EXECUTIVE SUMMARY
Recommendation to approve Amendments and Attestation Statements with Area Agency on Aging
for Southwest Florida, Inc. for the Community Care for the Elderly, Alzheimer's Disease Initiative,
and Home Care for the Elderly programs to extend the term of the agreements and ensure
continuous funding for FY 2013/2014.
OBJECTIVE: To provide uninterrupted support services to Collier County Services for Seniors'
elderly clients.
CONSIDERATIONS: The Housing, Human, and Veteran Services (HHVS) Department's
Services for Seniors program has been providing support to Collier County's frail and elderly
citizens for over thirty -two years through the Community Care for the Elderly (CCE),
Alzheimer's Disease Initiative (ADI), and Home Care for the Elderly (HCE) grant programs.
These grants are funded by the Florida Department of Elder Affairs (DOER) through the Area
Agency on Aging of Southwest Florida, Inc. ( "Agency "). The Area Agency on Aging has
recently dropped their doing- business -as name of Senior Choices. These grants fund services that
enable seniors to remain in their homes and live with independence and dignity.
On July 24, 2012, the Board approved the current grant contracts with the Agency (Agenda item
16D15). Numerous amendments have been approved by the Board. The contracts have a two -
year term, effective July 1, 2012 through June 30, 2014, with two two -year renewal options. The
funding for the successor agreements for the Services for Seniors program is not yet available to
the Agency from the Florida Department of Elder Affairs. Each proposed amendment extends
the current contract to August 30, 2014, allowing the programs to continue operation until
renewals are presented. This change does not affect the overall program funding awards. When
final renewal agreements are received they will be prepared and presented to the Board.
FISCAL IMPACT: There is no new fiscal impact associated with this action. Funding resides
in Human Services Grant Fund 707 and Human Services Match Fund 708, CCE project 33271,
HCE project 33272, and ADI project 33273.
GROWTH MANAGEMENT: There is no growth management impact associated with this
action.
LEGAL CONSIDERATIONS: This is a standard form amendment provided by the Area
Agency on Aging for Southwest Florida. Area Agency on Aging of Southwest Florida is
requiring that these amendments be processed separately from the amendments related to
funding also being presented on this same agenda. Accordingly, this item has been approved for
form and legality and requires a majority vote for Board approval. — JAB
RECOMMENDATIONS: That the Board of County Commissioners approves and authorizes
the Chairman to sign Amendments and Attestation Statements with Area Agency on Aging for
Southwest Florida for the Community Care for the Elderly, Alzheimer's Disease Initiative, and
Home Care for the Elderly programs to extend the term of the agreement and ensure continuous
funding for FY 2013/2014.

referred to as the "Agency," and Collier County Board of County Commissioners, hereinafter referred to as the
'*Contractor," collectively stated as the "Parties" which amends contract CCE 203.12.
This amendment shall be effective July 1, 2014.
RECITALS:
WHEREAS, on August 9, 2012, the parties entered into the Standard Contract for social services to be
provided to Collier County's frail and elderly seniors (hereinafter referred to as "Contract "): and
WHEREAS, on July 1, 2012, the parties entered into a First Amendment to the Contract; and
WHEREAS, on July 1, 2012, the parties entered into a Second Amendment to the Contract; and
WI- IEREAS, on April 11, 2013, the parties entered into a Third Amendment to the Contract; and
WHEREAS, on June 6, 2013, the parties entered into a Fourth Amendment to the Contract; and
WHEREAS, on July 1, 2013, the parties entered into a Fifth Amendment to the Contract; and
WHEREAS, on August 8, 2013, the parties entered into a Sixth Amendment to the Contract; and
WHEREAS, still pending execution, the parties entered into a Seventh Amendment to the Contract; and
WHEREAS, still pending execution, the parties entered into an Eighth Amendment to the Contract: and
WHEREAS, Section 3 of the Contract provides that the agreement is to expire bn June 30, 2014;
and
WHEREAS, the funding source for a successor agreement for senior services is not yet available to the
Agency from the Florida Department of Elder Affairs;
WHEREAS, the Parties are satisfied with the arrangement that they currently enjoy under the Contract so
that both parties wish to extend the Contract to August 30, 2014 while they await notification of funding of a
successor contract.
NOW, THEREFORE, in consideration of the foregoing Recitals, and other good and valuable consideration, the
receipt and sufficiency of which is hereby mutually acknowledged, the Parties agree as follows:
1. The foregoing Recitals are true and correct and are incorporated by reference herein.
2. Both parties agree to extend the contract to August 30, 2014.
All other provisions of the Contract are to remain in effectt and are to be performed at the level
specified in the agreement.
