Item 16D7: Section 9.B.5 of the Subrecipient Agreement Amendment between Collier County
Housing,Human and Veteran Services and Habitat for Humanity of Collier County is revised to
include the following additional language: Pursuant to HUD letter dated January 8,2014 from
Steven A.Bales,Labor Relations Specialist,this project is not required to meet Davis Bacon
requirements unless HOME funds are utilized in the project,for 12 or more HOME assisted units
pursuant to 24 CFR 92.354. (Staff's request)
Item 16H4: Executive Summary title corrected to read: Recommendation to approve an
Interlocal Agreement(ILA)with Golden Gate Fire Control and Rescue District(GGFD) providing
that the County assume fire plans review duties on behalf of ENFD GGFD and the County's
dependent fire districts,including Isles of Capri,Ochopee and Collier Fire District One,and
authorizing the ENFD Fire Chief or his designee to act as the approving authority over fire plans
review for the County's dependent fire districts. (Staff's request)

EXECUTIVE SUMMARY
Recommendation to approve the Second Amendment to the Home Investment Partnership
Program Subrecipient Agreement with Community Assisted and Supported Living, Inc., for the
Multi-Family Property Acquisition Project,providing for project updates.
OBJECTIVE: To provide safe and affordable housing to low- and very-low-income adults with
disabilities.
CONSIDERATIONS: Collier County is the recipient of HOME Investment Partnerships (HOME)
Program funds from the United States Department of Housing and Urban Development (HUD) as
provided by the Cranston-Gonzalez National Affordable Housing Act. HOME funds may be used to
provide incentives to develop and support affordable rental housing and homeownership affordability.
On October 8, 2013, the Board approved a Subrecipient agreement with Community Assisted and
Supported Living, Inc. (CASL) for acquisition of multi-family rental housing for persons with disabilities
(Agenda item 16D10). The project will target eight low and very low income adult individuals with
disabilities. The housing facilities will consist of four two bedroom units.
On January 14, 2014,the Board approved Amendment No. 1 to the Subrecipient Agreement, allowing for
deposit of monies via wire transfer(Agenda item 16D6).
At the time the Subrecipient Agreement was written, CASL had a property under contract, but that
property was not ultimately acquired. The project specific address and minimum square footage have
been removed from the agreement, and also a time extension has been included to allow the Subrecipient
time to locate a new property to acquire. In addition,the staff contact for the agreement has changed and
project description is changing to be consistent with the project description in the approved One-Year
Action Plan.
In summary, Amendment No. 2 to the Subrecipient Agreement reflects the following changes:
1. SCOPE OF SERVICES; Project Component One: Update language to be consistent with the
project description in the approved One-Year Action Plan.
2. TIME OF PERFORMANCE; Extend end date of contract to April 30,2015.
3. AGREEMENT AMOUNT; Include Project Component Two: Environmental Review to Budget
and Deliverable tables.
4. NOTICES; update HHVS contact.
The proposed modification is allowable under the grant, and will not increase the amount of grant funds
or impact the budget for this project.
FISCAL IMPACT:The proposed actions described in this executive summary have no new fiscal
impact. The funding source for the grant award is the Home Investment Partnership(HOME)Grant. The
grant award for this project is $405,564. It is budgeted in the Urban Improvement Grants Fund(121)for
Project No. 34550,34555,34560, 34565, 34570, 44550(HOME Grants). The match of$101,391 will be
met thru the use of SHIP funds; however,the subrecipient is encouraged to provide match funds for the
HOME eligible activities. Should the subrecipient provide eligible match it will reduce the amount of
SHIP match required.

LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote
for approval.-JAB
GROWTH MANAGEMENT IMPACT: Acceptance and implementation of this amendment will
further the Goals, Objectives, and Policies within the Housing Element of the Growth Management Plan.
RECOMMENDATION: That the Board of County Commissioners approves and authorizes the
Chairman to execute the Second Amendment to the Subrecipient Agreement with Community Assisted
and Supported Living, Inc. for the Multi-Family Property Acquisition Project.

This Amendment, is entered into this day of ,2014, by and between
"RENAISSANCE Manor, Inc. d/b/a Community Assisted and Supportive Living, Inc." (CASL) a
private not-for-profit corporation existing under the laws of the State of Florida, herein after referred to
as SUBRECIPIENT and Collier County, Florida, herein after to be referred to as "COUNTY,"
collectively stated as the "Parties."
RECITALS
WHEREAS, on October 8, 2013, the COUNTY entered into an Agreement with CASL
allowing HOME Investment Partnerships (HOME) Program funds to be used for a Multi Family
Property Acquisition Project(hereinafter referred to as the"Agreement"); and
WHEREAS, on January 14, 2013, Item 16D10, the COUNTY approved the first amendment
to this agreement; and
WHEREAS,the Parties desire to amend the Agreement to include language consistent with the
Collier County Consolidated Plan One-Year Action Plan and update department contact information.
NOW, THEREFORE, in consideration of foregoing Recitals, and other good and valuable
consideration, the receipt and sufficiency of which is hereby mutually acknowledged,the Parties agree
to amend the Agreement as follows:
