EXECUTIVE SUMMARY
Recommendation to approve a Resolution superseding Resolution No. 2012 -192 and authorize staff
to honor the Elderly/Disabled reduced fare for the subcontracted transportation provider with the
Florida Commission for the Transportation Disadvantaged that provides transportation services
under the non - emergency transportation Medicaid contract for Collier County
OBJECTIVE: To ensure that recipients that qualify for non- emergency Medicaid transportation have
access to affordable transportation.
CONSIDERATIONS: - The Florida Commission for the Transportation Disadvantaged (FCTD) is
responsible for contracting with a vendor to provide non - emergency transportation (NET) for eligible
Medicaid recipients. 'On, March 27, 2012 the Board elected to cease being the provider responsible. for
NET. Effective July 1, 2012. FCTD contracted with Medical Transportation Management (MTM), Inc. to
provide NET for eligible Medicaid recipients in Collier County.
MTM is reimbursed approximately $65,500 per month for all Medicaid NET trips and is experiencing
costs of approximately $75,000 per month. The FCTD is attempting to assist MTM with lowering
operating costs so that it can continue to provide service in Collier County. One facet of the cost control
effort is to utilize Collier Area Transit (CAT) fixed route service in lieu of dedicated NET trips.
Transitioning appropriate passengers to the CAT system is the most cost- effective way to manage NET
costs. The currently approved CAT fare structure provides for a discounted fare (half of the full fare) for
the elderly and disabled.. FCTD has requested that the reduced fare structure be extended to the
n subcontracted provider for Medicaid NET. In the 5 months that MTM has provided services in Collier
County, it has purchased 100 daily fares and 50 monthly fares at the normal full -fare. MTM anticipates
purchasing 150 more daily passes over its remaining contract period (through June 30, 2013).
The Local Coordinating Board (LCB) for the Transportation Disadvantaged endorsed extending the
discounted fare structure to MTM in order to facilitate the continuation of Medicaid transportation
services in Collier County.
Alterative Transportation Modes (ATM) staff recommends Board approval of the extension of the
discounted fare structure to MTM and that Resolution No. 2012 -192 be superseded to indicate that the
discounted Elderly/Disabled Pass type fare also applies to the subcontracted transportation provider
responsible for the Collier County non - emergency Medicaid transportation contract.
ATM staff will require quarterly reports from the provider showing the number and type of passes
purchased and the number and type of passes provided to their clients for use. These reports will be
provided at the regularly scheduled LCB meetings.
FISCAL IMPACT: If MTM were to purchase an additional 150 daily passes at the Elderly/Disabled
reduced fare, Collier County would receive $300 in lieu of the full fare amount of $600, therefore, the
fiscal impact to Collier County would be $300.00 for the remainder of the contract period. Based on the
amount of passes that the provider has purchased and anticipates purchasing, Collier County would
receive $1,375 in lieu of the full fare amount of $2,750, therefore, the annual fiscal impact is anticipated
to be $1,375.00.
LEGAL CONSIDERATIONS: This item has been reviewed and approved by the County Attorney's
n Office, is legally sufficient for Board action and only requires a majority vote for approval —SRT.

GROWTH MANAGEMENT IMPACT: There is no growth management impact associated with this
Executive Summary.
RECOMMENDATION: That the Board approves the attached proposed Resolution superseding
Resolution 2012 -192 and authorize staff to extend the Elderly/Disabled reduce 1hre to the subcontracted
transportation provider with the Florida Commission for the Transportation Disadvantaged for
transportation services under the non - emergency transportation Medicaid contract for Collier County.

