EXECUTIVE SUMMARY
Recommendation to approve Amendments and Attestation Statements with Area Agency
on Aging for Southwest Florida, Inc. for the Community Care for the Elderly and
Alzheimer's Disease Initiative programs to increase funding for FY 2013/2014 (Net Fiscal
Impact $55,356).
OBJECTIVE: To provide uninterrupted support services to Collier County Services for
Seniors' elderly clients.
CONSIDERATIONS: The Housing, Human, and Veteran Services Department's Services for
Seniors program has been providing support to Collier County's frail and elderly citizens for
over thirty -two years through the Community Care for the Elderly (CCE) and Alzheimer's
Disease Initiative (ADI) grant programs. These grants are funded by the Florida Department of
Elder Affairs (DOEA) through the Area Agency on Aging of Southwest Florida, Inc.
( "Agency "). The Area Agency on Aging has recently dropped their doing- business -as name of
Senior Choices. These grants fund services that enable clients to remain in their homes and live
with independence and dignity.
On July 24, 2012, the Board approved the current grant contract with the Agency (Agenda item
16D15). The contract has a two -year term, effective July 1, 2012 through June 30, 2014, with
two two -year renewal options.
The CCE Amendment shifts funds between allocation categories within the grant and increases
overall funding to the grant award to assist those eligible for services in our community. The
ADI program funding award has been increased to provide services to eligible clients. These
budget adjustments reflect fluctuations in the case workers' client load and client services that
routinely result from the loss or addition of clients and increase or decrease in client service
needs. Reallocation and increases of funding will allow Services for Seniors to provide case
management and services based on current and projected program needs. The effective date of
these amendments is May 27, 2014.

FISCAL IMPACT: Budget amendments are required to recognize additional grant revenue
from the Florida Department of Elder Affairs in amount of $50,300, which will reside within
Human Services Fund 707 CCE and ADI Projects 33271 and 33273. A budget amendment is
also required for the resulting CCE local match requirement of 10% in the amount of $5,056
which will reside in Human Services Match Fund 708 Project 33271. Matching funds are not
required in the ADI program.

LEGAL CONSIDERATIONS: This is a standard form amendment provided by the Area
Agency on Aging for Southwest Florida, Inc. The Area Agency on Aging of Southwest Florida
is requiring that these amendments be processed separately from the amendments related to
extending the contract term that is presented on this same agenda. This item is approved as to
form and legality and requires a majority vote. — JAB
GROWTH MANAGEMENT: There is no growth management impact associated with this
action.
RECOMMENDATIONS: That the Board of County Commissioners approves and authorizes
the Chairman to sign Amendments and Attestation Statements with Area Agency on Aging for
Southwest Florida, Inc. for the Community Care for the Elderly and Alzheimer's Disease
Initiative programs to increase funding for FY 2013/2014, and approves necessary budget
amendments.

THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida, Inc. ( "Agency ") and
Collier County Board of County Commissioners, ( "Recipient"), amends agreement CCE 203.12.
The purpose of this amendment is to transfer $16,688.00 from Case Management and Case Aide to Spending Authority
and increase Spending Authority $45,500.00, affecting Year Two, 2013 -2014 for Collier County, and revise
ATTACHMENT III EXHIBIT — I and ATTACHMENT VIII, Budget Summary.
This amendment shall be effective on May 27, 2014. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
