EXECUTIVE SUMMARY
This item requires that ex parte disclosure be provided by Commission members. Should a hearing
be held on this item, all participants are required to be sworn in. Recommendation to approve a
Resolution renaming Creekside Way to Arthrex Boulevard, which is located in Creekside
Commerce Park West Unit One, Tract R, east of Goodlette -Frank Road and just south of
Immokalee Road in Section 27, Township 48 South, Range 25 East, Collier County, Florida.
OBJECTIVE: For the Board of County Commissioners to consider a Resolution to rename
Creekside Way to Arthrex Boulevard.
CONSIDERATIONS: The Petitioner has met the requirements for requesting a street name
change under Section 22 -358, Collier County Code of Laws and Ordinances.
The subject street is entirely within the Creekside Commerce Park West subdivision. A petition in
support of the proposed change, signed by sixty -six percent of the property owners abutting the
street, is required to be submitted. In this case, the applicant has provided support from one
hundred percent of the property owners abutting Creekside Way.
A "supermajority vote of at least four affirmative votes, shall be required to effectuate the
renaming of the street or roadway" (Section 22.357, Collier County Code of Laws and
Ordinances).
FISCAL IMPACT: Fiscal impact will be incurred in the form of staff time spent noting changes
on plats by the Engineering Section, map changes by the GIS Section and address records by the
Addressing Section; these impacts have been considered as part of the $500.00 review fee paid by
the applicant. Fiscal impact to the Fire District, Sheriff, Post Office, Emergency Medical
Services, the Emergency Operations Center, and 911 Emergency Services will be incurred in the
form of costs to update records and maps: The Petitioner will bear the cost of purchasing and
installing new street signs.
GROWTH MANAGEMENT IMPACT: The renaming of the street will have no impact on the
growth management plan.
NEIGHBORHOOD INFORMATION MEETING: A Neighborhood Information Meeting
was held at Naples Daily News Community Room on July 11, 2014. The applicant and County
Staff were the only attendees.
LEGAL CONSIDERATION: This item has been approved as to form and legality. Four
affirmative votes are required for Board approval. (SAS)
COLLIER COUNTY PLANNING COMMISSION RECOMMENDATION: The Collier
County Planning Commission does not review street name change petitions.

COMMISSIONERS OF COLLIER COUNTY, FLORIDA,
RENAMING CREEKSIDE WAY TO ARTHREX
BOULEVARD, WHICH STREET IS LOCATED IN
CREEKSIDE COMMERCE PARK WEST UNIT ONE
TRACT R, IN SECTION 27, TOWNSHIP 48 SOUTH,
RANGE 25 EAST, COLLIER COUNTY, FLORIDA;
PROVIDING FOR AN EFFECTIVE DATE. [SNR-

WHEREAS, the legislature of the State of Florida by enacting § 336.05, Florida Statutes,
has conferred on all counties in Florida the power to name or rename streets and roads, except for
certain state roads; and
WHEREAS, Collier County pursuant thereto has adopted Collier County Ordinance No.
07 -62, as codified in Chapter 22, Article X, of the Collier County Code of Laws and Ordinances,
which establishes criteria and procedures to rename streets or roads; and
WHEREAS, the Collier County Board of County Commissioners (Board) has held a
public hearing after providing notice as required by said ordinance and has considered the
advisability of granting the application of Arthrex, Inc., on behalf of property owner Creekside
Commerce Park Property Owners Association, Inc., to rename Creekside Way to Arthrex
Boulevard, as illustrated on the attached Exhibit "A ". This street is located in Creekside
Commerce Park Unit One Tract R, according to the plat thereof, recorded in Plat Book 29, Page
57 of the Official Records of Collier County, Florida; and
WHEREAS, the Board has determined that all criteria set forth in Chapter 22,
Article X, have been met, specifically those set forth in Section 22- 358(1) (b) as follows:
"Public notification process when renaming streets or roadways:
The applicant shall be responsible for conducting at least one neighborhood
informational meeting. This meeting shall be held 30 days prior to changing
the roadway and the petitioner is required to install street signs. The location
of the neighborhood informational meeting should be reasonably convenient to
those property owners affected by the renaming of the street and the facilities
shall be of sufficient size to accommodate expected attendance. The applicant
shall cause a display advertisement, in type no smaller than 12 point and said
advertisement shall not be placed in that portion of the newspaper where legal
notices and classified advertisements appear. The advertisement shall state the
purpose, location, and time of the meeting and shall be placed with a
newspaper of general circulation in the county at least seven days prior to the
` neighborhood informational meeting."; and

WHEREAS, the Board finds that there is no other street in incorporated or
unincorporated Collier County with the same name or any similar sounding name as is being
requested; and
WHEREAS, the Board has determined that it is necessary for identification purposes to
confirm the name of this street.
NOW THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF COLLIER COUNTY, FLORIDA, that:
Effective this date, the name of the street described in the above recitals is hereby
changed from Creekside Way to Arthrex Boulevard, and is confirmed as such.
BE IT FURTHER RESOLVED that this Resolution be recorded in the Public Records of
Collier County, Florida, and noted upon the maps of the street and zoning atlases of Collier
County, and notations made on the referenced Plat.
This Resolution adopted after motion, second and super majority vote this day of

Request: The petitioner is asking the County to approve this Street Name Change application to
revise the current street name from "Creekside Way" to "Arthrex Boulevard ".

Information Meeting will be held to provide you an opportunity to become fully aware of the

Notice is hereby given that on Tuesday, September 23, 2014 in the Board of County
Commissioners. Meeting Room, Third Floor, Collier Government Center, 3299 East
Tamiami Trail, Naples FL., the Board of County Commissioners (BCC) will consider
the enactment of a Resolution. The meeting will commence at 9:00 A.M. The title
of the proposed Resolution is as follows:
A RESOLUTION OF THE BOARD OF COUNTY COMMISSIONERS OF COLLIER COUNTY,
FLORIDA, RENAMING CREEKSIDE WAY TO ARTHREX BOULEVARD, WHICH STREET IS
LOCATED IN CREEKSIDE COMMERCE PARK WEST, UNIT ONE, TRACT R, IN SECTION
27, TOWNSHIP 48 SOUTH, RANGE 25 EAST, COLLIER COUNTY, FLORIDA, PROVIDING
FOR AN EFFECTIVE DATE.

A copy of the proposed Resolution is on file with the Clerk to the Board and is
available for inspection. All interested parties are invited to attend apd be heard.
NOTE: All persons wishing to speak on an agenda item must register with the
County` manager prior to presentation of y the agenda .item to be addressed.
Individual speakers will be limited to 5 minutes on any item. The selection of any
individual to speak on behalf of an organization or group is encouraged.. If
recognized. by the Chairman, a spokesperson for, a group or organization may be
allotted 10 minutes to speak on an hem.
Persons wishing to have written or graphic materials included in the Board agenda
packets must submit said material a minimum of 3 weeks prior to the respective
public hearing. In any case, written materials intended to be considered by the
Board shall be submitted to the appropriate County staff a minimum of seven days
prior to the public .hearing. All materials used in presentations before the Board
will become a permanent part of the record.
Any person who decides to appeal any decision of the Board will need a record of
the proceedings pertaining thereto and therefore, may need to ensure that a
verbatim record of the proceedings is made, which record includes the testimony
and evidence upon which the appeal is based.
