EXECUTIVE SUMMARY
Recommendation to approve Extensions of Agreement with the David Lawrence Center, NAMI of
Collier County, Inc, and the Collier County Sheriffs Office for the Criminal Justice Mental Health
Substance Abuse Program providing for time extensions and continuity of service.
OBJECTIVE: To continue to provide mental and substance abuse treatment to people in our community.
CONSIDERATIONS: The Criminal Justice Mental Health Substance Abuse (CJMHSA) program was
established by the State to provide funding to counties with which they can plan, implement, or expand
initiatives that increase public safety and avert increased spending on criminal justice. The goal is to
improve the accessibility and effectiveness of treatment services for adults and juveniles who have a
mental illness, substance abuse disorder, or co- occurring mental health and substance abuse disorders and
who are in, or at risk of entering, the criminal or juvenile justice systems.
Collier's Memorandum of Understanding (MOU) with the Department of Children and Families (DCF)
for the CJMHSA program became effective February 24, 2011 and expires February 23, 2014. On April
12, 2011, the Board approved subrecipient agreements with David Lawrence Center and NAMI of Collier
County. On May 10, 2011, the Board approved the subrecipient agreement with Collier County Sheriff's
Office. All three subrecipient agreements expire on February 23, 2014.
For months, Housing Human and Veteran Services (HHVS) have been requesting that DCF transmit an
amendment to the MOU extending funding. On February 19, 2014, the MOU was received by HHVS.
Consequently, pursuant to CMA 5330, the Division Administrator signed the amendment extending the
termination date of the MOU to June 30, 2014. DCF has no objection. The MOU is back -up to this item.
Staff has waited to present the Extensions of Agreement to the Board until it received the MOU. The
proposed Extensions of Agreement extend the expiration dates to June 30, 2014, to provide uninterrupted
services.
The subrecipients have agreed to these Extensions of Agreement, noting that these changes will allow for
uninterrupted services used by grant funds allowing more individuals to be served.
FISCAL IMPACT: The proposed actions described in this executive summary have no new fiscal
impact. Remaining funds are available within Housing Grant Fund (707) Project 33147.
GROWTH MANAGEMENT IMPACT: There is no Growth Management Impact associated with this
Executive Summary.
LEGAL CONSIDERATIONS: This item is approved for form and legality and requires a majority vote.

THIS AMENDMENT, entered into between the State of Florida, Department of Children and
Families, hereinafter referred to as the "Department" and Collier County Board of County
Commissioners hereinafter referred to as the "County ", amends Memorandum of
Understanding (MOU) # LHZ25.
Amendment Number 0001, executed 08/29/12, added reporting requirements, added a liability
responsibility, updated the County's Grant Manager's contact information, updated the
Substance Abuse and Program Office contact and the Grant Manager for the Department and
added the County's responsibility to return any unmatched grant funds, any unused advanced
grant funds, and any unapplied accrued funds.
This amendment updates the Department's Grant Manager and extends the MOU end date to
June 30, 2014.
The Department and County hereby agree to execute a subsequent amendment to make
necessary conforming adjustments to the original financial documents incorporated in
Attachment 1, pages 19 -30, specifically the Criminal Justice, Mental Health and Substance
Abuse Reinvestment Grant Budget, Budget Narrative, Commitment of Match /Donation Forms
and Match Collection Summary Report to reflect the new end date of the MOU. Any
subsequent conforming amendment shall not increase the funding level under this MOU.
1. Page 4, MOU, Item 19, Is hereby amended to read:
19. This Grant Memorandum of Understanding shall be effective on the date signed by both
parties. It shall end at midnight, Eastern time, on June 30, 2014.
2. Page 5, MOU, Item 24.d), Is hereby amended to read:

THIS EXTENSION OF AGREEMENT is entered into this day of
2014, by and between Collier County, a political subdivision of the State of Florida,
(hereinafter the "County ") and the Collier County Sherriff's Office, whose business
address is 3319 Tamiami Trail E., Naples, Florida, 34112, hereinafter called the
"Subrecipient ".
WITNESSETH
WHEREAS, on May 10, 2011, Collier County entered into an Agreement with the
Collier County Sheriff's Department for the Criminal Justice, Mental Health, and
Substance Abuse Program, hereinafter the "Agreement;" and
WHEREAS, on December 13, 2012, Collier County entered into Amendment 1 to
the Agreement with the Collier County Sheriff's Department for the Criminal Justice,
Mental Health, and Substance Abuse Program; and
WHEREAS, on November 13, 2012, Collier County entered into Amendment 2 to
the Agreement with the Collier County Sheriff's Department for the Criminal Justice,
Mental Health, and Substance Abuse Program; and
WHEREAS, the parties desire to extend the Agreement in order to fully expend
available grant funding; and
WHEREAS, as provided in Paragraph 1 "COMMENCEMENT" of Amendment 1
to the Agreement, the term of the Agreement expires February 23, 2014; and
WHEREAS, effective February 23, 2014, the Parties wish to extend the term until
June 30, 2014 to provide uninterrupted services.
NOW, THEREFORE, in consideration of the foregoing Recitals, and other good
and valuable consideration, the receipt and sufficiency of which are hereby mutually
acknowledged, the Parties agree as follows:
1. The foregoing Recitals are true and correct and are incorporated by reference herein.
2. This Extension of Agreement is effective February 23, 2014.
3. The Agreement is hereby extended to June 30, 2014,

THIS EXTENSION OF AGREEMENT is entered into this day of

Lawrence Center), authorized to do business in the State of Florida, whose business
