EXECUTIVE SUMMARY
Recommendation to approve amendment No. 1 to the Home Investment Partnership (HOME)
Program Subrecipient Agreement between Collier County and Community Assisted and Supported
Living, Inc. (CASL) in order to update the reimbursement method language and also make
necessary Fiscal Funding Year updates. Fiscal Impact$0.
OBJECTIVE: Amend the subrecipient agreement to include language allowing direct payment of
eligible expenditures via Wire Transfer and also to make necessary Fiscal Funding Year updates in order
for complete project acquisition.
CONSIDERATIONS: On October 8, 2013, Item 16D10, the Board of County Commissioners (BCC)
approved an agreement with Community Assisted and Supported Living, Inc. (CASL) for multi-family
rental housing acquisition for persons with disabilities. The project will target eight(8) low and very low
income adult individuals with disabilities. The units consist of four(4)two bedroom units.
The subrecipient agreement modification(s)is proposed as follows:
1. AGREEMENT AMOUNT; Deliverables Table, Update payment method language to allow for
wire transfer.
2. Make necessary Fiscal Funding year update on table on top of page one and Fiscal Year updates
on page two of agreement.
The proposed modification is allowable under the grant, and will not increase the amount of grant funds
or impact the budget for this project.
FISCAL IMPACT: There will be no effect on ad-valorem or general fund dollars.Funds are available
within the Housing Grant Fund(705) as outlined in the following chart of accounts:

The HOME program requires matching contributions in an amount equal to no less than 25 percent of the
total HOME funds expended for project costs. The match requirement for this project is $101,391.00.

LEGAL CONSIDERATIONS: This item is approved as to form and legality and requires majority vote

GROWTH MANAGEMENT IMPACT: Acceptance and implementation of this amendment will
further the Goals, Objectives, and Policies within the Housing Element of the Growth Management Plan.
RECOMMENDATION: That the Board of County Commissioners approves and authorizes the
Chairwoman to execute the First Amendment to the subrecipient agreement between Collier County and
Community Assisted and Supported Living, Inc. (CASL) for the Multi- Family Property Acquisition
Project.

This Amendment, is entered into this day of , 2014, by and between
"RENAISSANCE Manor, Inc. d/b/a Community Assisted and Supportive Living, Inc." a private not-
for-profit corporation existing under the laws of the State of Florida, herein after referred to as
SUBRECIPIENT and Collier County, Florida, herein after to be referred to as "COUNTY,"
collectively stated as the "Parties."
RECITALS
WHEREAS, on October 8, 2013, the COUNTY entered into an Agreement with the HOME
Investment Partnerships (HOME) Program funds to be used for a Multi Family Property Acquisition
Project(hereinafter referred to as the "Agreement"); and
WHEREAS, the Parties desire to amend the Agreement to include language allowing direct
payment of eligible expenditures via Wire Transfer.
NOW, THEREFORE, in consideration of foregoing Recitals, and other good and valuable
consideration, the receipt and sufficiency of which is hereby mutually acknowledged, the Parties agree
to amend the Agreement as follows:
