EXECUTIVE SUMMARY
Recommendation to approve Amendments and Attestation Statements with Area Agency on Aging
for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida for the Community Care for
the Elderly and Alzheimer's Disease Initiative programs to ensure continuous funding for FY
2013/2014.
OBJECTIVE: To provide uninterrupted support services to Collier County Services for Seniors
frail, elderly clients.
CONSIDERATIONS: The Housing, Human, and Veteran Services' "Services for Seniors"
program has been providing support to Collier County's frail and elderly citizens for over thirty-
two years through the Community Care for the Elderly (CCE) and Alzheimer's Disease Initiative
(ADI) grant programs. Both grants are funded by the Florida Department of Elder Affairs
(DOEA) through the Area Agency on Aging of Southwest Florida, Inc., d/b/a Senior Choices of
Southwest Florida (Senior Choices). These grants fund services that enable seniors to remain in
their homes and live with independence and dignity.
On July 24, 2012, the Board approved the current grant contracts with Senior Choices (Agenda
item 16D15). The Board has approved amendments to the agreements numerous times. The
contracts have a two-year term, effective July 1, 2012 through June 30, 2014.
Amendments and Attestation Statements are needed to shift funds between allocation categories
within the grants. Within the CCE program, $6,000.00 is being moved from case management to
direct services. Within the ADI program, $1,380 is being moved from direct services to case
management. These moves reflect fluctuations in the case workers' client load that routinely
result from the loss or addition of clients. Reallocation of funds will allow Services for Seniors to
provide the case management and services based on current and projected program needs. These
shifts in funding do not affect the overall program awards. The effective date of these
amendments is February 13, 2014.
FISCAL IMPACT: The proposed actions described in this executive summary have no new
fiscal impact. The funding source for the grant awards is the Florida Department of Elder
Affairs. Budget amendments are attached to properly align funding in Human Services Grant
Fund 707 and Human Services Match Fund 708 for CCE project 33271 and ADI project 33273.
LEGAL CONSIDERATIONS: This is a standard form amendment provided by the Area Agency on
Aging for Southwest Florida, Inc. d/b/a Senior Choices of Southwest Florida. This item has been
approved for form and legality and requires a majority vote.—JAB
GROWTH MANAGEMENT: There is no growth management impact associated with this action.
RECOMMENDATIONS: That the Board of County Commissioners approves and authorizes the
Chairman to sign Amendments and Attestation Statements with Area Agency on Aging for Southwest
Florida, Inc. d/b/a Senior Choices of Southwest Florida for the Community Care for the Elderly and
Alzheimer's Disease Initiative programs to ensure continuous funding for FY 2013/2014 and approves all
related budget amendments.

THIS AMENDMENT is entered into between the Area Agency on Aging for Southwest Florida,Inc. dba Senior Choices
of Southwest Florida("Agency") and Collier County Board of County Commissioners, ("Recipient"), amends agreement
CCE 203.12.
The purpose of this amendment is to move $6,000.00 Collier County allocations from Case Management and Case Aide
to Spending Authority affecting Year Two,2013-2014,and revise ATTACHMENT VIII,Budget Summary.
This amendment shall be effective on February 13, 2014. All provisions in the agreement and any attachments thereto in
conflict with this amendment shall be and are hereby changed to conform with this amendment.
All provisions not in conflict with this amendment are still in effect and are to be performed at the level specified in the
agreement.
This amendment and all of its attachments are hereby made a part of this agreement.