A RESOLUTION SUPERSEDING RESOLUTION NO. 2012-
192, WHICH ESTABLISHED THE COLLIER AREA
TRANSIT (CAT) BUS FARES AND TRANSIT FEE POLICY,
BY ADDING AN ADDITIONAL NEW POLICY
PROVIDING FOR AN ELDERLY/DISABLED REDUCED
FARE FOR THE SUBCONTRACTED TRANSPORTATION
PROVIDER WITH THE FLORIDA COMIVIISSION FOR
THE TRANSPORTATION DISADVANTAGED THAT
PROVIDES TRANSPORTATION SERVICES UNDER THE
NON- EMERGENCY TRANSPORTATION MEDICAID
CONTRACT FOR COLLIER COUNTY.
WHEREAS, the Board of County Commissioners ("Board ") has the authority to
approve the Collier Area Transit (CAT) bus fares and transit fee policy; and
WHEREAS, on March 27, 2012, the Board voted to provide notice to the Florida
Commission for the Transportation Disadvantaged (FCTD) and the community of the
intent to discontinue providing Medicaid transportation service in Collier County
effective June 30, 2012; and
WHEREAS, on July 1, 2012, Medical Transportation Management, Inc. (MTM)
began providing transportation services as the subcontracted transportation provider with
the FCTD under the non - emergency transportation Medicaid contract; and
WHEREAS, on October 23, 2012, the Board adopted Resolution 2012 -192
establishing the CAT bus fares and transit fee policy; and
WHEREAS, in December 2012, MTM notified the FCTD of the intent to
discontinue providing Medicaid transportation services in Collier County because it is
operating at a loss; and
WHEREAS, on December 5, 2012, the Local Coordinating Board for the
Transportation Disadvantaged endorsed the County's provision of assistance to MTM,
Inc., in order to continue Medicaid transportation services in Collier County.
NOW, THEREFORE BE IT RESOLVED BY THE BOARD OF COUNTY
COMIVIISSIONERS OF COLLIER COUNTY THAT:
1. The purpose of this policy is to establish user fees that will partially offset the cost
of providing services to the taxpayers of Collier County.

2. The Board of County Commissioners shall set all fees and charges. No fees may
be waived, except as stated herein.
3. Collier Area Transit (CAT) is hereby authorized to set bus fares in accordance

COMMISSIONERS, COLLIER COUNTY, FLORIDA,
MEMORIALIZING THE BOARD'S PREVIOUSLY
APPROVED COLLIER AREA TRANSIT (CAT) BUS
FARES, INCLUDING THE COLLIER COUNTY
EMPLOYEE DISCOUNTED MONTHLY PASS,
ESTABLISHING THE COLLIER AREA TRANSIT FEE
SCHEDULE AND ESTABLISHING NEW SMART CARD
MEDIA FEES.
WHEREAS, on December 16, 2008, the Board of County Commissioners approved by
majority vote an increase to the Collier Area Transit (CAT) bus fares, after affording the public
an opportunity to offer comment, and fully considering those public comments (Agenda Item
I OD); and
WHEREAS, those previously approved fares are set forth and memorialized in the table
set forth in numbered paragraph one (1), below; and
WHEREAS, table one (1) also incorporates a previously Board approved $10.00
discounted monthly pass for Collier County employees; and
WHEREAS, the Director of the Collier County Alternative Transportation Modes
( "ATM") Department is recommending that the Board of County Commissioners adopt the
CAT Fee Schedule set forth below in numbered paragraph two (2); and
WHEREAS, the ATM Director further recommends that the Board of County
Commissioners adopt the Smart Card Media fee set forth below in numbered paragraph three (3);
and
WHEREAS, the Board of County Commissioners has the legal authority to adopt a
resolution authorizing the establishment of CAT bus fares, fees and establishing a transit fee
Schedule.
NOW, THEREFORE BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY THAT:

2. The CAT Fee Schedule is hereby established to institute user fees that will partially offset the
cost of providing services to the taxpayers of Collier County. The Board of County
Commissioners shall set all fees and charges. No fees may be waived, except as provided by
the Board of Commissioners.
3. Smart Card Media Fees
CAT is in the process of adding electronic fareboxes to its fixed -route buses. These fare-
boxes require the use of Smart Cards; which are plastic, pocket size cards with an embedded
chip for transferring fare information to and from the electronic farebox. In order to offset
the Purchase and administrative cost for the Smart cards, a registration and replacement fee
will be charged. The cards are similar to a bus pass and will be required when purchasing a 7
day (weekly) or a 30 day (monthly) pass. The Smart Card initial cost of $2.00 will be borne
by all riders purchasing a pass. The registration fee is the cost for associating the card to the
owner in order to identify the owner and/or deactivate the card if lost or stolen. With the
registration the rider can ensure that the amount of funding remaining on the pass not be lost.
In order to issue a new card when lost or stolen a replacement fee of $2.00 will be charged.
The proposed cost for the replacement of the card for those that have registered their card is
$1.00. The Smart Card Media Fee schedule is as shown in Attachment "A."
4. This Schedule shall be effective November 1, 2012.
