetricians/Gynecologists recommends that a full practice ÒB/GY~~~~~'~
have 180 to 200 deliveries per year or 15 to 17 per month for quality

Dr. Polkowski stated that various options have been explored to
relieve the burden and the private sector is actively recruiting new
doctors to the area. She advised that a current obstetrician is
dropping obstetrics because of the high malpractice insurance rate in
Collier County. She said that the difficulties being encountered in
attracting new obstetricians to this County have been the high
malpractice insurance rates, as well as the high indigent load. She
stated the indigents, as a gr~up, have higher medical problems which
lead to increased difficulties during pregnancy and greater care and

responsibility for the obstetrician. She said the indigents also have
an increased risk of.poor pregnancy outcome ðnd babies are damaged
more, ,which leads to higher taxpayer dollars.
Dr. Polkowski stated that this proposal is for temporary relief
of the current situation by contracting with a private group called
Sunlife OB/GYN Services. She said this is not,for all indigent
care, but at least 71% and the balance will still be carried by the
private sector. She stated the contract calls for two obstetricians
who will do indigent care only. She stated these two obstetricians
will deliver, perfor~ surgeries, and conduct high risk clinics. She
said the routine regular care will continue to be provided by the
regular Health Department Staff.
Dr. Polkowski stated the contract will be between HRS and the

contract will at least ease the crisis that exists of too few

they are paying for a service that Collier County obstetricians do not
have time to give.

Dr. Polkowski stated that this proposal is being presented now
rather than at regular budget time because there will be only 4 pri-
vate physicians as of July 1st and it will take 90 days for Sunlife to
institute the service and the fall period of September and October
historically show an increased number of births. She also noted that
if one of the 4 physicians becomes ill, the burden on the remaining 3
physicians will be totally unrealistic.

Dr. Polkowski stated that the $201,000 is "up-front money". She
stated the actual net cost to the County will be approximately

$111,000 or less due to the return of fees collected from patients :fi~J;
third party payors. She advised that the net effect could be lower
thah estimateò du~ to State Legislation passed last week al!owing'an
expansion of the Medicaid Program coverage and service payments~ .
Dr. Francisco Smith, Chairman of Department of Obstetrics and
Gynecology for Naples Community Hospital, stated there is a deep con-

cern about the seriousness of this crisis. He stated that since the
last time he spoke to the Board the situation has not improved, but
rather, has become worse. He advised that he has personally inter-
viewed 4 new candidates, but they have not made any commitment to
practice in Collier County, largely because of the high malpractice
insurance rates in Collier County and the high indigent load. He

stated that when a new doctor comes to this county he has to pay his
malpractice .insurance in advance and probably will receive only indi-
gent patients for the first year.

Dr. Smith stated that one of his responsibilities as Chairman of
the OB/GYN Department is to insure that the public is given the best
medical care available. He said the present situation is risky and
unsafe for the mother and baby, as well as the physician. He advised
that they have worked very hard to alleviate the present situation,
and the best immediate solution for temporary relief is the Sunlife
Program. He ~dvised that this program has proven to work very well on
the east coast of Florida.
'·'Dr. Smith stated that there has been some concer~expressed about

this program setting a precedent for requests from other medical spe-
cialty fields. He stated that the crisis does not exist in any other
:~~~ field. He advised that 25% of the Florida obstetrici~ns have stopped.
practicing. He also advised that when recruiting young doctors coming
out of residency programs it must be done before July 1st and,~if suc-
cessful, the physician would arrive in November. He stated they have
begun recruitment for the next graduates in July, 1988.
In answer to Commissioner Pistor, Dr. Smith stated that the
malpractice rate in Collier County are quite a bit higher than in Lee
County. He stated that he started practicing obstetrics in 1982 and

paid $7,000 for malpractice insurance. He advised that this year he
will pay $89,000 for obstetrical premiums. H.e...·said, to his knowledge,
the Orlando, Tampa, and Ft. Myers areas are paying $60,000. He stated
that for some reason Collier County seems to be getting a "trickle"
ef.fect from Dade and Broward Counties.
Also in answer to Commissioner Pistor, Dr. Smith stated that the
majority of the medically indigent patients are the agricultural
migrants, however, a good number are the same private paying patients
of three or four years ago. He stat~d that insurance deductib1es' and *
overage costs have not kept up with medical costs and deductibles
which used to be $200 or $300 are now approximately $1,000. He a.:~~~~.;
stated that jobs which do not provide insurance are usually low paying
jobs and those people usually qualify as ~edically indigent.
Dr. polkowski advised that many of Collier County indigents are
seeking treatment at Lee Memorial and it is becoming a great concern
to Lee Memorial. She also stated that many of the Health Department
patients have one member of the household working in a low paying job
without insurance. She also mentioned that the Sarasota County
Program has no cap on income and anyone can use their services.
Chairman Hasse stated that he received a letter signed by 18 area
physicians which states there are several new physicians waiting to
practice in Naples, with one entering the Bonita area immediately.

