1. Does the document require the chairman',original nature?

3. Original document has been signed/initialed for legal sufficiency. (All documents to be signed by
the Chairman,with the exception of most letters,must be reviewed and signed by the Office of
the County Attorney.

7. In most cases(some contracts are an exception),the original document and this routing slip
should be provided to the County Attorney Office at the time the item is input into SIRE. Some I�
documents are time sensitive and require forwarding to Tallahassee within a certain time frame or
the BCC's actions are nullified. Be aware of your deadlines!
8. The document was approved by the BCC on 03-27-2018 and all changes made during the
meeting have been incorporated in the attached document. The County Attorney's Office
has reviewed the changes,if applicable.
9. Initials of attorney verifying that the attached document is the version approved by the BCC,all v.

RESOLUTION 2018- 59
A RESOLUTION OF THE BOARD OF DIRECTORS OF THE COLLIER
COUNTY BOARD OF COUNTY COMMISSIONERS, APPROVING THE
AFTER-THE-FACT FILING OF AN APPLICATION FOR A SHIRLEY
CONROY RURAL AREA CAPITAL ASSISTANCE PROGRAM GRANT
APPLICATION WITH THE FLORIDA COMMISSION FOR THE
TRANSPORTATION DISADVANTAGED.
WHEREAS, this BOARD has the authority to file this Grant Application and to undertake a
transportation disadvantaged service project as authorized by Section 427.0159, Florida Statutes,and Rule
41-2, Florida Administrative Code.
NOW, THEREFORE,BE IT RESOLVED BY THE BOARD THAT:
I. The BOARD has the authority to file this grant application.
2. The BOARD authorizes and approves of staff's execution and filing of the Shirley Conroy Rural Grant
application on behalf of the Collier County Board of County Commissioners with the Florida
Commission for the Transportation Disadvantaged.
3. The BOARD'S Registered Agent in Florida is:Jeffrey A. Klatzkow,County Attorney. The registered
address is 3299 Tamiami Trail,Naples, Fl 34112.
4. The BOARD authorizes its Chairman to sign any and all agreements or contracts which are required in
connection with the application.
5. The BOARD authorizes the County Manager to sign any and all assurances, reimbursement invoices,
warranties, certifications and any other documents which may be required in connection with the
application or subsequent agreements.
THIS RESOLUTION IS ADOPTED this 27th day of March 2018, after motion, second and majority
vote in favor of adoption.