experiencing these problems. He stated that this is not a spur of the
moment request, but "as been studied and discussed at every level
within the hospital and everyone is concerned with the present
situation.
Dr. Smith stated the problem is real and will not go away by
itself. He said the area is growing at an incredible rate, and that
is not going to change. He urged the Commission to help solve the
crisis before it becomes disastrous. He stated again that they are
requesting help for ~ne year. only.
Ms. Sharon Warnecke, Health Department Nursing Program Specialist,
advised that during the time other p~ssibilities were being explored,
Dr. Warren Crosby, head of President Reagan's Ì986 Maternal Task Force
was contacted. She said that Dr. Crosby advised there was no other
~
group, with the exception of Sunlife that does contractual OB/GYN care.
~~~~~~he stated they also could offer no alternative to using the type of
system that is currently being utilized, other than Sunlife.
Chairman Hasse stated that he could not dispute the word of 18

Sunlife OB/G'fN Services, stated that their co'n,pany exists because of
the real need in this field of medicine. In answer to Commissioner
Pistor, Dr. Gardy sta~ed that Sunlife is able to recruit because there
are physicians who prefer to work for a company rather than themselves
because they have set days off and vðcations. He stated their physi-
cians have no other obligations and they have enough doctors to cover
illnesses. He stated they pay wages comparable to private practicing
physicians and have a mechanism that makes malpractice insurance a
little bit more affordable.
In answer to Chairman Hasse, Dr. Gardy stated the members of
Sunlife's Board of Directors are: steven Scott, Dan L~wis, John
Hemingway, anè Jolfn Larson. He ~aid the holdi'11g company for!' Sunlife
is Coastal Group, Inc. Also in answer to Chairman Hasse, Dr. G¡ra~ ~c
assured him that no one on the Board of Directors is from the Naples
area.
In answer to Commissioner Glass, County Manager Dorrill stated'
that for the FY 1987/88 there is $165,000 for IPOP which they are
asking to eliminate and reappropriate with new money to equal approxi-
mately $300;000 per year. He s.:!lid this is the contract amount for the
S'.Jnlife OB/GYN Services. Dr. Polkowski stated that the contract for

stated that the counties surroundihg Collier have excellent indigent
programs and it is very doubtful that anyone would come to Collier
from the East Coast or the Keys just to use this program¡ the Health
Department works wi'~ and has referral services with other physicians
in other specialty fiel~.è. and there is no crisis in any of them
except OB/GYN; the purpose of thi9 request is to relieve the indigent
load for a period of 12 months to allow time and better environment
with which to recruit new OB/GYN physicians to the area¡ it is accep-
table if the Board states they will not reconsider this request next
year.

stated that Palm Beach County does not give any funding for this type
of service, and she feels Collier County will be setting a very

Dr. Polkowski advised that Palm Beach County hospitals are funded

experts. He stated he has a lot of conf~dence in Dr. polkowski. He
stated that this item does not crel!te a precedence unless the 'Board'

allows it to become ~ precedent; there is a real problem in this
county with OB/GYN services for permanent residents as well as indi-
gents and the Board needs to deal with County problem~;rthis shd~ld be
accomplished through the regular budget process, however ~ the"progrl!m"
takes 90 days to institute and the most critical period is September
and October; and, the amount of money being requested is not substan-
tial when looking a~ the grand schème of things. He stl!ted he sup-
ports the program, and the Board needs to take a position on 'this'
matter today.

Chairman Hasse stated his feelings that this program will make it
more attractive for tllegal aliens to come to Collier County to have
their ,babies.
In answer to Commissioner Glass, Mr. Dorrill stated that the money
would have to come from the General Reserve Fund and would have to be
new money or replace other programs for next year. He stated the
alternative would be to raise taxes.
Commissioner Saunders moved to approve the $201,000 tor the
Sunlite OB/GYN Services Program with the stipulation that it will bè a
one year program and will not bft expanded or continued beyond that
time. The motion died tor lack ot a second.
Commissioner Saunders moved, seconded by Commissioner Glass and

of Venice, requesting a six foot variance from the maximum height of a
principal structure of 30 feet to 36 feet.
Planner Weeks stated that properties to the north, east and south
are zoned RSF-4 which has a height limitation of 30 feet, and property
to the west is zoned RMF-16, which has a height limitation of 100
feet. He said portions of the church belfry and towers extend a total
of 75 feet in height, however, these are specifically excluded from
the height limitations in the Ordinance. He said the request is
strictly acoustical and Staff finds no land related hardship, and for

Mr. Weeks stated that Section 7.27 of the Zoning Ordinance allows
for minor changes in height subject to Board approval. He advised
Staff feels these changes are minor and recommend approval. He stated
that the CCPC heard this petition on May 21, 1987 and recommended
approval with the stipulation that no homesites be situated to face
the wall.
In answer to Commissioner Pistor, Mr. Weeks stated that this petri.

Hasse, Mr. Weeks stated that this request refers only to the areas
outlined on the disp~ðyed map, and 'if the petitioner wishes to extend
beyon~ that point, the request would have to be reviewed again.
Mr. Bob Lockhart, Wilson, Miller, Barton, Soli & Peek, Inc.,
stated that the overpass in question ranges between 13 feet and 4 feet
in height and the wall will be used to shield ~he residents from traf-
fic on that road. He stated the wall on Santa Barbara could be
stepped down if the Board desires.
Mr. Bob Siebold, resident, stated that he is ag~in8t walls and
requested the Board to require trees instead of a wvll.

installed to propel the leachate to retention/treatment ponds for

been several request~ to change the name of the Golden Gate
Neighborhood Park to Aaron Lutz Neighborhood Park. He advised that
Mr. Lutz was the first principal at Golden Gate Elementary School and

Mr. O'Donnell stated that the Parks & Recreation Advisory Board
recommended approval of this request at their last meeting in May.

FOR PHASB I OF THE SOUTH COUNTY REGIONAL WASTEWATER FACILITIES IN THE

Utilities Engineering Director Madajewski stated thðt Phase I of
the South County Regional Wastewater Facilities contains 10 master
sewage pump stations. He sðid 5 of these sites are reðdy to be
appraised in order to move forward. He stated that three appraisal
firms on the County's appraisal list were contacted for' quotes ðnd twó
declined to bid. He advised that Some out-of-county appraisal firms
were contacted, however, they stated they could not meet the It'ime_,...~.......;
frama involved in the project.
11r. Madajewski advised that the bid r~ceived is from the firm of
Carroll & Carioll in the amount of $16,000 for all 5 properties.
Chairman Hasse expressed his displeasure with receiving only one

Assistant to the County Manager Olliff stated that Staff is recom-
mending thal the County Manager be authorized to approve consent
agenda items, budget amendments, and emergency items during the
Board's absence. He stated that this procedure has been followed
annually during the Board's vacation period.

COUNTY MANAGER CONTRACT - APPROVED WITH CHANGBS AND CHAIRMAN

Chairman Hasse stated that he has met with Neil Dorrill and they
have,agreed on a contract. He said he distributed copies of this
'," agreement to the Board members and the County Attorney on June 5,.
1987. He stated his feelings that Mr. Dorrill is knowledgeable and
will be an asset to Collier County.
County Attorney Cuyler stated that there are a few changes which
need to be made for clarification. County Manager Dorrill stated that
he has not had a chance to review the changes, however, he suggested
that the contract be approved 8S presented, and amendments for clari-
fication be made at next week's meeting. Chairman Hasse expressed his
extreme displeasure that he was not advised of these changes before'
the meeting. Commissioner Pistor stated his feeling that the contract
should be approved as presented and the amendments, could then be

Section 1 - Add at end of paragraph: "as amended by County
Ordinance No. 87-43, and ðs it may from time to time be
amended in the future.
Section 2a - Add after the words "at any time,": "with or without
cause in accordance with the provisions of Ordinance No.
84-44, as amended by Ordinance No. 87-43."
Section 2b - last line: Change the phrase "Section 2 b" to
"Section 3 b".
Section 3c - This section should read: "In the event..that. Employee
vOluntarily resigns his position, suspension with pay after
such resignation is not considered discipline under County
Manager Ordinance No. 84-44, as amended by County Ordinance
No. 87-43."
